Pathogenic lipid-binding antiphospholipid antibodies are associated with severity of COVID-19
CONCLUSIONS: COVID-19 patients rapidly expand B1a cells secreting pathogenic lipid-binding aPL with broad thrombotic and inflammatory effects. The association with markers of inflammation and coagulation, clinical severity and mortality suggests a causal role of aPL in COVID-19 associated coagulopathy.PMID:34242469 | DOI:10.1111/jth.15455

Traditional risk factors and combined genetic markers of recurrent ischemic stroke in adults
CONCLUSIONS: Our findings suggest that cumulative effect of both traditional and common genetic risk factors were associated with recurrence of ischemic stroke. We demonstrated for the first time that a combined genotype FVL/MTHFR profile increase the risk of a second cerebral ischemic attack.PMID:34241942 | DOI:10.1111/jth.15448

Circulating cytokines and growth factors in acute cerebral large vessel occlusion - association with success of endovascular treatment
In conclusion, this study provides first evidence that cytokines and growth factors are potential effectors in patients undergoing MT for the treatment of acute ischemic stroke.PMID:34225367 | DOI:10.1055/a-1544-5431

Risk of ischemic stroke in asymptomatic atrial fibrillation incidentally-detected in primary care compared with other clinical presentations
CONCLUSION: Ischemic stroke risk in patients with AA-AF incidentally-detected in primary care is far from benign, and not less than incident AF presenting clinically in general practice or hospital. This provides justification for identification of previously undetected AF, e.g. by opportunistic screening, and subsequent stroke prevention with thromboprophylaxis, to reduce the approximately 10% of ischemic strokes related to unrecognized AF.PMID:34192776 | DOI:10.1055/a-1541-3885

Suggested treatment of serious complications to Covid-19 vaccination with IdeS, a bacterial antibody-cleaving enzyme
J Thromb Haemost. 2021 Jun 24. doi: 10.1111/jth.15433. Online ahead of print.ABSTRACTSeveral recent reports have independently described a rare thrombosis with thrombocytopenia syndrome (TTS) post vaccination with ChAdOx1 nCoV-19. In J. Thromb. Haemost., Ankerlund Blauenfeldt et al. [1] described the clinical presentation of a case of fatal acute ischemic stroke associated with profound thrombocytopenia. The pathogenesis behind this case may involve mechanisms similarly to autoimmune heparin-induced thrombocytopenia (HIT) since anti-platelet factor 4(PF-4) antibodies were detected in the patient plasma.PMID:34165239 | DOI:10.1111/jth.15433

Hemorrhagic Transformation and Stroke Recurrence in Children with Cardiac Disease Receiving Antithrombotic Therapy for Secondary Stroke Prevention
CONCLUSIONS: In spite of the 24% hemorrhagic transformation rate, antithrombotic therapy has a positive risk-balance in certain cardioembolic stroke subgroups, particularly in those with single-ventricle physiology, when accounting for stroke volume.PMID:34152075 | DOI:10.1111/jth.15428

Considerations for use of direct oral anticoagulants in arterial disease
Res Pract Thromb Haemost. 2021 May 28;5(4). doi: 10.1002/rth2.12502. eCollection 2021 May.ABSTRACTCardiovascular diseases including coronary heart disease, stroke, and peripheral arterial disease were responsible for an estimated 18 million deaths in 2017. Despite advances in management over the past several decades, these patients continue to have substantial risk of subsequent cardiovascular events. We provide a narrative review of randomized clinical trials evaluating direct oral anticoagulants (DOACs) for the treatment of acute coronary syndromes, noncardioembolic ischemic stroke, embolic stroke of undetermined source,...

An older thrombus delays reperfusion after mechanical thrombectomy for ischemic stroke
CONCLUSIONS: An older thrombus delays reperfusion after mechanical thrombectomy for ischemic stroke. Adding therapies targeting thrombus maturation may improve the efficacy of mechanical thrombectomy.PMID:34077976 | DOI:10.1055/a-1522-4507

Television Viewing, physical activity and Venous Thromboembolism Risk: the REasons for Geographic and Racial Differences in Stroke (REGARDS) Study
CONCLUSIONS: In this contemporary racially and geographically diverse United States cohort, there was no association between TV viewing and VTE risk, before and after accounting for physical activity. The high burden of traditional VTE risk factors in REGARDS may mask any association of TV viewing with VTE, or TV viewing may have only a modest association with VTE risk.PMID:34077616 | DOI:10.1111/jth.15408
