Hemoglobin A1c-related histologic characteristics of symptomatic carotid plaques
Conclusions: Increased HbA1c values seem to contribute to plaque instability through the formation of a thin fibrous cup. Thus, of the carotid artery plaque parameters including fibrous cup thickness, plaque rupture, lipid core, inflammation, intraplaque hemorrhage, thrombus, calcification, necrotic core, and neovascularization, fibrous cup thickness is the only histomorphological feature that affected by HbA1c.

Teleneurology service provided via tablet technology: 3-year outcomes and physician satisfaction.
CONCLUSION: In the assessment of neurology patients, tablets are a more cost-effective alternative to traditional telehealth technologies. The devices promote efficiency in consultations through ease of use and low transfer rates, and survey results indicate community physician satisfaction. PMID: 30825873 [PubMed - in process]

Personalized Prehospital Triage in Acute Ischemic Stroke.
Conclusions- The preferred prehospital transportation strategy for suspected stroke patients depends mainly on the likelihood of large vessel occlusion, driving times, and in-hospital workflow times. We constructed a robust model that combines these characteristics and can be used to personalize prehospital triage, especially in more remote areas. Rural Americans More Likely to Die from Preventable Causes
Blog post from the National Conference of State Legislatures celebrating National Rural Health Day and drawing attention to rural health disparities. Although the country as a whole has seen declines in four of five leading causes of death, that decline is slower in rural areas. Rural people experience higher death rates from heart disease, cancer, unintentional injury, chronic lower respiratory disease, and stroke.

Development of a 3D, networked multi-user virtual reality environment for home therapy after stroke.
CONCLUSIONS: Multi-user VR environments hold promise for home therapy, although the importance of reducing complexity of operation for the user in the VR system must be emphasized. A modified version of the VERGE system is currently being used in a home therapy study. PMID: 30290777 [PubMed - in process]

Optimal Strategies for the Diagnosis of Acute Pulmonary Embolism: Recommendations
Authors: Abstract Acute pulmonary embolism (PE) is the third most common acute cardiovascular disease, after myocardial infarction and stroke. The optimal diagnostic strategy for suspected PE among experts remains controversial, and it can differ based on factors related to the health care setting (i.e., urban, rural, or remote) that may impact access to diagnostic technologies. For instance, provision of timely diagnosis may be more challenging in rural and remote facilities due to lack of access to certain testing and imaging modalities and specialist expertise, as well as geographical barriers to care. Inabili...

When Seconds Count: One Funder's Work to Improve Stroke Outcomes in Rural America
Highlights the philanthropic work of the Leona M. and Harry B. Helmsley Charitable Trust to expand and enhance stroke care in the upper Midwest, with a specific focus on their recent work in Nebraska. Stroke mortality in rural areas can be as much as 30% higher than in urban areas, but investments in these largely-rural states with longer emergency response time work to reduce this disparity.

The neuroprotection effect of oxygen therapy: A systematic review and meta-analysis
This study reviews the oxygen therapy (normobaric oxygen [NBO] and hyperbaric oxygen [HBO]) in both stroke and traumatic brain injury (TBI) patients and meta-analyzes the efficacy of two oxygen therapies in different kinds of injuries. In stroke patients, NBO showed significant improvement in reperfusion rate while there is no favorable outcome effect of HBO treatment. In patients with TBI, HBO showed significant improvement of Glasgow outcome scale score and reduction of overall mortality while NBO may play a favorable role in improving brain metabolism.
