Next generation sequencing in familial cerebral small vessel disease - an ongoing study
Cerebral small vessel disease (SVD) is the most common form of stroke and vascular dementia. CADASIL (notch3 mutations) is most frequent but other monogenic causes more recently identified include CARASIL (HTRA1 gene), RVCL (TREX1 gene) and COL4A1 and 2. Diagnostic tests for these are often inaccessible and expensive and there are families with clinical monogenic SVD in whom no known variants are detected. Next generation sequencing offers the potential to screen for these diseases, which present with similar phenotypes, more cost-effectively and rapidly in a single test. Safety and efficacy of eslicarbazepine acetate in elderly patients

Delayed diagnosis of cadasil in a patient with hemiplegic migraine
Conclusion prevalence of CADASIL may be underestimated partly due to underdiagnoses, especially in patients presenting with migraine. Every attempt to gain an appropriate family history should be made to avoid misdiagnosis.

Extensive cerebral microbleeds predict parenchymal haemorrhage and poor outcome after intravenous thrombolysis
Conclusions The presence of extensive (≥3) CMBs increased the risk of PH 24 h after intravenous thrombolysis, and predicted poor clinical outcome independently.

Higher mortality in patients with right hemispheric intracerebral haemorrhage: INTERACT1 and 2
Conclusions Right hemispheric lesion was associated with increased risk of death in patients with acute ICH. The laterality of the ICH does not appear to affect the level of disability in survivors. Cerebrovascular injury as a risk factor for amyotrophic lateral sclerosis
Conclusions Cerebrovascular injury from a variety of causes, rather than the presence of AVM or the associated embolisation procedure per se, may be a risk factor for ALS within the context of a more complex multiple-hit model of pathogenesis.

Rate of perihaematomal oedema expansion is associated with poor clinical outcomes in intracerebral haemorrhage
Conclusions Rate of PHE growth over 72 hours was an independent predictor of mortality and poor functional outcomes following ICH. Baseline haematoma volume and gender appear to influence PHE growth.

Setting up an undergraduate neurology teaching day: 7 steps
The University of Bristol medicine course has one of the shortest neurology placements nationally, just one week. This is despite stroke being the 2nd most common cause of death and neurological illness accounting for 20% of acute medical admissions. Neurology training is set to be reduced even further by GMC regulations limiting clinical placements to the first four years with the final year as a "student assistantship". Assessment of platelet reactivity with cerebral microemboli in carotid stenosis
Discussion Reduction in HTPR prevalence in symptomatic patients over time may reflect successful carotid intervention and/or acute phase response resolution. Larger studies should confirm whether therapeutic intervention(s) may reduce HTPR in symptomatic patients to enhance prevention.

Using asl mri to characterise decline in cerebral blood flow in cadasil
Conclusion CBF in CADASIL patients declines faster than is reported for healthy individuals. This is the first study to show ASL is a feasible method to study CBF in CADASIL and has demonstrated changes over the time period which may be suitable for a clinical trial.

Neck vessel dissection following whiplash-type neck injury
Arterial dissection is a recognised complication of neck manipulation and trauma. In a systematic audit of 148 complex medicolegal whiplash cases 131 had only neck pain, 8 had radiculopathic symptoms, and 9 had symptoms and signs suggesting an arterial dissection of a neck vessel with presumed secondary thromboembolism. Their ages ranged from 28–55. Three had hemipareses supported by scan abnormalities, and one had a probable posterior fossa stroke. Five cases had prolonged symptoms without imaging abnormalities. Case presentation: the dancing man from sierra leone
Discussion Making this diagnosis is challenging, as brain imaging often shows non specific changes and blood results are usually normal. CSF may reveal raised proteins and WBCs. A high index of suspicion is required to arrange a brain biopsy as this confirms the diagnosis.
