An acute phase reaction from zoledronate mimicking symptoms seen in opioid withdrawal: a case report
CONCLUSION: Zoledronate, well known for causing acute phase reactions, was likely the cause of withdrawal-like symptoms. Acute phase reactions with bisphosphonates mostly occur in the first infusion, and the incidence decreases with subsequent infusions. Symptoms typically occur 24-72 h post-infusion, and last at most for 72 h. Cognitive bias led the primary team to be concerned with opioid withdrawal rather than investigating other causes for the patient's presentation. Therefore, providers should thoroughly investigate potential etiologies and rule them out accordingly to provide the best care. Health care providers shou...

Outcomes of Adjunctive Therapy with Open Arthrotomy for Patients with Septic Arthritis Due to Melioidosis: A Retrospective Study
Am J Trop Med Hyg. 2024 Apr 30:tpmd230812. doi: 10.4269/ajtmh.23-0812. Online ahead of print.ABSTRACTMelioidosis is a potentially fatal infection caused by the bacterium Burkholderia pseudomallei. Septic arthritis caused by this infection is uncommon and difficult to treat. The role of adjunctive open arthrotomy in this type of infection has not yet been elucidated. We conducted a retrospective study of patients with microbiologically confirmed melioidosis between January 2002 and December 2022. Patients with a clinical condition of septic arthritis and positive cultures for B. pseudomallei were included. Comparisons were ...

The Effect of Crystal Arthropathy on the Diagnostic Criteria of Native Septic Arthritis
DISCUSSION: The presence of synovial crystals reduced the likelihood of septic arthritis and altered the laboratory diagnostic criteria. PMN% was nondiagnostic in the setting of synovial crystals.PMID:38652879 | DOI:10.5435/JAAOS-D-23-00857 (Source: The Journal of the American Academy of Orthopaedic Surgeons)

Clinical characteristics of neonatal and infant osteomyelitis and septic arthritis: a multicenter retrospective study
CONCLUSION: Distinct differences were found between neonatal and infants with osteoarticular infections. Future studies should focus on improving diagnosis and subsequent treatment regimens for younger age groups.PMID:38642591 | DOI:10.1016/j.jped.2024.03.003 (Source: Jornal de Pediatria)

Orthopedic Articular and Periarticular Joint Infections
Emerg Med Clin North Am. 2024 May;42(2):249-265. doi: 10.1016/j.emc.2024.01.002. Epub 2024 Mar 2.ABSTRACTAcute nontraumatic joint pain has an extensive differential. Emergency physicians must be adept at identifying limb and potentially life-threatening infection. Chief among these is septic arthritis. In addition to knowing how these joint infections typically present, clinicians need to be aware of host and pathogen factors that can lead to more insidious presentations and how these factors impact the interpretation of diagnostic tests.PMID:38641390 | DOI:10.1016/j.emc.2024.01.002 (Source: The Medical Clinics of North America)
