Blood Transfusions In Elective Cardiac Surgery- The debate continues
In this issue of the Journal of Cardiothoracic and Vascular Anesthesia, Sanaiha et al presented a manuscript entitled “Morbidity and Mortality Associated with Blood Transfusions in Elective Adult Cardiac Surgery.” The authors looked at the association between transfusions of stored packed red blood cells (pRBC) or coagulation products (CP), and the increased risk of 30 day mortality, prolonged mechanical ventil ation, acute kidney injury and stroke in patients undergoing elective cardiac surgery.

Association between risk of stroke and delirium following cardiac surgery and a new EEG index of interhemispheric similarity
Neurological complications after cardiac surgery remain a major concern despite advancements in surgical techniques and technology 1. The prevalence of stroke is reported to be as high as 6% 2, whereas post operative delirium (POD) might affect up to half of the patients in the post operative period 3.

Association Between Risk of Stroke and Delirium After Cardiac Surgery and a New Electroencephalogram Index of Interhemispheric Similarity
Neurologic complications after surgery (stroke, delirium) remain a major concern despite advancements in surgical and anesthetic techniques. The authors aimed to evaluate whether a novel index of interhemispheric similarity, the lateral interconnection ratio (LIR), between 2 prefrontal electroencephalogram (EEG) channels could be associated with stroke and delirium following cardiac surgery.

Measurement of stroke volume with echocardiography compared to gold standard cardiac magnetic resonance imaging An observational study
The assessment of Doppler-derived left ventricular outflow tract (LVOT) stroke volume (SV) is fundamental in outpatient, perioperative and critical care echocardiography.1 Calculation of SV allows assessment of the severity of aortic stenosis (AS) and evaluation of the haemodynamic state during undifferentiated hypotension.2 The use of focused transthoracic echocardiography (TTE) has appeal in a critical care environment, where after a single measurement of the LVOT area, pulsed wave Doppler velocity time integral (VTI) in the LVOT is used to calculate LVOT SV.

Pain tolerance after stroke: The Troms ø study
ConclusionsStroke survivors, with and without chronic pain, had lower cold pressor pain tolerance, with possible clinical implications for pain in this group.SignificanceWe found lower pain tolerance in participants with previous stroke compared to stroke-free participants of a large, population-based study. The association was present both in those with and without chronic pain. The results may warrant increased awareness by health professionals towards pain experienced by stroke patients in response to injuries, diseases and procedures.

Perioperative adverse cardiac events and mortality after non-cardiac surgery: a multicenter study
CONCLUSIONS: A nationwide multicenter study showed that PACE was significantly associated with increased one-year mortality. This association was stronger in high-risk surgery, older, male, and chronic kidney disease subgroups. Further studies to improve mortality associated with PACE are needed.PMID:37169362 | DOI:10.4097/kja.23043

A protocol based on Hypotension probability indicator vs. standard care to prevent intraoperative hypotension during supratentorial brain surgery: a prospective randomized pilot trial
CONCLUSIONS: In this pilot trial, the HPI-based protocol decreased the incidence of hypotension during the anesthesia maintenance but non-significant trends among secondary outcomes were also documented. Larger trials are needed to confirm our findings.PMID:37158627 | DOI:10.23736/S0375-9393.23.17197-5
