Medical nutrition therapy of stroke patients with dysphagia – 2021
Orv Hetil.

Impact of organizational innovation in stroke care on post-acute phase health expenses in France
ConclusionsPost-stroke expenditure, the first year after stroke is mainly made up of rehabilitation expenses and depends on the type and intensity of the rehabilitation. Concerning inital care in neurovascular unit, it allows for better quality of life.Key messagesPost-stroke expenditure, the first year after stroke is mainly made up of rehabilitation expenses and depends on the type and intensity of the rehabilitation.Inital care in neurovascular unit, it allows for better quality of life.

Stroke as an Antecedent Risk Factor for Depression among Post-Menopausal Women
ConclusionsA significant association between previous stroke and current depression was found in this population, independent of vascular disease risk measures among post-menopausal women. These findings highlight the possible importance of screening for and treating depression among stroke survivors. Longitudinal studies need to be conducted assessing the mental health impact of stroke.Key messagesOccurrence of stroke may lead to an increased likelihood of depression, especially among post-menopausal women.Due to the global COVID-19 pandemic, surveillance efforts should include mental health complication including those related to stroke.

The effect of time spent in rehabilitation on activity limitation and impairment after stroke
CONCLUSIONS: An increase in time spent in the same type of rehabilitation after stroke results in little to no difference in meaningful activities such as activities of daily living and activities of the upper and lower limb but a small benefit in measures of motor impairment (low- to very low-certainty evidence for all findings). If the increase in time spent in rehabilitation exceeds a threshold, this may lead to improved outcomes. There is currently insufficient evidence to recommend a minimum beneficial daily amount in clinical practice. The findings of this study are limited by a lack of studies with a significant con...

Supported self-management in community stroke rehabilitation: what is it and how does it work? A protocol for a realist evaluation study
Introduction A growing evidence base demonstrates the effectiveness of supported self-management in stroke for stroke survivors and their families. However, there is significant variation in its implementation in community stroke care and little understanding about how supported self-management works and is delivered across different settings, models used and contexts of community stroke rehabilitation. Methods and analysis Using a mixed method, realist approach across two phases, this protocol describes a study on community-based supported self-management. The aim is to identify the mechanisms and outcomes of supported s...

Hyperacute stroke thrombolysis via telemedicine: a multicentre study of performance, safety and clinical efficacy
Conclusions The results from this large hyperacute telestroke cohort indicate two important points for clinical practice. First, telemedicine via a hubless horizontal model provides a clinically effective and safe method for delivering hyperacute stroke thrombolysis. Second, improved door-to-needle times were offset by a concerning rise in prehospital timings. These findings indicate that although telemedicine may benefit in-hospital hyperacute stroke care, improvements across the whole stroke pathway are essential.

Detailed risks and characteristics of postepilepsy stroke in non-traumatic adult-onset epilepsy
CONCLUSION: Since the increased risk of stroke in epilepsy cohort peaked immediately after epilepsy diagnosis, early implementation of prevention strategies is considered.PMID:35484004 | DOI:10.1016/j.jfma.2022.04.003

Stroke prevention of thoracoscopic left atrial appendage clipping in patients with non-valvular atrial fibrillation at high risk of stroke and bleeding: study protocol for a non-randomised controlled clinical trial
Introduction Non-valvular atrial fibrillation (NVAF) is a high-risk factor for ischaemic stroke. The 2016 European Society of Cardiology Atrial Fibrillation Management guidelines recommend oral anticoagulants (OACs) to prevent stroke in men with CHA2DS2-VASc scores ≥2 and women ≥3. However, in patients with a high risk of stroke and a high risk of bleeding (HAS-BLED (Hypertension, Abnormal renal/liver function, Stroke, Bleeding history or predisposition, Labile international normalized ratio, Elderly (> 65 years), Drugs/alcohol concomitantly) score≥3), OAC had a higher risk of bleeding. Left atrial appendage cl...

Exploring views on long term rehabilitation for people with stroke in a developing country: findings from focus group discussions
Conclusions: Barriers to the provision of long term stroke rehabilitation services are multi-factorial. Establishment of community-based stroke rehabilitation centres and training family members to conduct home-based therapy are two potential strategies to enable the continuation of rehabilitation for long term stroke survivors.
