“It's not something that's really been brought up”: Opportunities and challenges for ongoing advance care planning discussions among individuals living with mechanical circulatory support
Advance care planning (ACP) among individuals living with a mechanical circulatory support (MCS) device is complex as the trajectory is typified by recurring life-threatening complications with a limited three-year survival rate of 57%.1 Additionally, 89.2% of MCS patients report experiencing complications such as gastrointestinal bleeding, stroke, and sepsis.2 –4 Against this background, in 2013, the Centers for Medicare and Medicaid Services (CMS) and the Joint Commission required MCS teams to include palliative care consultations to enhance ACP communication early in the trajectory, specifically during the evaluation for MCS surgery.

Virtual reality (VR) is emerging as a treatment modality across a variety of healthcare settings. Integrating a new technology as a form of therapy requires informed analysis to ensure that it is an effective, efficient, safe and valuable addition to the client experience. Using the Alberta Quality Matrix for Health as a framework, this detailed analysis examines the application of VR in pain management, mental health, stroke rehabilitation and palliative care. Through this multifocal lens, gaps are identified and a glimpse is provided into VR's potential for widespread adoption across healthcare settings.

Sometimes a Difficult Decision to Swallow: Ethical Dilemmas When Patients with Dysphagia who Lack Capacity Want to Eat
Mr. J is an 82-year-old retired farmer with a preexisting diagnosis of moderate vascular dementia who recently suffered a stroke resulting in a new diagnosis of dysphagia and a loss of complex decision-making capacity. He is admitted to a hospital rehabilitation unit for a course of speech, physical and occupational therapy. His wife, Mrs. J, is his surrogate and has made clear that the couple's shared goals of care are for him to regain as much function as possible so he can return to his own home.

Factors associated with discharge from hospital to residential aged care for younger people with neuropsychiatric disorders: an exploratory case-control study in New South Wales, Australia
Conclusions There are multiple intersecting and interacting pathways culminating in discharge from hospital to RAC among younger people with neuropsychiatric disorders. Improved capacity for interdisciplinary home care and alternative housing and support options for people with high support needs are required.
