Current practice of outpatient rehabilitation services in patients with mobility-impaired paralysis due to stroke or spinal cord injury: a qualitative interview study in Germany
CONCLUSION: Areas of improvement identified focused on: setting rehabilitation goals towards participation, training therapists on evidence-based treatments and shared decision-making, updating the outpatient service catalogue, and implementing coordination actions. Implementation of these recommendations should be evaluated.PMID:37732606 | DOI:10.1080/09638288.2023.2259301

Family caregiver adjustment and stroke survivor impairment: A path analytic model.
Objective: Depressive symptoms are a common problem among family caregivers of stroke survivors. The purpose of this study was to examine the association between care recipient’s impairment and caregiver depression, and determine the possible mediating effects of caregiver negative problem-orientation, mastery, and leisure time satisfaction. The evaluated model was derived from Pearlin’s stress process model of caregiver adjustment. Method: We analyzed baseline data from 122 strained family members who were assisting stroke survivors in Germany for a minimum of 6 months and who consented to participate in a randomized ...

Effects of Improved Detection And Treatment of Atrial Fibrillation On Burden of Disease Caused By Stroke In Germany
Atrial fibrillation (AF) is a major risk factor for stroke with a prevalence of 1-2% of the general population. Patients with non-valvular AF show a 4 to5 times higher risk of stroke. According to a German registry study, AF goes undetected in one third of affected patients. Even if correctly diagnosed, AF is not always treated with oral anti-coagulants (OAC) according to treatment guidelines. For Germany, the estimated proportion of patients actually treated with OAC among all eligible AF-patients ranges from 38% to 83%.

Social Gradients in Myocardial Infarction and Stroke Diagnoses in Emergency Medicine.
CONCLUSION: This study shows that the case-activity rate of the emergency medical services is significantly higher in socially disadvantaged neighborhoods, both with respect to total numbers and with respect to individual diseases. It demonstrates a problem affecting society as a whole, which should be taken into account in the organization of medical rescue services. PMID: 29467072 [PubMed - in process]

Budget impact of botulinum toxin treatment for spasticity after stroke — a German perspective
ConclusionThe botulinum toxin therapy is associated with additional costs, but an increased use of botulinum toxin would be consistent with guideline-based therapy. In particular, it has to be considered that spasticity as a chronic condition is accompanied by a relevant loss of quality of life. Thus, considering only the therapy costs may not be sufficient for making final therapeutic decisions.

Pediatric Acute Ischemic Stroke by Age-Group: A Systematic Review and Meta-Analysis of Published Studies and Hospitalization Records
Conclusions: Incidence rates of pediatric AIS by age-groups followed a consistent overall pattern of a reverse J-shaped curve, with the highest rates in neonates, across predominantly European and North American countries. Further research is warranted to examine if this pattern is observed in other geographic regions and to identify AIS risk factors specific to different phases of childhood development.Neuroepidemiology

Reorganizing stroke and neurological intensive care during the COVID-19 pandemic in Germany
Acta Biomed. 2021 Nov 3;92(5):e2021266. doi: 10.23750/abm.v92i5.10418.ABSTRACTThe outbreak of coronavirus disease 19 (COVID-19) has dramatically imposed healthcare systems to reorganize their departments, including neurological wards. We aimed to describe the rearrangements made by stroke units (SU) and neurological intensive care units (ICU) in several German community and university hospitals facing the pandemic. This cross-sectional, survey-based, nationwide study collected data of 15 university and 4 community hospitals in Germany, being part of IGNITE Study Group, from April 1 to April 6, 2020. The rearrangements and ...

The German version of the Stroke Upper Limb Capacity Scale: Translation, cross-cultural adaption and evaluation of its psychometric properties
CONCLUSION: Due to the good to excellent psychometric properties and the low level of floor effects of the German version of the SULCS, the usage for clinical and scientific purposes can be recommended.PMID:34789042 | DOI:10.1177/02692155211043502
