Decompressive craniectomy in malignant middle cerebral artery infarction: family perception, outcome and prognostic factors.
CONCLUSIONS: We present a 21 patients group where the best outcome was achieved in patients ≤60 years old. The severe neurological sequelae in patients with malignant infarction subjected to decompressive hemicraniectomy are tolerated and accepted by most families to the benefit of survival. Total Small Vessel Disease Burden Predicts Functional Outcome in Patients With Acute Ischemic Stroke
Conclusion: Total SVD burden negatively associated with functional outcome at 3 months in patients with acute ischemic stroke and is superior to individual SVD feature in prediction of functional outcome. MRI-based assessment of total SVD burden is highly valuable in clinical management of stroke victims, and could help guide the allocation of resources to improve outcome.

Staphylococcus aureus prosthetic valve endocarditis (SAPVE) has a poor prognosis. There are no large series that accurately describe this entity. This is a retrospective observational study on a prospective cohort from 3 Spanish reference hospitals for cardiac surgery, including 78 definitive episodes of left SAPVE between 1996 and 2016. Fifty percent had a Charlson Index score>5; 53% were health care-related. Twenty percent did not present fever. Prevalence, risk factors and outcomes of patients coming from the community with sepsis due to multidrug resistant bacteria
ConclusionIn light of the prevalence and impact of MDR bacteria causing sepsis in patients coming from the community, physicians should consider ESBL coverage when starting an empiric antibiotic therapy in patients with specific risk factors, especially in the presence of septic shock.

The predictive dysphagia score (PreDyScore) in the short- and medium-term post-stroke: a putative tool in PEG indication
ConclusionBMI<  20 and mRS ≥ 3 are easily measurable bedside predictive factors of persistent dysphagia. PreDyScore showed good sensitivity and very good specificity and enables the prediction of persistent dysphagia with great accuracy in any clinical setting.

Safety of urgent endarterectomy in acute non ‐disabling stroke patients with symptomatic carotid artery stenosis: an international multicenter study
ConclusionsOur findings highlight that urgent CEA performed within two days from the index event is related to a non ‐significant increase in the risk of peri‐procedural stroke.
