NobleStitch Patent Foramen Ovales Closure for Recurrent Strokes in a Patient with COVID-19 on Extracorporeal Membrane Oxygenation
SEVERE HYPOXEMIA, secondary to the SARS-COV-2 (COVID-19) pneumonia, is a significant contributor to morbidity and mortality.1 Patent foramen ovales (PFOs) can exacerbate hypoxemia through an intracardiac shunt, slowing recovery and worsening outcomes.2-4 Cryptogenic stroke is a complication commonly associated with PFOs; however, current guidelines do not address PFO management in COVID-19 patients who experience recurrent paradoxical emboli, especially in the setting of COVID-19 pneumonia, increased right-sided pressures, and a hypercoagulable state.

Outcomes of Dexmedetomidine with Local Regional Anesthesia in Carotid Endarterectomy
CONCLUSIONS: The use of dexmedetomidine in addition to LRA is a safe and acceptable alternative to conventional GETA or LRA alone in CEA with shorter length of hospital stay when compared with GETA, improved patient tolerance based on physician observation and similar rates of immediate and short-term complications, as well as post-operative pain scores.PMID:36229003 | DOI:10.1016/j.avsg.2022.08.009

A practical guide for perioperative smoking cessation
AbstractThe perioperative management of patients who are smokers presents anesthesiologists with various challenges related to respiratory, circulatory, and other clinical problems. Regarding 30-day postoperative outcomes, smokers have higher risks of mortality and complications than non-smokers, including death, pneumonia, unplanned tracheal intubation, mechanical ventilation, cardiac arrest, myocardial infarction, and stroke. Given the benefits of smoking cessation and the adverse effects of smoking on perioperative patient management, patients should quit smoking long before surgery. However, anesthesiologists cannot ad...

The response of internal carotid artery blood flow velocity to fluid challenge under general anesthesia in pediatric patients with moyamoya disease: a prospective observational study
CONCLUSIONS: The internal carotid artery blood flow velocity did not increase in pediatric patients with moyamoya disease under general anesthesia, despite fluid challenge and corresponding changes in the blood pressure and cardiac output. Intraoperative hemodynamic management to improve the cerebral blood flow in these patients requires further investigation.PMID:36164813 | DOI:10.1111/pan.14558

Can ED chest pain patients with intermediate HEART scores be managed as outpatients?
CONCLUSION: This study demonstrates that rapid access clinics are likely a safe alternative to admission for intermediate-risk chest pain patients and could reduce costly inpatient admissions for chest pain. With angiography excluded, MACE rates were well below the American College of Emergency Physicians cited 2% threshold.PMID:36129627 | DOI:10.1007/s43678-022-00355-4

Chronic Use of Angiotensin Converting Enzyme Inhibitors and/or Angiotensin Receptor Blockers is Not Associated With Stroke After Noncardiac Surgery: A Retrospective Cohort Analysis
Conclusion: We did not detect an effect of chronic ACEI/ARB use on postoperative strokes in patients who had non-neurological, noncarotid and noncardiac surgery; however, power was extremely limited.

Effect of Intensive Glucose Control on Outcomes of Hyperglycemic Stroke Patients Receiving Mechanical Thrombectomy: Secondary Analysis of the SHINE Trial
Conclusions: Intensive blood glucose control among AIS patients presenting with hyperglycemia and treated with MT was not associated with lower rates of death or higher rates of long-term favorable outcomes when compared with standard treatment.

Chinese registry of rheumatoid arthritis (CREDIT) V: sex impacts rheumatoid arthritis in Chinese patients
CONCLUSIONS: In Chinese patients with RA, men were found to have more active disease, as well as more cases of CAD and stroke. Therefore, sex should be carefully considered during the personalization of RA treatment.PMID:36103962 | DOI:10.1097/CM9.0000000000002110

Incidence and risk factors of intraoperative hyperglycemia in non-diabetic patients: a prospective observational study
Intraoperative hyperglycemia has been associated with multiple postoperative complications such as surgical site infection, myocardial infarction, stroke, and death. These complications are not confined to onl...

Opioid-Free Cardiac Surgery: A Multimodal Pain Management Strategy With a Focus on Bilateral Erector Spinae Plane Block Catheters
CARDIAC SURGERY REQUIRING STERNOTOMY presents unique challenges for perioperative and postoperative analgesia. Intraoperatively, the sympathetic response to surgical stimulation must be modulated carefully to prevent excessive bleeding, maintain the integrity of surgical repairs, and decrease the risk of stroke or iatrogenic aortic injury without compromising myocardial and systemic perfusion. This must be achieved in patients whose underlying abnormal physiology, coupled with the effects of the surgical procedure, often result in tenuous hemodynamics.

A Case of Acute Cerebral Infarction With Chief Complaints of Abdominal Pain and Bloody Diarrhoea: The Power of a Patient-Centered Inclusive Diagnostic Team
We present the case of a 66-year-old man who presented with acute abdominal pain and bloody stool as his chief complaints and was finally diagnosed with ischemic colitis from colon cancer and acute cerebral infarction. Although several cognitive biases led to physicians missing the presence of acute stroke, a diagnostic team consisting of the patient, his family members, a ward nurse, and the physician worked effectively to reach the correct diagnosis soon after admission. A physician is not the only person involved in the diagnostic process. A patient-centered diagnostic team is necessary.PMID:36046325 | PMC:PMC9418667 | ...

Male Sexual Dysfunction
Health Psychol Res. 2022 Aug 20;10(3):37533. doi: 10.52965/001c.37533. eCollection 2022.ABSTRACTMale sexual dysfunction is a series of conditions, most notably including erectile dysfunction (ED), Peyronie's disease (PD), and premature ejaculation (PE), defined by impaired sexual functioning. The prevalence of male sexual dysfunction increases with age and is relatively high with greater than 50% of men aged 40 to 70 describing some degree of erectile dysfunction. Risk factors for male sexual dysfunction include age, diabetes mellitus (DM), cancer, stroke, hypertension, penile trauma, depression, anxiety, and disturbance i...
