Newer Anti-epileptic Drugs.
CONCLUSIONS: Oxcarbazepine is established as effective initial monotherapy for children with partial-onset seizures. Vigabatrin is the drug of choice for infantile spasms associated with tuberous sclerosis. Lamotrigine, levetiracetam and lacosamide are good add-on drugs for patients with partial seizures. Lamotrigine may be considered as monotherapy in adolescent females with idiopathic generalized epilepsy. Levetiracetam is a good option as monotherapy for females with juvenile myoclonic epilepsy. Topiramate is a good add-on drug in patients with epileptic encephalopathies such as Lennox-Gastaut syndrome and myoclonic ast...

Get the Download on Drugs: Help Us Shatter the Myths!
National Drug Facts Week is coming, and teens all over the country will be talking. To get the conversation started, NIDA has created a booklet, Drugs: Shatter the Myths, that separates fact from fiction about drug abuse and addiction. As you’ll notice, the ‘thought’ questions below also appear at the back of the booklet. We want your opinions—so join in the discussion by leaving us a comment. It can be about one of these ideas, or your own. 1. How do you convince a friend who is using drugs that they may be at risk for addiction or other bad consequences even though they feel fine right now? 2. Knowing what we do ...

Do proton pump inhibitors cause heart attacks?
This study used a technique called “data-mining” to extract information from years of electronic medical records (EMRs) and included about 70 thousand patients in their primary analysis.  They describe the data-mining technique in the article, which seems to boil down to assigning a mathematical function to certain defined variables (patients taking PPIs) and an outcome (heart attack) to see if the two events are associated. Love ’s Labour Lost: An act of desperation leads to a bad law
There is a saying in the law that “hard cases make bad law”.   This tragic story is one of those hard cases:   Last year in June, a 17 year old girl, seven months pregnant, was told by her boyfriend, the baby ’s father, that he would leave her if she didn’t get rid of the unborn child.   So, the girl gives 21 year old Aaron Harrison $150 to beat her up and induce a miscarriage; it didn ’t work – the baby survived, was born in August and, fortunately, adopted. The girl pled no contest to a second-degree felony count of criminal solicitation to commit murder, but the charges were later dropped as ...

It is as simple as saying “Hello, my name is…”
by Christian Sinclair This weekend I was on-call for our inpatient palliative care consult service. As I reviewed the list, I knew only a handful of the patients from seeing them earlier in the week. We had three new consults and had to make a handful of follow-up visits for symptom control and goals of care discussions. Many of the people I met this weekend, I had never met before and I knew we would need to start talking about very challenging issues within in a short order of time. Thankfully I had a simple tool, that made these potentially awkward situations much easier for me, the patients, the families and staff: a ...

Nearly One-Third Of New Drugs Are No Better Than Older Drugs, And Some Are Worse
A Focus On Expediting Drug Development Congress has instructed the Food and Drug Administration (FDA) to expedite the development and review of promising new drugs through the creation of four programs: accelerated approval, fast-track, breakthrough, and priority review. In a recent Health Affairs article, James D. Chambers and colleagues reported that drugs approved under one or more expedited programs were, on average, associated with larger health gains than those approved under conventional development and review programs. They concluded that the FDA has prioritized the review of drugs that offer the largest clinical a...
