Why it's safe to take medications after your embryo transfer
A lot of IVF patients are very worried about taking any medication at all during the 2ww( two week wait). For example, if they get a cough or a cold or a diarrhea, they're not sure whether it's safe to take medicines to treat this. The reason is they're worried that these medicines will have an adverse effect on the embryo which is in their uterus. They know that some medications are dangerous during pregnancy , and are worried that these will harm their embryo. The truth is that it's safe to take any medicine you need to during your 2ww, until your HCG test is positive. This is because even though your embryo's inside y...

Trail desk at the Davis Wetlands
So I am just finally coming out of a rough patch of about three weeks with an infection or series of infections. Still have a bad cough and my brain is in a fuzz.  But I felt physically a bit better today.  As I did not want to risk spreading my cooties to people at work, I stayed away from the office and ended up scheduling four phone calls / video chats today.  Two of them were this AM and so, since I did not feel horribly bad, I decided to do the AM calls while at one of my trail desks. My "trail desk" is the term I use to refer to doing calls while outdoors.  Today I went to the Davis Wetlands since ...

One Step Forward …
I picked up particular respirator masks so that I can go outdoors again. I ’ve apparently developed asthma due to the pollution in Salt Lake Valley and the California forest fires. It’s why I’ve been so horribly sick since September. I’ve been a near complete shut-in. Alas, I overdid things yesterday. I looked like a dork in my mask, but I happily ran errands all over town. I also worked out on my treadmill, exercise bike, and did twenty minutes of aerobic activity. Unfortunately, I forgot to use both my inhaler& nebulizer. My lungs are burning again, and the cough has returned. 🙄 I feel so feeble. I ki...

Monk fruit –More than a healthy sweetener?
Because I wanted a benign and healthy way for followers of the Wheat Belly lifestyle to recreate dishes such as chocolate chip cookies, cheesecake, and pies with none of the health problems of grains or sugars, I helped Wheat-Free Market develop its Virtue Sweetener  product. Yes, you could do without such sweeteners. But I learned long ago when I introduced Wheat Belly concepts to patients in my cardiology practice that having options while entertaining friends, during holidays, and pleasing kids was important for staying on course on this lifestyle. Before I understood how to use such natural sweeteners, patients would ...

Teenager with chest pain and slightly elevated troponin. What happens then?
This is a previously healthy male teenager who was awoken by chest pain.  He was seen at another hospital and found to have a slightly elevated troponin, then underwent a CT pulmonary angiogram (PE) protocol which revealed a right sided pneumonia.  He was treated with Ceftriaxone and azithromycin.p.p1 {margin: 0.0px 0.0px 0.0px 0.0px; font: 12.0px Calibri; background-color: #fefefe}The pain is described as located in the midsternal area, radiating to the right arm, described as 8-9/10 and worse with deep inspirations.  He endorsed cough, fever, and body aches in the previous day...

Artificial Intelligence vs. Tuberculosis, Part 1
By SAURABH JHA, MD Slumdog TB No one knows who gave Rahul Roy tuberculosis. Roy’s charmed life as a successful trader involved traveling in his Mercedes C class between his apartment on the plush Nepean Sea Road in South Mumbai and offices in Bombay Stock Exchange. He cared little for Mumbai’s weather. He seldom rolled down his car windows – his ambient atmosphere, optimized for his comfort, rarely changed. Historically TB, or “consumption” as it was known, was a Bohemian malady; the chronic suffering produced a rhapsody which produced fine art. TB was fashionable in Victorian Britain, in part, because c...

Poem
GrimHow do we show courage in a time of laughter? The same way we snicker before the doctordelivers his grim practiced monologue. Before we cough blood there is a rasp of a laugh. Before we speak ill of the dead there is a tickle in the back of your throat.I know how to make her laugh;Under her arms, between her rib slats,A pantomime of my own unenviable decline.Falling face down.Everything is funny and just fine. The first wave of the army is hardened by rows of wry grins,Belying whites of eyes the enemy always mistakes for fear.12/2/19 (Source: Buckeye Surgeon)

The Numbered Pediatric Rashes Revisited
I have been seeing a lot of second disease and fifth disease—it's that time of year. School is back in session, and winter is just around the corner. The rash-numbering system for these diseases is now a historical footnote, but fifth disease is still commonly used by physicians to refer to erythema infectiosum, a parvovirus. I suspect that this system was created as a memory device for similar names and the obscure Latin terms used for these diseases. Erythema infectiosum is also easy to confuse with the many other erythema rashes such as erythema migrans, erythema marginatum, erythema toxicum, and erythema multiforme...

“I Did Not Have Sexual Relations with That Woman”
Recently, I sat across from an accomplished high school athlete who had just told me that he did not smoke and had not used any drugs. But when I asked directly about whether he vaped he openly said, “Of course!” He went on to explain that vaping isn’t smoking, isn’t unhealthy, and that THC is natural, not a drug. Teen Vaping By some reports, as many as 20 percent of high school students vape. And they often begin in middle school. Juul is the top-selling e-cigarette brand (and very cuul among kids). A single Juul pod can contain as much nicotine as a pack of 20 regular cigarettes. Nicotine harms the developing ado...

A single Juul pod can contain as much nicotine as a pack of 20 regular cigarettes. Nicotine harms the developing adolescent brain. Moreover, e-cigarette aerosol is not harmless water vapor. It’s a chemical cocktail that’s inhaled deep into the delicate lungs. We don’t yet know the long-term effects. The Tip of the Teen Vaping Iceberg CDC is in the process of investigating multiple cases of severe pulmonary disease in people who use e-cigarett...
