Outcomes of descending and thoracoabdominal aortic repair in connective tissue disorder patients
CONCLUSIONS: In dedicated units, open surgical DTAA and TAAA repair in patients with CTD can be performed with a very low risk of death, severe complications and, late re-intervention. For CTD patients with reasonable risk, OSR should remain the first line of treatment.PMID:36151718 | DOI:10.1080/14017431.2022.2125174

Elephant trunk simplifies thoracoabdominal aortic aneurysm repair without impacting operative risk
ConclusionsExtent I and II TAAA repair after ET can be safely performed in a tertiary referral center with shorter bypass and cross-clamp times. ET eliminates the need for circulatory arrest or clamping a hostile arch.

Distal Aortic Progression After Hemi- Zones 1-3 Arch Replacement in Acute Type A Aortic Dissection
CONCLUSIONS: There was no significant difference in long-term survival, distal aorta growth, or reoperation rate for distal aortic aneurysm after Hemiarch or Zones 1-3 arch replacement. Patient-specific arch replacement strategies may be utilized rather than defaulting to aggressive arch replacement for all ATAAD patients.PMID:36368349 | DOI:10.1016/j.athoracsur.2022.10.035

The Contemporary Impact of Body Mass Index on Open Aortic Aneurysm Repair
The Centers for Disease Control and Prevention (CDC) has deemed obesity a national epidemic and contributor to other leading causes of death including heart disease, stroke, and diabetes. Accordingly, the role of body mass index (BMI) and its impact on surgical outcomes has been a focus of persistent investigation. The purpose of this study was to quantify the effect of BMI on open abdominal aortic aneurysm repair (oAAA) outcomes in contemporary practice.

Using The Vascular Quality Initiative To Show Upper Extremity Access For Complex Aortic Aneurysm Repairs Is Associated With Worse Perioperative Outcomes
This study compares outcomes after PMEG and CMD in a national database between transfemoral only (TTF) access and mixed upper extremity (UEM) access.

Use of Secondary Iliac Branch Devices after Previous Endovascular Abdominal and Thoraco-Abdominal Aortic Aneurysm Repair < sup > † < /sup >
CONCLUSION: Secondary IBD after EVAR is a safe and effective procedure with high technical success and low complication rates. The technique of choice to revascularise the IIA seems to not affect early and follow up results. Long term durability of IBD repair is acceptable with low rates of IIA re-interventions.PMID:36707020 | DOI:10.1016/j.ejvs.2023.01.033

The effect of adding an open distal anastomosis to proximal aneurysm repairs in bicuspid aortopathy
ConclusionWe found no significant inter-group differences in survival, reintervention, or in-hospital complication rates, with low rates of mortality, and aortic arch-related reintervention, suggesting adding open distal anastomosis may not provide benefit in BAV patients undergoing proximal aortic aneurysm repairs.

Activated Clotting Time Guided Heparinisation During Open Abdominal Aortic Aneurysm Repair (ACTION-1) - Rationale and Design of a Randomised Trial
Unfractionated heparin has been used by vascular surgeons worldwide for more than 70 years during non-cardiac arterial procedures (NCAP) to prevent clot formation and reduce thromboembolic complications (TEC), for example, arterial thrombosis, peripheral emboli, myocardial infarction, colonic ischaemia, deep venous thrombosis, and stroke.1,2 In this Research Letter, shortcomings and high variation in current intra-operative heparin use, the need for scientific evidence, and the design of a randomised controlled trial (RCT) are discussed.

Comparison of Unibody and Non-Unibody Endografts for Abdominal Aortic Aneurysm Repair in Medicare Beneficiaries: The SAFE-AAA Study
CONCLUSIONS: In the SAFE-AAA Study, unibody endografts failed to meet noninferiority compared with non-unibody endografts with respect to aortic reintervention, rupture, and mortality. These data support the urgency of instituting a prospective longitudinal surveillance program for monitoring safety events related to aortic stent grafts.PMID:36866664 | DOI:10.1161/CIRCULATIONAHA.122.062123

Outcomes of endovascular repair of infrarenal penetrating aortic ulcers
Conclusions: According to this registry data, endovascular repair of PAU is technically feasible with acceptable perioperative outcomes, but further studies investigating mid- and long-term data are needed before invasive treatment of PAU disease in an elderly and comorbid patient population should be recommended.PMID:36880201 | DOI:10.1024/0301-1526/a001060
