The new program of the Family and Community Medicine specialty: An opportunity not to be missed
This article proposes contributions to deepen the specific contents of the specialty in order to guide towards a more significant competency development. To carry out values-oriented training, it is suggested to deploy the values function and promote the creation of care spaces where the daily experience of those values is possible. It is proposed to establish a scale of values where the two essential values of the family physician are, in this order, a commitment to the individual person, and a commitment to the group of people under their care. Additionally, it is proposed to reorganize the competency map around five com...

Cardiovascular-related death in infancy and childhood: a clinicopathological study of two referral institutions in England
AbstractTo review post-mortem findings among deaths presenting as sudden and/or unexpected deaths in two centers in the UK during a 16-year period in order to identify those related to cardiovascular conditions. The post-mortem databases of two tertiary referral institutions were searched, and all reports were reviewed. Histological features and results of ancillary investigations were noted. All cases of sudden and/or unexpected cardiac deaths (SCD) between 2003 and 2018 were identified. The study was PRISMA compliant and approved by clinical governance. 68/1129 cases of SCD (6.0%) were identified in one center and 83/753...

The Medical Examiner: reviewing all non-coronial deaths in England and Wales
The role of Medical Examiner was originally introduced in England and Wales by the Coroners and Justice Act 2009. A key aim is to ensure that concerns about the care of the deceased by families and whistle-blowers are identified and acted upon, to avoid repeated community and hospital-based care scandals. Since 2019 all acute hospital trusts have established their own Medical Examiner Service which is tailored to local needs and systems under the guidance of the National Medical Examiner. Medical Examiners are independent, senior, registered medical practitioners whose role is to ensure accuracy of the Medical Certificate ...

P03 Leflunomide treatment for inflammatory bowel disease and intestinal failure caused by TTC7A deficiency
TTC7A deficiency Ultra-rare autosomal-recessive variants in tetratricopeptide repeat domain 7A gene (TTC7A) have been discovered in patients presenting with severe intestinal disease. Mutations in the TTC7A gene cause intestinal epithelial and immune defects resulting in apoptotic enterocolitis, multiple intestinal atresia, and recurrent intestinal stenosis. Patients face high mortality rates with palliation as the current standard of care.1 Leflunomide In 2020 a high throughput screen identified drugs that increased cell viability in patients with TTC7A; leflunomide reduced caspase 3 and 7 (responsible for cell death) a...

IJERPH, Vol. 20, Pages 2192: Evaluating the Effects of Capacity Building Initiatives and Primary Care Networks in Singapore: Outcome Harvesting of System Changes to Chronic Disease Care Delivery
ra Hildon The high tertiary healthcare utilisation in Singapore due to an ageing population and increasing chronic disease load has resulted in the establishment of primary care networks (PCNs) for private general practitioners (GPs) to provide team-based, community care for chronic diseases. A total of 22 PCN leaders and programme managers from 10 PCNs participated in online group discussions and a survey. Outcome harvesting was used to retrospectively link the intended and unintended outcomes to the programme initiatives and intermediate results (IRs). The outcomes were generated, refined and verified before shortlis...

Physician Associates (PAs) perceptions and experience in Pleural procedures
Conclusion: PAs represent a newer cadre of healthcare professionals that with appropriate training, support and clinical governance infrastructures could be invaluable in assisting in the pathways and flow of patients with pleural disease, a cohort which often have a higher burden of disease and symptoms and often truncated life expectancy. With a focussed approach including e-learning, simulation and practical workshops; PAs may play a pivotal role (Source: European Respiratory Journal)

IJERPH, Vol. 19, Pages 15653: Digital Health Care, Telemedicine, and Medicolegal Issues in Orthopedics: A Review
Biagio Moretti The use of technologies in medicine has great potential to reduce the costs of health care services by making appropriate decisions that provide timely patient care. The evolution of telemedicine poses a series of clinical and medicolegal considerations. However, only a few articles have dealt with telemedicine and orthopedics. This review assesses the ethical and medicolegal issues related to tele-orthopedics. A systematic review was performed including papers published between 2017 and 2021 focusing on the main medicolegal and clinical-governance aspects of tele-orthopedics. Most of the articles were...

Current pressure on the UK imaging workforce deters imaging research in the NHS and requires urgent attention
Medical imaging is a multidisciplinary specialty, combining clinical expertise from medical physics, radiography, and radiology, and plays a key role in patient care. Research is vital to ensure the care delivered to patients is evidence-based, and is a core component of clinical governance; however, there are pressures on the imaging workforce, which are significantly impeding imaging research. This commentary presents a research gap analysis pertaining to the multidisciplinary imaging workforce on behalf of the National Institute for Health Research (NIHR) Imaging Workforce Group. Mapping complications in thyroid surgery: statistical data are useful for medico-legal management of a recurrent safety issue
AbstractQuality of care assessment is a crucial tool for patient safety implementation. Litigation relating to thyroid surgery is one of the most represented sectors also due to the continuous increase in the number of thyroid interventions. Given the incidence of the problem, the present study aims to outline an operational methodology for risk mapping and litigation management in thyroid surgery. The study was conducted through the analysis of data collected at Umberto I General Hospital in Rome from 2007 to 2018. All thyroid surgery claims were included and, subsequently, a descriptive statistical analysis of the catego...

How should we describe complications and stratify error in the treatment of facial fractures? A systematic review of the literature
Oral and Maxillofacial (OMFS) facial fractures account for approximately 5-10 % of presentations to emergency departments in the UK. Although most trauma is treated operatively, different methods of surgery exist for the same clinical presentation and non- surgical management is in some cases appropriate. Analysis of patient morbidity is an essential component of clinical governance in surgery. OMFS units in the UK should hold regular morbidity and mortality (M&M) meetings, but no consensus exists for which cases should be discussed . Use of echocardiography in the neonatal intensive care unit
The role of echocardiography in the neonatal intensive care unit (NICU) has rapidly evolved over the last two decades and there are now guidelines to direct Neonatologist Performed Echocardiography (NPE). Expert consensus statements have provided a framework for echocardiography training, scope of practice, and clinical governance structure for neonatologists. Now NPE is routinely being used a standard of care in the NICUs across the world. It is referred to as NPE, targeted neonatal echocardiography, point of care echocardiography or functional echocardiography, depending upon the country's guidelines.
