Epidemiological Analysis of 37,424 Carotid Artery Stenosis Intervention Procedures During 11 Years in the Public Health System in Brazil: Stenting has Been More Common Than Endarterectomy
Stroke (cerebrovascular accident) figures among CVDs as the second leading cause of death worldwide, responding for approximately 5.7 million cases per year, which represent 10% of global mortality,1 as well as the third leading cause of disability globally.2 In Brazil, stroke is currently the second leading cause of death and impairing symptoms.3 In 10 –20% of cases, strokes are secondary to carotid atherosclerosis,4 hence the relevance of analyzing carotid bifurcation operative procedures as a sensible preventative measure for cerebral ischemic diseases.

However, female gender is still considered as an independent risk factor for CAS procedures, potentially limiting immediate and long-term benefits. Decision-making for discharge destination following a CEA to home or non-home locations is important due to the differences in survival and post-operative complications. While primary outcomes such as mortality and occurrence of stroke following CEA have been extensively studied, there is a paucity of information characterizing outcomes of discharge destination and the factors associated.

The Underutilization of Regional Anesthesia for Carotid Endarterectomy: An Anesthesiology Perspective
We read with great interest the recent article by Lumas and colleagues, entitled “Regional Anesthesia is Underutilized for Carotid Endarterectomy Despite Improved Perioperative Outcomes Compared with General Anesthesia.”1 This retrospective study of 26,206 carotid endarterectomy (CEA) patients demonstrated a small but statistically significant decrease in composite rates of 30-day mortality, myocardial infarction (MI), and stroke among patients who received regional, as opposed to general, anesthesia.
