Nonoperative management of biliopleural fistula following living ‐donor liver transplantation: A case report
Image of the percutaneous transhepatic cholangiography demonstrating dilated intrahepatic bile ducts with stricture at the site of hepaticojejunostomy anastomosis and leakage from cut surface resulting in biliopleural fistula (white arrow). Bilio-enteric continuity was seen with free passage of injected dye to the jejunal limb during cholangiography (black arrow). Key Clinical MessageBiliopleural fistula is a rare but serious complication after liver transplantation that should be managed nonoperatively with antibiotics, pleural drainage, decompression of high-pressure biliary tract, or ultimately surgery in unresponsive c...

Is there a common definition for massive pleurisy?
Updates Surg. 2023 Nov 9. doi: 10.1007/s13304-023-01695-1. Online ahead of print.ABSTRACTThe concept of massive pleurisy (MP) is frequently used to emphasize the significance of the amount of pleural effusion. However, there are significant disagreements about it due to the lack of a universal definition for MP. In our study, we sought to elucidate these distinctions. We employed a questionnaire comprised of visual and true/false sections. In the visual section, participants were shown real-time lung radiographs and schematic drawings and asked which ones were MP. On the other hand, suggestions regarding diagnosis, treatme...

Is chest drain insertion and fibrinolysis therapy equivalent to video-assisted thoracoscopic surgery to treat children with parapneumonic effusions?
Scenario A 12-year-old boy, previously fit and well, presents to his local paediatric emergency department after 5 days of fever and a cough, and 2 days of increasing difficulty breathing. His oxygen saturation is 90% in air, and he requires 2 L of oxygen via nasal cannula. He is started on broad-spectrum antibiotics and further investigations are arranged. A chest X-ray is performed and shows a large left-sided pleural effusion. An ultrasound demonstrates the effusion is complex and loculated, with underlying lung consolidation, in keeping with a stage II parapneumonic effusion. The medical team wonders whether insertion ...

Management of Blunt Chest Trauma
Common mechanisms of blunt thoracic injury include motor vehicle collisions and falls. Chest wall injuries include rib fractures and sternal fractures; treatment involves supportive care, multimodal analgesia, and pulmonary toilet. Pneumothorax, hemothorax, and pulmonary contusions are also common and may be managed expectantly or with tube thoracostomy as indicated. Surgical treatment may be considered in select cases. Less common injury patterns include blunt trauma to the tracheobronchial tree, esophagus, diaphragm, heart, or aorta. Operative intervention is more often required to address these injuries. Bronchial fistulas complicating pneumonectomy: predictors and treatment
Conclusion: The occurrence of bronchopleural fistulae following pneumonectomy is a life threatening event. Chronic smoking, the existence of COPD and right- side pulmonary resection were significant risk factors. It can be managed either by conservative endoscopic therapy or surgical approach. Once treated swiftly, the results are very satisfactory. (Source: European Respiratory Journal)

The effect of covid-19 vaccines on pneumothorax in geriatric patients hosp İtalized in intensive care unit due to covid-19
Acta Clin Croat. 2022 Dec;61(4):655-660. doi: 10.20471/acc.2022.61.04.12.ABSTRACTIn our study, we examined the effect of COVID-19 vaccination on the incidence of pneumothorax in intensive care patients over age 65. COVID-19 intensive care patients that presented to our department between April 2020 and May 2021 during the COVID-19 pandemic were evaluated retrospectively. Patients were divided into two main groups, i.e., before and after the vaccination period. Patients were evaluated retrospectively for the following parameters: gender, age, side of pneumothorax, mortality, discharge, comorbidity, and additional pleural co...
