Efficacy of haploidentical hematopoietic stem cell transplantation compared to chemotherapy as post-remission treatment for children with intermediate-risk acute myeloid leukemia in first complete remission

The treatment option for children with intermediate-risk acute myeloid leukemia (IR-AML) in first complete remission is controversial. We retrospectively analyzed the outcomes of 80 children with IR-AML, and compared the effect of chemotherapy with haplo-HSCT as post-remission treatment. Compared with chemotherapy group, haplo-HSCT group had a significantly lower risk of relapse, especially in patients with minimal residual disease ≥10-3 after induction therapy.

Dynamics of WT1 expression during the course of chemotherapy was tested in 106 cases of intermediate and high-risk acute myeloid leukaemia in peripheral blood. High WT1 expression after 2 cycles of chemotherapy was a significant prognostic factor for OS and EFS in the whole cohort, as well as in patients treated without allogeneic stem cell transplantation (HSCT).

We report the use of fractioned gemtuzumab ozogamicin associated with high-dose chemotherapy in 18 patients. The overall response rate was 72% (13/18) including 7 complete remissions. No death was attributed to treatment toxicity.
