Acute Ischemic Stroke in Pregnancy
Stroke is an uncommon but serious potential complication of pregnancy. The management of acute ischemic stroke in pregnant women remains a complex challenge that extends beyond the limits of clinical trial evidence. Patient 1 was a 29-year-old woman 27 weeks into her first pregnancy, without remarkable past medical history or vascular risk factors. She was admitted 1 h after sudden onset of a left total anterior circulation syndrome (National Institute of Health Stroke Scale [NIHSS] score of 23). CT and angio-CT scans were normal. Thrombolysis was performed, with mild clinical improvement. Brain MRI showed multi-territoria...

Neonatal Forearm Compartment Syndrome: Look for Cerebral Stroke
A male term newborn presented with a neonatal forearm compartment syndrome together with an ipsilateral cerebral stroke. Pregnancy and delivery were uneventful, except for oligohydramnios. His mother had celiac disease and hypothyroidism. Since birth, he showed a 4-cm bullous-ulcerated lesion on the left volar forearm and elbow fold (, A), with normal pulses, and an ipsilateral upper limb paresis with reduced motility, dropped wrist, no fingers extension, and grasping. Tendon reflexes were normal with Babinski sign bilaterally evident. Radiography showed a mild carpus asymmetry (left left). He had a residual scar on the fo...

Annelizabeth ’s story: Care that feels like home, close to home
When you’re 5, it’s nice to have a place that feels like a second home. Where there are lots of hugs. And songs. And games. And you can curl up and watch “Frozen,” your favorite movie. For Annelizabeth Jean-Baptiste, a spunky Waltham kindergartener, that place is Boston Children’s Hospital at Waltham. Annelizabeth, or Annie (but never Anna, she says), first came to Boston Children’s at Waltham two weeks after she was born. Her mother Elcie wasn’t expecting that her fourth child would need special care. “It was a difficult pregnancy. I was very excited and relieved when she was born.” But that sense of rel...

The Association of Arsenic Metabolism with Cancer, Cardiovascular Disease, and Diabetes: A Systematic Review of the Epidemiological Evidence
Conclusions: Population level of iAs% and DMA%, but not MMA%, were associated with arsenic exposure levels. Overall, study findings suggest that higher MMA% was associated with an increased risk of cancer and cardiovascular disease, while lower MMA% was associated with an increased risk of diabetes and metabolic syndrome. Additional population-based studies and experimental studies are needed to further evaluate and understand the role of arsenic exposure in arsenic metabolism and the role of arsenic metabolism in disease development. https://doi.org/10.1289/EHP577 Received: 01 June 2016 Revised: 26 February 2017 Acce...

Influence of maternal characteristics and gestational age on haemodynamic indices: NICOM device ‐specific reference ranges
ConclusionsThis study presents device ‐specific reference ranges for stroke volume, cardiac output and systemic vascular resistance for the NICOM® device in pregnancy and describes the maternal characteristics that are associated with the values of these haemodynamic measurements. Studies using NICOM® in pregnancy can use these ranges in order to evaluate observations relative to those expected in uncomplicated pregnancies conditional on maternal characteristics. This article is protected by copyright. Preterm delivery and elevated maternal total vascular resistances (a prelude to the future maternal cardiovascular risk?)
ConclusionsWomen with diagnosis of TPD showing TVR values >1000 dynes.sec.cm−5 and elevated CRP levels are at high risk of preterm delivery. An impaired maternal cardiovascular adaptation during pregnancy in these patients might suggest a possible higher risk for subsequent future cardiovascular disease.

Preterm delivery and elevated maternal total vascular resistance: signs of suboptimal cardiovascular adaptation to pregnancy?
ConclusionsWomen with a diagnosis of TPD showing TVR values of > 1000 dynes × s/cm5 and elevated levels of CRP are at high risk of preterm delivery. Fetal Alcohol Exposure Alters Blood Flow and Neurological Responses to Transient Cerebral Ischemia in Adult Mice.
CONCLUSIONS: Our data collectively show that PAE adult mice exhibit a persistent, long-term loss of cranially directed blood flow, and decreased capacity to compensate for brain trauma due to acute-onset adult diseases like ischemic stroke. PMID: 27987329 [PubMed - as supplied by publisher]

Impaired Maternal Hemodynamics in Morbidly Obese Women: A Case –control Study
ConclusionThe findings of our study suggest that cardiac function is significantly altered in morbidly obese pregnant women. In order to make appropriate comparisons between individuals, it is imperative that hemodynamic parameters are indexed for BSA – as indeed they are in pediatric cardiology. The novel finding of a reduced CI in morbidly obese pregnant women may explain the predisposition to preeclampsia and other adverse outcomes in this population and warrants further investigation.

Effect of change in posture on maternal functional hemodynamics at 35 –37 weeks’ gestation
ConclusionsParadoxically, in late third trimester normal pregnancies both change from the supine to the sitting position and passive leg raising may result in increase in preload with consequent increase in cardiac index and stroke volume index and decrease in total peripheral resistance index. In pregnancies that develop PE or GH the effects of postural change on cardiovascular parameters are similar but less marked than in normotensive pregnancies.
