ASTRO Town Hall Discussion (Poll % on site)
Figured this would need it's own thread. Overall I think this may have been the best discussion ASTRO has done, much better than the previous discussions and especially workforce discussions. People were allowed to write their comments in multiple chats. People could ask questions by raising their hand. The answers didn't seem avoided, although at times political. Definitely not perfect, and nothing will ever satisfy everyone. Interested to see the discussion further on SDN. (Source: Student Doctor Network)

Zap-X and SRS
I know it was discussed a while back, but it has been a while and I wonder if anyone has had recent experience using Zap-X for brain radiosurgery. Specifically, are people treating more than 2-3 brain mets, and how is the quality of the treatment planning? Would appreciate any thoughts/feedback! (Source: Student Doctor Network)

I think I ’m being bullied at my job
Hi, when I joined my job - I was supposed to be single hospital based with 3 inpatient and 2 outpatient days. We are in a way RVU based , and the amount I get paid is pretty much in line with what is fair market value in this area. We are a hospital employed group. However the structure of my job is I think extremely lopsided in terms of call and weekend coverage . For instance weekends are unequally distributed - I got more weekend days than the rest - and when I asked why I was told... Read more (Source: Student Doctor Network)

Breast RNI and IMN coverage Discussion.... Again. Breast is the worst x 4?
New breast is the worst case. Just saw a 39 yo with a BRCA mutation who had what was called a cT2N2 ER+ PR-/HER2- breast cancer. This was in the setting of a biopsied 4.6 cm primary and an MRI with 7 abnormal-appearing axillary nodes, none of which were biopsied. Got neoadjuvant chemo followed by lumpectomy and ax dissection. Final staging was ypT1aN0. Residual primary was 1.5 mm with treatment effect noted. 0/17 nodes with no treatment effect noted. For reference, an abnormal node... Adjuvant Reirradiation in H & N cancer
What is your general approach on H&N adjuvant reirradiation? There are a lot of retrospective series out there covering the field of reirradiation in H&N cancer, but I feel most publications / guidelines throw everything in a single bucket. - From true reirradiation (recurrent tumor completely in the CTV of the first treatment), to marginal misses, to recurrences outside the initial CTV (e.g. contralateral neck recurrences). - From retreatment with the goal of durable control, to... Learning in Fellowship w/o Fellows Clinic?
I’m a 3rd year IM resident who is preparing to start fellowship in July at an NCI CCC, but not an extremely prestigious one. Although my program has a lot of positives to it, it does not have a fellows clinic, even at the VA interestingly enough. I have often heard about the value of the autonomy you get in a fellows clinic. Although I’m not sure I can close the clinical gap all the way, I would like to be as well trained as possible. How should I approach preparing for an attendings clinic... Read more (Source: Student Doctor Network)

COVID - a post-mortem
Once my referring docs stop wearing masks, I will! The only problem is I have to take it off fast when I see other referring docs on the other end of the spectrum. The worst part is when I see them together and I pretend I’m drinking water. (Source: Student Doctor Network)

Change in IPSS w Hydrogel
We have our urologists place the hydrogel and fiducials, they have been doing it for many years and have done a great job. Recently have had some issues. Urologist and rep came down to talk to me couple weeks ago after one placement on a patient that was a very good SBRT candidate with IPSS of 4 when I spoke with him. they were concerned the hydrogel went subcapsular into the prostate bc they met a lot of resistance but werent certain, when they looked with ultrasound they ultimately didn’t... Why is rad onc not more competitive?
Couple years ago I was convinced that it’s because of job market concern but now i’m starting to think that the lack of exposure and medical students knowledge about the field might be the bigger reason for that. I’m not saying the job market concerns aren’t real but here’s some of the offers right now: -SOMC Portsmouth OH $668k + $90k SB -Arnot health Elmira NY $600k -Trinity Waterloo IA $550k + $75k SB -Jefferson Healthcare port townstead WA $600-800k -EOCC Richmond VA $650-750k -St...
