Hyperaldosteronism induces left atrial systolic and diastolic dysfunction
Patients with hypertension and hyperaldosteronism show an increased risk of stroke compared with patients with essential hypertension. Aim of the study was to assess the effects of aldosterone on left atrial function in rats as a potential contributor to thromboembolism. Osmotic mini-pumps delivering 1.5 μg aldosterone/h were implanted in rats subcutaneously (Aldo, n = 39; controls, n = 38). After 8 wk, left ventricular pressure-volume analysis of isolated working hearts was performed, and left atrial systolic and diastolic function was also assessed by atrial pressure-diameter loops. Moreover, left atrial myocytes were...

Effects of Combined Milrinone and Levosimendan Treatment on Systolic and Diastolic Function During Postischemic Myocardial Dysfunction in a Porcine Model
It is not known whether there are positive or negative interactions on ventricular function when a calcium-sensitizing inotrope is added to a phosphodiesterase inhibitor in the clinical setting of acute left ventricular (LV) dysfunction. We hypothesized that when levosimendan is added to milrinone treatment, there will be synergetic inotropic and lusitropic effects. This was tested in an anesthetized porcine postischemic global LV injury model, where ventricular pressures and volumes (conductance volumetry) were measured. A global ischemic injury was induced by repetitive left main stem coronary artery occlusions. Load-ind...

Low Dose Combination Antihypertensive Pharmacotherapy Can Be Very Effective. Angiotensin Receptor Blockers/Angiotensin Converting Enzyme Inhibitors Are Often Unnecessary
Introduction: Hypertension treatment often fails to achieve good blood pressure (BP) control, despite a wide range of available pharmacotherapy, partly because of poor side effect issues, which are often dose-related. Angiotensin receptor blockers, ARBs and angiotensin converting enzyme inhibitors, ACE-Is are known to increase BP variability, which may predispose to falls and stroke, compared to diuretics and calcium channel blockers (CCBs), which reduce BP variability.

Renin ‐Angiotensin System Inhibitors vs Other Antihypertensives in Hypertensive Blacks: A Meta‐Analysis
The purpose of this study was to assess the effects of renin‐angiotensin system (RAS) inhibitors vs other antihypertensive agents on cardiovascular outcomes in hypertensive black patients. The authors performed a systematic review and meta‐analysis of studies that compared the effects of angiotensin‐converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) with calcium channel blockers (CCBs), diuretics, and β‐blockers in hypertensive black patients on cardiovascular outcomes. A total of 38,983 patients with a mean age of 60 years and mean follow‐up of 4 years were included in our meta‐analy...

Circulating Proneurotensin Concentrations and Cardiovascular Disease Events in the Community: The Framingham Heart Study.
CONCLUSIONS: Higher concentrations of proneurotensin are associated with a greater risk of incident cardiovascular events in the community. This association did not vary according to sex, baseline low-density lipoprotein, or sortilin receptor 1 genotype. PMID: 27312221 [PubMed - as supplied by publisher]

Obesity alters molecular and functional cardiac responses to ischemia/reperfusion and glucagon-like peptide-1 receptor agonism
This study tested the hypothesis that obesity alters the cardiac response to ischemia/reperfusion and/or glucagon like peptide-1 (GLP-1) receptor activation, and that these differences are associated with alterations in the obese cardiac proteome and microRNA (miRNA) transcriptome. Ossabaw swine were fed normal chow or obesogenic diet for 6 months. Cardiac function was assessed at baseline, during a 30-minutes coronary occlusion, and during 2 hours of reperfusion in anesthetized swine treated with saline or exendin-4 for 24 hours. Cardiac biopsies were obtained from normal and ischemia/reperfusion territories. Fat-fed a...

Association of Thoracic Aorta Calcium Score With Exercise Blood Pressure Response and Clinical Outcomes in Elderly Individuals: Differential Impact of Aorta Calcification Compared With Coronary Artery Calcification Preventive Cardiology
Conclusions Aortic calcification was related to SBP response during exercise and was an independent predictor for outcomes, especially stroke, regardless of resting SBP or CACS.

The Impact and Pathophysiologic Consequences of Coronary Artery Calcium Deposition in Percutaneous Coronary Interventions.
Authors: Lee MS, Shah N Abstract The presence of coronary artery calcium is indicative of advanced coronary artery disease and is a predictor of clinical events including stroke, myocardial infarction, and cardiac arrest. Recognition of the risk factors and the clinical implications associated with coronary artery calcium is vital in identifying both preventative measures as well as treatment options. Non-invasive and invasive strategies can help quantify these calcified lesions and aid in appropriate patient selection for possible use of atheroablative devices. While mild to moderately calcified lesions can be pre...

Relationship between HRV measurements and demographic and clinical variables in a population of patients with atrial fibrillation
Abstract Little is known about the role of HRV in atrial fibrillation (AF) patients. Aim of our study was to assess the relationship between HRV measurements and demographic and clinical variables in a population of 274 AF patients. We selected all consecutive patients with persistent/permanent AF among whom had performed a Holter ECG in our Department from April 2010 to April 2015. Time-domain analysis of HRV was evaluated. Demographic and clinical variables were collected for each patient. At multivariable logistic regression, a higher pNN50 was associated with ACE inhibitors/ARBs (p = 0.016) and a lower pNN50...
