Prevalence of pre-stroke sleep apnea risk and short or long sleep duration in a bi-ethnic stroke population
• High risk of pre-stroke SA is common in ischemic stroke patients.• The prevalence of high pre-stroke SA risk was not higher in MAs than NHWs.• Short or long sleep duration prevalence was not higher in MAs than NHWs.• Sleep duration and pre-stroke SA risk does not explain bi-ethnic stroke disparities.

Sleep Apnea and Stroke
Abstract Clinical evidence has established that sleep apnea is a risk factor for stroke. Patients with stroke have a high prevalence of sleep apnea that may have preceded or developed as a result of the stroke. Well-established concurrent stroke risk factors for stroke like hypertension and atrial fibrillation respond favorably to the successful treatment of sleep apnea. The gold standard diagnosis of sleep apnea is obtained in the sleep laboratory, but unattended polysomnography is gaining acceptance. Positive airway pressure (PAP) (continuous positive airway pressure [CPAP] or bilevel positive airway pressure [...

Nocturnal Hypoxemia Is Associated with White Matter Hyperintensities in Patients with a Minor Stroke or Transient Ischemic Attack
Conclusions:We conclude that nocturnal hypoxemia, predominantly related to OSA, is independently associated with WMH in patients who present with minor ischemic stroke and TIA and may contribute to its pathogenesis.Citation:Patel SK, Hanly PJ, Smith EE, Chan W, Coutts SB. Stroke Unit Evaluation of Sleep Apnea: Validity of Screening Tools and Use of a Portable Sleep Study (P6.011)
Conclusions: The STOP-BANG questionnaire, administered to hospitalized stroke patients, had high sensitivity and low-moderate specificity, compared to two other commonly used screening tools. The feasibility of using an unattended inpatient portable sleep study on stroke patients is demonstrated.Disclosure: Dr. Aparicio has nothing to disclose. Dr. Sturzoiu has nothing to disclose. Expiratory Positive Airway Pressure for Sleep Apnea after Stroke: A Randomized, Crossover Trial
Conclusions:This study suggests that EPAP is not an effective alternative to CPAP in acute stroke patients with OSA. Further work is needed to identify other more effective alternatives.Clinical Trial Registration:ClinicalTrials.gov, ID: NCT01703663Citation:Wheeler NC, Wing JJ, O'Brien LM, Hughes R, Jacobs T, Claflin E, Chervin RD, Brown DL. Impact of obstructive sleep apnea on neurological recovery after ischemic stroke: A prospective study
ConclusionIschemic stroke patients with OSA tend to have poor neurological and functional recovery, across all segments of stroke and OSA severity.

Recurrent Ischemic Stroke: Strategies for Prevention.
Abstract Recurrent strokes make up almost 25% of the nearly 800,000 strokes that occur annually in the United States. Risk factors for ischemic stroke include hypertension, diabetes mellitus, hyperlipidemia, sleep apnea, and obesity. Lifestyle modifications, including tobacco cessation, decreased alcohol use, and increased physical activity, are also important in the management of patients with a history of stroke or transient ischemic attack. Antiplatelet therapy is recommended to reduce the risk of recurrent ischemic stroke. The selection of antiplatelet therapy should be based on timing, safety, effectiveness, ...

The Effect of an Educational Pamphlet on Patient Knowledge of and Intention to Discuss Screening for Obstructive Sleep Apnea in the Acute Ischemic Stroke Population
Conclusions: A brief educational pamphlet written using health literacy concepts was considered valuable and improved patient knowledge and intention to discuss OSA screening with a physician. Further work is needed to determine whether the pamphlet can promote a discussion and referral for OSA screening at the primary care level.

Sleep Apnea Is a Risk Factor for Stroke and Vascular Dementia
AbstractPurpose of ReviewIn this article, we review the cerebrovascular complications of sleep apnea (SA). SA is the major sleep disorder associated with stroke and vascular dementia.Recent FindingsSleep apnea syndrome of moderate to severe intensity affects 17% of 50 –70-year-old men and 9% of 50–70-year-old women, making SA a notorious and prevalent disorder. SA increases the risk of hypertension, stroke, myocardial infarction, and atrial fibrillation (AF) and is closely linked to vascular dementia. In addition, SA may worsen the neurologic outcome in acute stroke patients and interferes with rehabilitation after str...

Home Sleep Apnea Testing is a Feasible and Accurate Method to Diagnose Obstructive Sleep Apnea in Stroke Patients During In-Hospital Rehabilitation
Conclusions:These findings confirm a good feasibility and sufficient accuracy of HSAT attached in a stroke rehabilitation unit. Therefore, the authors suggest that American Academy of Sleep Medicine recommendations for HSAT should include stroke patients.Citation:Saletu MT, Kotzian ST, Schwarzinger A, Haider S, Spatt J, Saletu B. Thrombolysis and evolution of sleep apnea in ischemic stroke
Conclusions: Sleep apnea is present in the vast majority of newly diagnosed ischemic stroke patients and the prevalence remained high after six months follow-up. Remarkable declining in ESS, ODI4 and hypopneas per hour was observed in the thrombolysis group. Thrombolysis seems to be an independent protective factor for developing sleep apnea post-stroke.

Oximetry as an Accurate Tool for Identifying Moderate to Severe Sleep Apnea in Patients With Acute Stroke
Conclusions:Nocturnal pulse oximetry has a high diagnostic accuracy in predicting moderate to severe SDB in patients with acute stroke. Oximetry can be a simple modality to rapidly recognize patients with more severe SDB and facilitate the referral to the confirmation sleep study.Citation:Lin SH, Branson C, Park L, Leung J, Doshi N, Auerbach SH. Evaluation of an OSA risk stratifying and treatment protocol during inpatient rehabilitation of post-stroke patients.
CONCLUSIONS: The use of APAP in high-risk patients was poorly tolerated and did not improve post-stroke recovery. Further studies with larger sample sizes are needed to determine the effect of APAP treatment on short-term recovery. PMID: 31290082 [PubMed - as supplied by publisher]

Obstructive sleep apnea syndrome and neuropsychological function in pediatric stroke.
CONCLUSIONS: The prevalence of OSAS in children with AIS compared to healthy controls is significantly elevated and is associated with poor neuropsychological outcomes. We highlight the importance of regular screening for OSAS - a modifiable risk factor - in children with AIS. The specific risk factors for OSAS and the potential benefits of therapeutic interventions in this patient population warrant further investigation. PMID: 31787553 [PubMed - as supplied by publisher]
