FrostyHammer Discussion
FrostyHammer said: Let me talk about moderation- A long while back I shared an unpopular (nonpolitical) opinion on this forum. I was name-called and ridiculed to no end, including by @evilbooyaa who is a moderator. Though I don't name-call others, even moderators feel that it's acceptable to call me names (and do it themselves). If this is the type of acceptable decorum you'd like to have with your colleagues, you're not doing yourselves any favors. In the end, you people got your wish to cancel me... Click to expand... New ACGME recommendations and implications for the RO residency programs
The new ACGME rules for radiation oncology recommend a resident-to-faculty ratio of 1:1.5, meaning for 8 residents, one needs 12 full-time faculty. I know at least one program that is sizing its resident complement from 9 to 7. Are anyone aware of what other programs are doing? (Source: Student Doctor Network)

Squamous carcinoma in situ of anal canal
50+ year old woman, otherwise healthy with squamous cell CIS of the anal canal (at dentate line). Discovered incidentally on endoscopy. Just roughness on DRE and can visualize on anoscopy. Unfortunately, significant circumferential involvement and unlikely to be a candidate for surgery with both adequate margin and sphincter preservation. Doing staging workup, etc. Assuming no more advanced disease will become evident. Concurrent chemorads just seems harsh here (could treat relatively low... Read more (Source: Student Doctor Network)

anal cancer poor chemo candidate
I have an elderly patient with a T3N0 anal squam. Not a great 5FU/MMC candidate per med onc. If you were to try to twist the arm of med onc into something radiosensitizing, what would you consider? xeloda or weekly cis? I see this case series using just 5FU. Any thoughts are appreciated. MDACC Austin
Austin is not underserved. And the TXONC group is mostly freestanding, so they are cost effective. This will drive up costs and hurt the groups there. For what? Market domination? Banner, FWIW, did not win the Phoenix market. AZCCC still rules, and Ironwood and Palos Verde do very well. Early stage NSCLC SBRT/hypofrac
Wanted to review how you guys would approach this Got an elderly guy, GOLD 4 COPD, FVC 1.8L, 45% predicted, FEV1 860cc, 28% predicted, FEV1/FVC ratio 47%, with a peripheral early stage NSCLC, about 1.5cm, close to the chest wall, biopsy proven, PET negative, MRI brain negative. No matter what we did, tumor movement was 2cm+ on 4D, even after re-sim, plate/belt, education, etc. I've heard off hand that if movement >1cm you should not do SBRT, others say it is OK if it is a small target... CAR-T as a career
Hi, I haven't posted on here in a while. I'm a first year Heme/Onc fellow at a mid tier academic program. I was wondering if anyone could shed some light on what CAR-T therapy might be like as a career. Can someone start working in this field as a career straight out of fellowship or would there need to be further specialization after graduating like must I do a nonaccredited CAR-T fellowship? Would I need to focus on BMT or hematologic malignancies in fellowship? What will the demand... Read more (Source: Student Doctor Network)

SBRT vs IMRT for prostate - rad onc is all about money...
I find this video very unprofessional and erroneous in the statements made. It is very disappointing to see rad onc's again portrayed by others as motivated solely by money. Furthermore, the video recommends patients pay out of pocket for a test that has no supporting data and is not FDA approved. "Malpracticy" (Source: Student Doctor Network)

“Anonymity”
I would caution every anonymous person on this board to be extremely careful with any details of your life that can allow people to figure out who you are. There are members that have recently been called at work, and they are as anonymous as can be. I think the powers that be at ASTRO are unhappy with criticism and with their power and authority being threatened. They want to shut down discussion here. They also want to shut down voices on ROHub like mine and keep me off any possible... Read more (Source: Student Doctor Network)

My post from last Friday
This is my 1st time on the board in 4 days. Clearly, I missed a lot! Yesterday at dinner, my kids told me I should take a look at the board today. They said a lot of people do not like me 😊 (they now are SDN “lurkers” – long story there) I wanted to address some of the replies that came up from my Friday post. These were mischaracterizations of my intentions in posting the WaPo article. For some reason, a few posters decided to assume what I was trying to say. I was not trying to tell... Common Sense Oncology
Simul and I hosted Aaron Goodman and Chris Booth on The Accelerators Podcast. They created Common Sense Oncology. If you have not heard, this is a new organization seeking to instill "common sense" back in to oncology by impacting evidence creation, evidence interpretation, and evidence communication. They have a Lancet publication and a lot of information at the link. On the show, we talk about how it can be hard to be critical of mainstream oncology and some of the issues driving...
