Understanding and informing community emergency cardiovascular disease preparedness during the COVID-19 pandemic: Stroke Ready
Acute stroke and acute myocardial infarction (AMI) treatments are time sensitive. Early data revealed a decrease in presentation and an increase in pre-hospital delay for acute stroke and AMI during the coronavirus disease 2019 (COVID-19) pandemic. Thus, we set out to understand community members ’ perception of seeking acute stroke and AMI care during the COVID-19 pandemic to inform strategies to increase cardiovascular disease preparedness during the pandemic.

Incidence of stroke and mortality due to stroke after acute coronary syndrome
Stroke is a known complication after myocardial infarction (MI) and it is associated with increased mortality. We aimed to establish the true cumulative incidence of stroke and its subtypes and the associated mortality in a contemporary setting among patients treated for acute coronary syndrome (ACS).

Carotid Atherosclerosis and Risk of Subsequent Coronary Event in Outpatients With Atherothrombosis Original Contributions
Conclusions— Carotid atherosclerosis was an independent predictor of coronary events across all types of symptomatic vascular disease and had an incremental effect on risk regardless of risk factors or location of vessel disease.

Vitamin B may reduce risk of stroke
Researchers have uncovered evidence that suggests vitamin B supplements could help to reduce the risk of stroke, according to a study published in the journal Neurology. Vitamin B supplements are said to be beneficial for many health issues, including stress, anxiety, depression, dementia, Alzheimer's disease and heart disease. However, according to Xu Yuming of Zhengzhou University in Zhengzhou, China, previous studies have conflicting findings regarding the use of vitamin B supplements and stroke or heart attack...

Three-Dimensional Carotid Ultrasound Plaque Texture Predicts Vascular Events Clinical Sciences
Conclusions— Changes in both plaque texture and volume are strongly predictive of vascular events. In high-risk patients, 3D ultrasound plaque measurements should be considered for vascular event risk prediction.

Smoking Paradox in Patients Hospitalized With Coronary Artery Disease or Acute Ischemic Stroke: Findings From Get With The Guidelines Original Articles
Conclusions— Among patients hospitalized with CAD and AIS, smoking is a risk factor for early age of onset, even among those with few vascular risk factors. The persistent association with lower in-hospital mortality after adjusted and stratified analyses probably represents residual unmeasured confounding, although a biological effect of smoking cannot be excluded. Further clinical and prospective population-based studies are needed to explore variables that contribute to outcomes in these patients.

Cardiovascular, Bleeding, and Mortality Risks of Dabigatran in Asians With Nonvalvular Atrial Fibrillation Clinical Sciences
Conclusions— In real-world practice, dabigatran was associated with a reduced risk of ischemic stroke, intracranial hemorrhage, all hospitalized major bleeding, and all-cause mortality compared with warfarin in Asian patients with nonvalvular atrial fibrillation. Dabigatran did not increase the risk of major gastrointestinal bleeding or myocardial infarction compared with warfarin.

Unfavorable neurological outcome in diabetic patients with acute ischemic stroke is associated with incomplete recanalization after intravenous thrombolysis
Conclusions Diabetic patients with AIS had unfavorable neurological outcome, potentially linked to incomplete recanalization after IVT.

Elevated FVIII Predicts Subsequent Acute Ischemic Stroke in Patients with Transient Ischemic Attack (S42.004)
CONCLUSIONS: In our sample, eFVIII was associated with increased risk of a subsequent AIS admission. These findings may indicate a greater risk for other future thrombotic events in post-TIA patients with eFVIII and begs the question as to optimal antithrombotic therapy in this population.Disclosure: Dr. Semmes has nothing to disclose. Ischemic Stroke Risk After Acute Coronary Syndrome Stroke
BackgroundPrior studies show an increased risk of ischemic stroke (IS) after myocardial infarction; however, there is limited evidence on long‐term risk and whether it is directly related to cardiac injury. We hypothesized that the risk of IS after acute coronary syndrome is significantly higher if there is evidence of cardiac injury, such as ST‐segment elevation myocardial infarction (STEMI) or non‐STEMI, than when there is no evidence of cardiac injury, such as in unstable angina. Methods and ResultsAdministrative claims data were obtained from all emergency department encounters and hospitalizations at California's...

Stroke and Mortality Risk in Patients With Various Patterns of Atrial Fibrillation: Results From the ENGAGE AF-TIMI 48 Trial (Effective Anticoagulation With Factor Xa Next Generation in Atrial Fibrillation-Thrombolysis in Myocardial Infarction 48) Original Article
Conclusions— In ENGAGE AF-TIMI 48 trial, patients with paroxysmal AF suffered fewer thromboembolic events and deaths compared with those with persistent and permanent AF. The efficacy and safety profile of edoxaban as compared with warfarin was consistent across the 3 patterns of AF. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Timely Reperfusion in Stroke and Myocardial Infarction Is Not Correlated: An Opportunity for Better Coordination of Acute Care Original Article
Conclusions— We found no correlation between hospitals’ observed or risk-adjusted DTN and D2B times. Opportunities exist to improve hospitals’ performance of time-critical care processes for AIS and STEMI in a coordinated approach.

Circulating MicroRNAs as Biomarkers for Acute Ischemic Stroke: A Systematic Review
Acute ischemic stroke is a leading cause of death and disability worldwide. Unlike myocardial infarction, there is no current blood test to diagnose acute ischemic stroke. MicroRNAs (miRNAs) are very stable in the blood and have been suggested as potential diagnostic markers.
