Platelet glycoprotein IIb/IIIa inhibitor tirofiban in clopidogrel-na ïve patients undergoing elective percutaneous coronary intervention
CONCLUSION: Ad-hoc PCI in clopidogrel-naïve patients who were treated with high-dose bolus of tirofiban followed by a clopidogrel loading dose immediately after the procedure appeared to be safe.PMID:37713051 | DOI:10.1007/s12471-023-01810-2

Temporal trends of case-fatality in patients undergoing dual-injection coronary chronic total occlusion recanalization
ConclusionAnnual number of CTO procedures performed in Germany increased from 2009 to 2020 with a concomitant anti-proportional decrease in the case-fatality. Our findings may help to draw more attention to predictors of in-hospital case fatality in patients hospitalized for CTO recanalization.Graphical abstract

Aircraft noise exposure and risk for recurrent cardiovascular events after acute coronary syndrome: A prospective patient cohort study
Environ Res. 2023 Sep 8:117108. doi: 10.1016/j.envres.2023.117108. Online ahead of print.ABSTRACTIn several population based cohort studies associations between aircraft noise and various diagnoses of cardiovascular disease were observed. However, no study has yet addressed the risk of recurrences in relation to transportation noise in patients with acute coronary heart disease. We conducted a prospective patient cohort study of 737 individuals recruited from eleven cardiac centers in the Rhine-Main region in the vicinity of Frankfurt Airport. All patients had an angiographically confirmed acute coronary syndrome diagnosed...

Boston Scientific Announces FDA Approval for the Latest-Generation WATCHMAN FLX ™ Pro Left Atrial Appendage Closure Device
Designed to further advance the procedural performance and safety of the WATCHMAN technology, which is indicated to reduce stroke risk in patients with non-valvular atrial fibrillation (NVAF) who need an alternative to oral anticoagulation therapy, the device now features a polymer coating, visualization markers and a broader size matrix to treat a wider range of patients.

Treatment of In-stent Restenosis of the Internal Carotid Artery Using Drug-eluting Balloons
ConclusionThe use of DEB angioplasty is a  safe and effective treatment of ICA ISR lesions, yielding significantly better results compared to other modalities. Randomized multicenter studies are warranted.

Effects of Compound Danshen Dripping Pills on Ventricular Remodeling and Cardiac Function after Acute Anterior Wall ST-Segment Elevation Myocardial Infarction (CODE-AAMI): Protocol for a Randomized Placebo-Controlled Trial
This study is designed to evaluate the efficacy and safety of CDDP in improving ventricular remodeling and cardiac function after AAMI on a larger scale.MethodsThis study is a multi-center, randomized, double-blind, placebo-controlled, parallel-group clinical trial. The total of 268 patients with AAMI after primary percutaneous coronary intervention (pPCI) will be randomly assigned 1:1 to the CDDP group (n=134) and control group (n=134) with a follow-up of 48 weeks. Both groups will be treated with standard therapy of ST-segment elevation myocardial infarction (STEMI), with the CDDP group administrating 20 tablets of CDDP ...

Intracranial haemorrhage in acute myocardial infarction: A rare but dramatic complication
The shift from thrombolysis to primary percutaneous coronary intervention (PCI) and the use of multidrug antithrombotic therapy in the management of acute myocardial infarction (AMI) have led to both a substantial improvement in outcomes and a reduction in bleeding complications [1,2]. In particular, in AMI, the rate of haemorrhagic stroke, the most dreaded haemorrhagic complication, decreased by 50% from 1998 to 2008 [1]. Despite its low incidence, intracranial haemorrhage (ICH) is often devastating, being associated with severe disability and high mortality rates.

Management of intracranial arterial stenosis during mechanical thrombectomy: Survey of neuro-interventionalists
CONCLUSION: There is notable variability in the diagnosis and management of intracranial stenosis related large vessel occlusion during mechanical thrombectomy. While most respondents felt rescue stenting was risky, the majority believed the benefit could outweigh the risk. The majority of respondents agreed that equipoise exists regarding the management of intracranial stenosis related large vessel occlusion, highlighting the need for clinical trials in this rare patient population.PMID:37606564 | DOI:10.1177/15910199231196618

Fractional Flow Reserve-Guided PCI or Coronary Bypass Surgery for 3-Vessel Coronary Artery Disease: 3-Year Follow-Up of the FAME 3 Trial
CONCLUSIONS: At 3-year follow-up, there was no difference in the incidence of the composite of death, MI, or stroke after FFR-guided PCI with current-generation drug-eluting stents compared with CABG. There was a higher incidence of MI after PCI compared with CABG, with no difference in death or stroke. These results provide contemporary data to allow improved shared decision-making between physicians and patients with 3-vessel coronary artery disease.REGISTRATION: URL: https://www.CLINICALTRIALS: gov; Unique identifier: NCT02100722.PMID:37602376 | DOI:10.1161/CIRCULATIONAHA.123.065770

O06/60 Diagnosis of intracranial atherosclerosis related large vessel occlusion before endovascular treatment
ConclusionA combination of clinical and radiological variables available before EVT can accurately predict the presence of an ICAD-LVO. The ICAD-Scale could be useful to perform a rapid assessment of underlying etiology and suggest specific pathophysiology-based measures (adjunctive pharmacological treatment, angioplasty and/or intracranial stenting).Disclosure of InterestDr Molina reported receiving personal fees from AstraZeneca for consultant services outside the submitted work.
