Stroke and Cardiovascular Outcomes in Patients With Carotid Disease Undergoing Transcatheter Aortic Valve Replacement Structural Heart Disease
Conclusions— CD is common among TAVR patients, present in 1 of 5. CD was not associated with an increased risk of stroke or mortality at 30 day or 1 year. Post-TAVR stroke seems to be because of mechanisms other than CD.

Neurological Events Following Transcatheter Aortic Valve Replacement and Their Predictors: A Report From the CoreValve Trials Structural Heart Disease
Conclusions— Predictors of early stroke after TAVR included clinical and procedural factors; predictors of later stroke were limited to patient but not anatomic characteristics. These findings indicate that further refinement of imaging to identify anatomic factors predisposing to embolization may help improve stroke prediction in patients undergoing TAVR. Insights Into Timing, Risk Factors, and Outcomes of Stroke and Transient Ischemic Attack After Transcatheter Aortic Valve Replacement in the PARTNER Trial (Placement of Aortic Transcatheter Valves) Structural Heart Disease
Conclusions— Risk of stroke or TIA is highest early after TAVR and is associated with increased 1-year mortality. Modifications of TAVR, emboli-prevention devices, and better intraprocedural pharmacological protection may mitigate this risk. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00530894.

Stroke in patients with prosthetic valve endocarditis : Single-center cohort study in China.
CONCLUSION: Stroke is a common complication in PVE that increases patient mortality. Rheumatic heart disease, double valve replacement, and fungal infection may be risk factors for patients with PVE complicated by stroke. PMID: 31076822 [PubMed - as supplied by publisher]

Stroke after Aortic Valve Surgery: Results from a Prospective Cohort.
CONCLUSIONS: Clinical stroke after AVR was more common than previously reported, more than double for this same cohort in the STS database, and silent cerebral infarctions were detected in over half of patients undergoing AVR. Clinical stroke complicating AVR is associated with increased length of stay and mortality. PMID: 24690611 [PubMed - as supplied by publisher]

Influence of Transcatheter Aortic Valve Replacement Strategy and Valve Design on Stroke After Transcatheter Aortic Valve Replacement A Meta-Analysis and Systematic Review of Literature
ConclusionsOur findings suggest that the risk of 30-day stroke after TAVR is similar between the approaches and valve types. There has been a decline in stroke risk after TAVR with improvements in valve technology, patient selection, and operator experience.

The Left Atrial Appendage: Target for Stroke Reduction in Atrial Fibrillation.
Abstract A patient with atrial fibrillation (AF) has a greater than 5% annual risk of major stroke, a 5-fold increase compared to the general population. While anticoagulation remains the standard stroke prevention strategy, the nature of lifelong anticoagulation inevitably carries an increased risk of bleeding, increased stroke during periods of interruption, increased cost, and significant lifestyle modification. Many patients with atrial fibrillation have had their left atrial appendage (LAA) ligated or excised by surgeons during cardiac surgery, a decision based largely on intuition and with no clear evidence ...

Stroke after Aortic Valve Replacement: The Known and Unknown.
Abstract Stroke following aortic valve surgery is known as a devastating complication and is associated with increased morbidity and mortality.(1) Transcatheter aortic valve replacement (TAVR) has emerged as a valid alternative for surgical aortic valve replacement (SAVR) in both inoperable patients and in those at high risk for surgery; however, the encouraging results were hampered by various complications including stroke. In the pivotal Placement of AoRTic TraNscathetER (PARTNER) trial, the initial stroke rates of TAVR patients were almost double in comparison to patients who underwent SAVR (3.8% vs. 2.1%; p=0...

Lack of Association Between Extracranial Carotid and Vertebral Artery Disease and Stroke Following Trans-catheter Aortic Valve Replacement
Conclusions The presence or absence of carotid or vertebral artery stenosis was not significantly related to the occurrence of post-TAVR stroke. Routine screening for carotid and vertebral artery disease prior to trans-catheter aortic valve replacement does not appear justified. Teaser In a retrospective cohort study, 19% (n=51) of 263 trans-catheter aortic valve replacement (TAVR) patients had CVD (> 50% stenosis of a carotid or vertebral artery). Post-TAVR stroke occurred in 7% (n=18). All strokes occurred in patients without CVD, and both 30-day mortality rates (10% vs 4%, p=0.11) and overall survival (log-rank t...

Frequency of and Determinants of Stroke After Surgical Aortic Valve Replacement in Patients With Previous Cardiac Surgery (from the Multicenter RECORD Initiative)
In conclusion, the risk of postoperative stroke after S-AVR in patients with previous cardiac surgery is high and has an impact on the immediate and late mortality. Excessive bleeding requiring blood transfusion and/or reexploration, prolonged cardiopulmonary bypass time, and use of intra-aortic balloon pump were associated with an extremely high rate of stroke.

Intravenous Recombinant Tissue Plasminogen Activator Therapy for Acute Basilar Artery Ischemic Stroke Following Transfemoral Transcatheter Aortic Valve Implantation.
Authors: Montarello NJ, Nelson AJ, Sidharta SL, Worthley SG Abstract Transcatheter aortic valve implantation (TAVI) can now be considered a standard of care for inoperable and high-risk surgical patients with severe aortic stenosis, and its uptake worldwide is rapidly increasing. Indeed, many centers performing the procedure have now moved towards treating intermediate-risk patients with TAVI rather than referring them for surgical aortic valve replacement. Although the incidence of peri-procedural acute and subacute stroke following TAVI has fallen to 2-5%, its occurrence can be life-threatening and life-changing,...

Factors associated with non –vitamin K antagonist oral anticoagulants for stroke prevention in patients with new-onset atrial fibrillation: Results from the Outcomes Registry for Better Informed Treatment of Atrial Fibrillation II (ORBIT-AF II)
Conclusions In contemporary clinical practice, up to three-fourths of patients with new-onset AF are now initially treated with a NOAC for stroke prevention. Those selected for NOAC treatment had lower stroke and bleeding risk profiles, were more likely treated by cardiologists, and had higher socioeconomic status.
