Chronic Obstructive Pulmonary Disease and the Risk of Stroke: the Rotterdam Study.
CONCLUSION: Our cohort study demonstrated a higher risk of both ischemic and hemorrhagic stroke in subjects with COPD, and revealed the importance of smoking as a shared risk factor. PMID: 26414484 [PubMed - as supplied by publisher]

The estimation of excessive daytime sleepiness in post-stroke patients - the polysomnographic study
Publication date: Available online 25 May 2019Source: Respiratory Physiology & NeurobiologyAuthor(s): Helena Martynowicz, Anna Jodkowska, Robert Skomro, Paweł Gać, Agnieszka Brylka, Maciej Bladowski, Anna Wojakowska, Grzegorz Mazur, Rafał PorębaAbstractExcessive daytime sleepiness (EDS) has been reported in stroke patients. EDS in acute stroke was studied repeatedly, but there is a modest amount of data in post-stroke patients. The aim of this study was to assess the frequency of EDS and characterize sleep architecture in patients>3 months after stroke and identify factors which may affect EDS.66 patients were enrolled...

Brainstem strokes are associated with increased obstructive apnea index during sleep acutely after stroke
Sleep-disordered breathing (SDB) and ischemic stroke are intertwined pathologies. Stroke topography may influence SDB presentation. We investigated the association of stroke topography with obstructive (OAI) and central apnea indices (CAI) in acute and subacute stroke. The prospective multicentre SAS-CARE study included 204 stroke patients. Demographic, anthropometric and stroke characteristics were recorded at admission. Stroke topography (left- and right-sided, supra- and infratentorial, cortical, subcortical, insular, brainstem, cerebellar) was visually scored on the MRI at admission and binary coded. Polysomnography was...

National Trends, Risk Factors, and Outcomes of Acute In-hospital Stroke After Lung Transplantation in the United States: Analysis of the United Network for Organ Sharing Registry
Chest. 2023 Apr 11:S0012-3692(23)00503-2. doi: 10.1016/j.chest.2023.04.007. Online ahead of print.ABSTRACTBACKGROUND: Lung transplantation is the definitive treatment for end-stage lung failure. However, there have been no large, long-term studies on the impact of acute in-hospital stroke in this population.RESEARCH QUESTION: What are the trends, risk factors, and outcomes of acute stroke in patients undergoing lung transplantation in the United States?STUDY DESIGN AND METHODS: We identified adult first-time, isolated lung transplant (LTx) recipients from the United Network for Organ Sharing (UNOS) database, which comprehe...

Intracranial haemorrhage and subsequent ischemic stroke in patients with atrial fibrillation: A nationwide cohort study.
Conclusion:In this large-scale study of AF patients treated with warfarin, first-time ICH was associated with an increased rate of ischaemic stroke/SE/TIA and mortality compared to the non-ICH group. There was a decrease in the warfarin prescription purchase rate post-ICH period compared to pre-ICH, which may partly explain the excess risk. Background: The risk of ischemic stroke/thromboembolic events after an intracranial haemorrhage (ICH) in atrial fibrillation (AF) patients on warfarin treatment is poorly characterised. The aim of this study was to assess the association between the risk of ischaemic stroke/thrombo...

The impact of associated diabetic retinopathy on stroke and severe bleeding risk in diabetic patients with atrial fibrillation: The Loire Valley Atrial Fibrillation Project.
ConclusionCrude rates of stroke/TE increased in a stepwise fashion when non-diabetic AF patients were compared to diabetics with no retinopathy and diabetics with retinopathy. However, we have shown for the first time that the presence of diabetic retinopathy did not emerge as an independent predictor for stroke/TE or severe bleeding on multivariate analysis. Background: Diabetes mellitus is recognized as a stroke risk factor in atrial fibrillation (AF). Diabetics with retinopathy have an increased risk of systemic cardiovascular complications, and also, severe diabetic retinopathy predisposes to ocular bleeds. We hyp...

EAN/ERS/ESO/ESRS statement on the impact of sleep disorders on risk and outcome of stroke
Sleep disorders are highly prevalent in the general population and may be linked in a bidirectional fashion to stroke, which is one of the leading causes of morbidity and mortality. Four major scientific societies established a task force of experts in neurology, stroke, respiratory medicine, sleep medicine and methodology, to critically evaluate the evidence regarding potential links and the impact of therapy. 13 research questions were evaluated in a systematic literature search using a stepwise hierarchical approach: first, systematic reviews and meta-analyses; second, primary studies post-dating the systematic reviews/...

Stroke impact symptoms are associated with sleep-related impairment
ConclusionsOur findings suggest that ongoing stroke impact symptoms are closely related to sleep-related impairment. An intervention targeting both stroke impact symptoms and sleep-related impairment may be useful in improving neurologic recovery and quality of life in stroke survivors.

Oral anticoagulation and the risk of stroke or death in patients with atrial fibrillation and one additional stroke risk factor: the Loire Valley Atrial Fibrillation Project.
Conclusion: Among AF patients with one single additional stroke risk factor (CHA2DS2-VASc score = 1 in males, 2 in females), OAC use was associated with an improved prognosis for stroke/systemic thromboembolism/death. PMID: 26425935 [PubMed - as supplied by publisher]

Does Speech Disorder (Aphasia) Affect Respiratory Muscle Strength in Stroke?
Conclusion: It is well known that respiratory muscle strength decrease in individuals after stroke. In addition, it seems that respiratory muscle strength of aphasic patients is significantly reduced compare to non-aphasic stroke patients. However, expiratory muscle strength is similar.

Narrative review of sleep and stroke.
Authors: Pérez-Carbonell L, Bashir S Abstract Sleep disorders, such as sleep-disordered breathing (SDB), insomnia or restless legs syndrome (RLS), are common in the general population and after stroke. In some cases, sleep disturbances are pre-existing, but can also appear de novo as a direct consequence of brain damage or due to stroke-related complications. Furthermore, some sleep conditions may act as a risk factor of stroke. This review explores the available evidence of the two-way relationship between sleep and stroke. Cardiovascular physiological changes during sleep are described, as well as the evidence o...
