265 Use of support vector machine to predict discharge destination on an acute stroke ward
Conclusion From data that is available from simple assessments done in routine clinical pratice it is possible to identify patients with acute stroke for whom discharge home or inpatient rehabilitation is highly likely. Collaborations between clinicians and data scientists may help to maximise the value of clinical assessment tools that are already in widespread use. Tom.Hughes2@wales.nhs.uk

198 Should we thrombolyse cognitively impaired patients with acute ischaemic stroke?
Conclusion Confounding variables were the main predictors of poorer thrombolysis outcomes in cogni- tively impaired stroke patients. A large portion of these poorer outcomes remains unexplained, leaving potential for further investigation.

Service provision for cerebral venous sinus thrombosis. a uk survey
Conclusion Provision of care for patients with CVST is mainly through specialist services and is evenly split between acute stroke and neurology. Most cases are cared for by a neurologist or stroke physician, but just over half would see a specialist within 24 hours of admission.

Late-onset epilepsy predicts future stroke: a systematic review and meta-analysis
Late-onset epilepsy (LOE) is closely associated with cerebrovascular disease. LOE also appears to be a harbinger of dementia. We performed a systematic review of observational studies, clinical studies and radiological studies relating to LOE. We conducted a meta-analysis of 5 observational studies and have found that patients presenting with LOE experience an increased risk of subsequent stroke, weighted OR 3.88 (95% CI 2.76 - 5.46). Above the age of fifty, incidence of new onset epilepsy is approximately 400/100,000 person years. Of these, 9.8–11.9% will have a stroke in the decade following epilepsy onset, transla...

206 Stroke and sociodemographic factors: a review
Discussion Certain areas within stroke medicine are well researched, with cause and effect relationships established. However, there are certain protected patient characteristics with minimal/no evidence which require further research to ensure equal access for all patients to stroke care.

Vagus nerve stimulation paired with rehabilitation for motor function, mental health and activities of daily living after stroke: a systematic review and meta-analysis
Conclusion The results suggest VNS+Rehab showed better motor function outcomes in patients after stroke, while no better than Rehab on mental health or ADL. Combinations of phase of stroke, specific parameters of VNS and VNS intervention frequency are key modulators of VNS effects. Trial registration number CRD42022310194

Factors associated with early outcome in patients with large-vessel carotid strokes
Conclusions Strokes due to large vessel disease in the carotid artery are in general of mild severity and have a high rate of END. The degree of stenosis has a clear association with higher severity and END risk.

The safety of aeroplane travel in patients with symptomatic carotid occlusion
Conclusions The risk of stroke as a consequence of air travel is low, even in a cohort of patients at high risk for future stroke owing to haemodynamic impairment. These patients with symptomatic carotid occlusion should not be discouraged from air travel.

Metacognitive function poststroke: a review of definition and assessment
Metacognition is the conscious knowledge individuals have about their own cognitive capacities and the regulation of these activities through self-monitoring. The aim of this review was to identify the definitions and assessment tools used to examine metacognition in relation to stroke studies. A computer database search was conducted using MEDLINE, CINAHL, PsycINFO, Cochrane Reviews, Scopus and Web of Science. A total of 1412 publications were retrieved from the initial database search. Following the removal of unrelated articles, 34 articles remained eligible. 5 studies examined metacognition in relation to cognitive and...

Outcomes of stent retriever thrombectomy in basilar artery occlusion: an observational study and systematic review
Conclusions Stent retriever thrombectomy is a safe treatment modality for patients with stroke presenting with BAO. Although the stent retrievers showed a good recanalisation rate, there are currently no randomised clinical trials to assess its clinical efficacy in comparison with the reference treatment.

The use of smartphone applications in patients attending tia clinic
Ten major risk factors account for 90% of the population attributable stroke risk. Smartphone applications may be used for education and self-managing risk factors. We aimed to explore use and attitudes towards smartphones in a TIA clinic. Methods From September 2015 consecutive patients attending University College Hospitals, London TIA clinic completed a questionnaire, relating to, stroke risk factors and use of smartphones. A likert scale (strongly disagree=1, strongly agree=10) evaluated attitudes towards Smartphone applications in stroke education and management. Results 118 patients completed the questionnaire. Mean...

Revolution in stroke
Video abstract In this special edition of Journal of Neurology, Neurosurgery, and Psychiatry, we highlight recent advances in stroke management. In 2015, a series of transformative neurointerventional trials established the benefit of mechanical thrombectomy in large vessel occlusion, dramatically changing the management of acute stroke and altering the natural history of this devastating stroke subtype. This breakthrough in science has forced a redesign of stroke care and reinvigorated research in the prevention and management of one of the leading causes of death and disability around the globe. One of the most dramatic ...

Increased thrombin generation potential in symptomatic versus asymptomatic moderate or severe carotid stenosis and relationship with cerebral microemboli
Discussion Thrombin generation potential is greater in patients with recently symptomatic than asymptomatic carotid stenosis, and decreases over time following TIA or stroke associated with carotid stenosis. These data improve our understanding of the haemostatic/thrombotic biomarker profile in moderate-severe carotid stenosis.
