Vascular Surgery In Low- And Middle- Income Countries: A State-of-the-art Review
Cardiovascular disease (CVD) represents 32% of all global deaths. Estimates project an increase in CVD mortality with the most substantial increase in low- and middle- income countries (LMICs). Within LMICs, we sought to 1) measure the burden of CVD with respect to aortic aneurysm (AA), ischemic stroke (IS), and peripheral artery disease (PAD); 2) quantify surgical access to vascular surgery services.

Effect of Postoperative Stroke Timing on Perioperative Mortality after Carotid Revascularization
In-hospital stroke (IHS) has been associated with worse outcomes than out-of-hospital stroke (OHS) due to delays in diagnosis and treatment. A paucity of studies exist comparing the timing of postoperative stroke after carotid revascularization. We aimed to study the effect of IHS vs. OHS on postoperative mortality in carotid revascularization patients in a large-scale national database.

Feasibility and accuracy of measuring carotid plaque volume (burden) with contrast-enhanced tomographic 3D ultrasound and ultrasound image fusion.
Stenosis severity has been the indication for carotid endarterectomy (CEA) for four decades, but the annual stroke risk in asymptomatic carotid stenosis>70% is under 2%. Atherosclerotic volume has emerged as a risk factor for future stroke, but needs to be measured non-invasively. Tomographic ultrasound (tUS) is a novel technology that assembles 3D images in seconds. We evaluated accuracy of measuring Carotid Plaque Volume (CPV) with tUS in patients undergoing CEA.

Controversy exists regarding the timing of intervention for patients with critical coronary artery disease (CAD) awaiting coronary artery bypass and severe carotid artery stenosis (CAS). Transcarotid artery revascularization (TCAR) is a minimally invasive revascularization alternative through direct transcervical carotid access that minimizes the chance of arch manipulation and consequent antegrade embolic stroke rate. While the TCAR procedure can be performed under local anesthesia (monitored anesthesia care [MAC]) versus general anesthesia, the hemodynamic benefits of local anesthesia in patients with severe CAD are significant.

Regarding an Apparently High Stroke Rate in Patients With Medically Managed Asymptomatic Carotid Stenosis at an Academic Center in the Southeastern United States
We respectfully disagree with Weiner et al and advise that it is premature to give up on current best non-invasive medical intervention (lifestyle coaching and medication) alone for patients with asymptomatic carotid stenosis (ACS).(1) Weiner et al reported an average annual ipsilateral 'stroke' rate of 2.9% (10 ipsilateral strokes, in 173 patients with>70% stenosis over 2 years).
