Door-to-Imaging Time for Acute Stroke Patients Is Adversely Affected by Emergency Department Crowding Clinical Sciences
Conclusions—ED crowding is associated with reduced odds of meeting DIT goals for acute stroke. In addition to improving stroke-specific processes of care, efforts to reduce ED overcrowding should be considered central to optimizing the timeliness of acute stroke care.

An EHR Template and Housestaff Incentive Program to Improve the Frequency of NIHSS Documentation for Code Stroke (P6.047)
Conclusions: At an academic center with housestaff-led Code Stroke teams, the proportion of Code Stroke activations with a documented NIHSS was higher after implementing an EHR note template and housestaff financial incentive.Disclosure: Dr. Jia has nothing to disclose. Electronic Stroke CarePath: Integrated Approach to Stroke Care Innovations in Care
We describe the development, implementation, and outcomes of the first 2 years of the Electronic Stroke CarePath, an initiative developed for management of ischemic stroke patients in an effort to improve efficiency and quality of care for patients. The CarePath consists of care pathways for ischemic stroke that are integrated within the electronic health record. Patient-reported outcomes are collected using an external software platform. Documentation tools, order sets, and clinical decision support were designed to improve efficiency, optimize process measure adherence, and produce clinical data as a byproduct of care th...

Factors associated with perforator stroke after selective basilar artery angioplasty or stenting
Conclusions Diabetes, time from last symptom to procedure, and pre-procedure stenosis percentage may be factors affecting the risk of perforator stroke associated with basilar artery angioplasty and/or stenting. These factors should be considered in planning of potential basilar artery angioplasty and/or stenting and prospectively evaluated in future multicenter trials.

CYP2C19 Loss-of-Function Associated with First-Time Ischemic Stroke in Non-surgical Asymptomatic Carotid Artery Stenosis During Clopidogrel Therapy
This study measures effect ofCYP2C19 genotype on ischemic stroke risk during clopidogrel therapy for asymptomatic, extracranial carotid stenosis patients. Using deidentified electronic health records, patients were selected for retrospective cohort using administrative code for carotid stenosis, availability ofCYP2C19 genotype result, clopidogrel exposure, and established patient care. Patients with intracranial atherosclerosis, aneurysm, arteriovenous malformation, prior ischemic stroke, or observation time<1 month were excluded. Dual antiplatelet therapy patients were included. Patients with carotid endarterectomy or ...

Palliative Care Consultation Did Not Reduce Length of Stay in a Comprehensive Stroke Center (P1.212)
Conclusion: Palliative care consultation for patients with ischemic or hemorrhagic strokes was not associated with decreased hospital costs or LoS. This is in contrast to previously reported studies. We will further analyze the effect of early palliative care on a larger sample size to validate the findings. We also propose that a standardized palliative care trigger tool be implemented for early PC consultation in patients with debilitating stroke, advanced age, and multiple comorbid conditions.Disclosure: Dr. Tversky has nothing to disclose. Comment: Does L-thyroxine prevent or cause stroke in hypothyroidism?
The association between overt hypothyroidism and atherosclerotic risk factors, especially hypertension and dyslipidemia, is clear. To date, only a few small and methodologically flawed epidemiologic studies have investigated the relationship between hypothyroidism and stroke, precluding definitive conclusions. In the current issue of Neurology®, Karch and Thomas1 present a large well-designed case-control study investigating this relationship. They compared 34,907 patients with autoimmune thyroiditis on treatment with l-thyroxine and 149,632 matched individuals without autoimmune thyroiditis from a UK primary care elec...

Recruiting Control Participants into Stroke Biomarker Studies
AbstractThe number of scientists using –omics technologies to investigate biomarkers with the potential to gauge risk and aid in the diagnosis, treatment, and prognosis of stroke continues to rise, yet there are few resources to aid investigators in recruiting control participants. In this review, we describe two major strategies to ma tch control participants to a stroke cohort-propensity score matching and one-to-one matching—including statistical approaches to gauge the balance between groups. We then explore the advantages and disadvantages of traditional recruitment methods including approaching spouses of enrolle...

Automating Ischemic Stroke Subtype Classification Using Machine Learning and Natural Language Processing
This study sought to use natural language processing of electronic health records (EHR) combined with machine learning methods to automate IS subtyping. Methods: Among IS patients from an observational registry with TOAST subtyping adjudicated by board-certified vascular neurologists, we analyzed unstructured text-based EHR data including neurology progress notes and neuroradiology reports using natural language processing.

Obesity and mortality after the first ischemic stroke: Is obesity paradox real?
ConclusionOur study results support the obesity paradox in ischemic stroke patients as shown by a significantly decreased hazard ratio for one-year mortality among overweight and obese patients in comparison to non-overweight patients.

Application of Machine Learning Techniques to Identify Data Reliability and Factors Affecting Outcome After Stroke Using Electronic Administrative Records
Conclusion: Electronic administrative records from this cohort produced reliable outcome prediction and identified clinically appropriate factors negatively impacting most outcome variables following hospital admission with stroke. This presents a means of future identification of modifiable factors associated with patient discharge destination. This may potentially aid in patient selection for certain interventions and aid in better patient and clinician education regarding expected discharge outcomes.

Ischemic Stroke in Young Adults: Profile of SARAH Hospital Brasília From 2008 to 2012
Conclusions: Etiological diagnosis of stroke in the young is challenging for clinicians. Hence, a more effective classification scheme, better investigative mechanisms, and correct determination of causal associations in ischemic stroke are needed. Thrombophilia screening should be performed in the presence of relevant clinical signs and/or family history.

Impact of renal function on ischemic stroke and major bleeding rates in nonvalvular atrial fibrillation patients treated with warfarin or rivaroxaban: a retrospective cohort study using real-world evidence.
CONCLUSIONS: Ischemic stroke rates were significantly lower in the overall NVAF population for rivaroxaban vs warfarin users, including patients with eCrCl below 50 mL/min. For all renal function groups, major bleeding risks were not statistically different between treatment groups. PMID: 28590785 [PubMed - as supplied by publisher]

Impact of emergency department arrival time on door-to-needle time in patients with acute stroke
ConclusionFactors that contribute most to a delay in DTN time include arrival during the night shift, lack of a dedicated stroke team, longer time-to-CT read, and arrival as a walk-in. All of these are addressable factors from an operational standpoint and should be considered when performing quality improvement of hospital protocols.
