Lessons learned from constipation
Here’s an excerpt from Wheat Belly Total Health about constipation. As uninteresting as it can seem at first glance, constipation can offer useful insights into diet and health, but not simple-minded insights like “get more fiber.”   A condition as pedestrian as constipation serves to perfectly illustrate many of the ways in which grains mess with normal body functions, as well as just how wrong conventional “solutions” can stray, Keystone Kops of health stumbling, fumbling, and bumping into each other, but never quite putting out the fire. Drop a rock from the top of a building and it predictabl...

An E.R. Kicks the Habit of Opioids for Pain - The New York Times
Brenda Pitts sat stiffly in an emergency room cubicle, her face contorted by pain. An old shoulder injury was radiating fresh agony down to her elbow and up through her neck. She couldn't turn her head. Her right arm had fallen slack. Fast relief was a pill away — Percocet, an opioid painkiller — but Dr. Alexis LaPietra did not want to prescribe it. The drug, she explained to Mrs. Pitts, 75, might make her constipated and foggy, and could be addictive. Would Mrs. Pitts be willing to try something different? Then the doctor massaged Mrs. Pitts's neck, seeking the locus of a muscle spasm, apologizing as the pat...

MKSAP: 54-year-old man with fatigue
Test your medicine knowledge with the MKSAP challenge, in partnership with the American College of Physicians. A 54-year-old man is evaluated because of fatigue. He also notes constipation and cold intolerance. Medical history is significant for tonsillar squamous cell carcinoma treated with radiation 3 years ago. There is no family history of thyroid disorders. On physical examination, the skin is dry. Mild periorbital edema is present. The thyroid is of normal size and without nodules. Reflexes are delayed. Laboratory studies show a hemoglobin level of 11 g/dL (110 g/L), a serum sodium level of 129 mEq/L (129 mmol/L), ...

UK health IT 'glitch': Hundreds of thousands of patients have potentially been given an incorrect cardiovascular risk estimation after a major IT system error
This in the UK.What is euphemistically referred to as an "IT system error" is, in reality, the mass delivery of a grossly defective medical device adversely affecting hundreds of thousands of patients.  I'm surprised not to see that other kindly euphemism, "glitch" (http://hcrenewal.blogspot.com/search/label/glitch):http://www.pulsetoday.co.uk/your-practice/practice-topics/it/gps-told-to-review-patients-at-risk-as-it-error-miscalculates-cv-score-in-thousands/20031807.article11 May 2016  By Alex Matthews-KingHundreds of thousands of patients have potentially been given an incorrect cardiovascular risk estimation ...

Tip for those getting ready to start internship – focus on poopology
You read that right.   Hospitalized patients often develop constipation.  Almost every day on rounds, patients complain about their lack of movement.  The best interns have a systematic approach towards helping these patients. The word poop has increased in usage since this century started.  I cannot recall when it became popular, but I do know that patients do not find poop jokes vulgar or inappropriate. I often use the term poopology (probably not a real word, but perhaps it should be), and patients instantly understand and smile. So if you are an impending intern, learn about laxatives, and develop a systematic appr...

Tell Me a Real Story
by Staci Mandrola Charlotte is three and a half years old. She loves stories. I tell real stories. John tells made up stories. The first words we hear when Charlotte walks in the house are “Tell me a story, PaPa!” Stories put me on a path more than 40 years ago. The path to being a doctor and then a hospice and palliative medicine doctor. I listened to my grandmother tell stories about her physician father leaving the house to check on a woman in labor or a dying patient. He might not return for days. His payment ranged from a chicken to a milk cow to a beat up John Deere. I listened to my dermatologist father tell the...

"Desperate Turds" Print Ad Added to Gallery of Drug Advertising Mascots
Are you constipated? Well, if it's caused by your opioid pain medication, you might need prescription Movantik (read "Cam Newton Was Not the Only One 'Blocked' During Super Bowl 50").But if it's just run of the mill "occasional constipation," you might try an over-the-counter product such as Dulcolax Laxative by Boehringer Ingelheim.I recently came across a Dulcolax print ad,  which ran in Singapore newspapers and bus shelters and which was on a 2014 shortlist under the outdoor category at Lions Health (read "Pharma Advertising is So Bad It Has No Big Winner at Cannes Lions Health ...

Learn or Die!
This post originally appeared on The Timmerman Report and then ran on Venture Valkyrie. I have a new favorite TV show: Join or Die with Craig Ferguson. Ferguson is a irreverent, sometimes raunchy Scottish comedian who used to host the Late Late Show alongside his skeleton puppet sidekick Geoff.   Join or Die airs Thursdays at 11 pm on the History Channel, which is your first clue that this isn’t your run of the mill late night talk show. The premise of Join or Die, named after Ferguson’s tattoo, is that four people, a random assortment of comedians, actors, historians and scientists plus Ferguson, debate a historic q...

TBT: Which is More Uncomfortable – The Colonoscopy or Treatment for Colon Cancer?
March is Colon Cancer Awareness Month, today’s TBT post provides some helpful information on colorectal cancer. Michelle was a healthy, active 47 year old. She tried to eat right and she exercised. It looked like the hard work was paying off: no health issues and lots of energy. Her work in the healthcare field motivated her to see her doctors regularly for checkups, to get mammograms and to have her blood work done annually. She knew she was getting close to the magical age of 50 and that soon she would need to get a colonoscopy to screen for colorectal cancer.  Since she had no family history of the disease she wa...

Recommended Probiotics for Kids
Promoting healthy, diverse gut bacteria appears able to make a big difference in keeping us healthy. That’s why I often recommend probiotics for the kids in my pediatric practice. This is especially true after a course of antibiotics. Many parents are confused by the wide array of probiotics available and ask for my recommendations. There are several nice options. I suggest trying one and observing changes in y0ur child’s digestive health. If you don’t see the desired result (either stool that is too loose or continued constipation), try a different brand. Brands of Recommended Probiotics for Kids Garden of ...
