A Stroke/Vascular Neurology Service Increases the Volume of Urgent Carotid Endarterectomies Performed in a Tertiary Referral Center
Background: Increasing evidence supports that urgent carotid endarterectomy (CEA), defined as CEA during the index hospitalization, may be undertaken in select patients with acute carotid-related neurologic symptoms to prevent recurrent ischemic events. We aimed to determine the effect of a stroke/vascular neurology service on the volume of urgent CEAs performed and assess perioperative outcomes. Methods: A retrospective review from a single tertiary referral center between June 2005 through December 2011 revealed 393 patients who underwent CEA. We identified the number of urgent CEAs before (June 2005–August 2008) and af...

Results of Adjunctive Spinal Drainage and/or Left Subclavian Artery Bypass in Thoracic Endovascular Aortic Repair
Conclusions: Our use of adjunctive procedures for TEVAR demonstrated better SCI results compared with those of prior reports of selective CSF drainage when SCI arises. Our approach was associated with improved 1-year survival. Preoperative CSF drain placement allows for rapid, intensive therapy for SCI and should be considered when clinically feasible.

Correlation of ABCD2 Score with Degree of Internal Carotid Artery Stenosis: An Observational Pilot Study
Conclusions: Because no correlation between ABCD2 scores and the degree of ICA stenosis was found, all patients with carotid territory TIA should undergo urgent imaging of the carotid arteries because a high proportion of these patients may benefit from RACE.

Mandibular Subluxation as an Adjunct in Very Distal Carotid Arterial Reconstruction: Incidence of Peripheral and Cerebral Neurologic Sequelae in a Single-Center Experience
Conclusions: MS-CEA can be a very useful technical adjunct for high-located carotid bifurcations or challenging carotid lesions, with an overall risk comparable to that of standard CEA.

Predictors of Cervical Bleeding after Carotid Endarterectomy
Conclusions: Postoperative severe bleeding after carotid surgery in our institution is not an uncommon complication. Its incidence is within the range reported in the literature, but it is not associated with major complications or mortality. Antiplatelet treatment with clopidogrel is the main risk factor associated with reintervention. Other factors, such as coagulation control, postoperative hypertension management, and the use of an autologous patch, could help reduce its incidence.

Perioperative Outcome of Carotid Endarterectomy According to Type and Timing of Neurologic Symptoms and Computed Tomography Findings
Background: Carotid endarterectomy (CEA) in the early period after the onset of cerebral neurologic symptoms seems to be useful in the prevention of recurrent stroke while safety is still under debate. Asymptomatic carotid artery stenosis could be associated with silent cerebral ischemic events (CIEs) detectable on cerebral computed tomographic (CT) scans, but is not clear if (or how) silent cerebral lesions could influence the outcome of CEA. The aim of our work was to analyze the outcome of CEA according to the timing and type of neurologic symptoms and to evaluate the outcome of CEA in asymptomatic patients with silent ...

Carotid Restenosis After Endarterectomy and Stenting: A Critical Issue?
Conclusions: This review of our 10-year experience confirms that patients who develop restenosis after CEA are also prone to developing in-stent restenosis after CAS.

A Retrospective Study on Early Carotid Endarterectomy within 48 Hours after Transient Ischemic Attack and Stroke in Evolution
Conclusions: CEA can be performed with an acceptable risk in properly selected symptomatic patients within 48 hours after TIA or SIE. The benefits of early CEA in symptomatic patients include the prevention of recurrent stroke.

Female Sex Does Not Influence 30-Day Stroke and Mortality Rates after Carotid Endarterectomy
Conclusions: Female sex does not influence 30-day stroke and mortality rates after CEA, regardless of preoperative symptom status.

Cannabis-Associated Arterial Disease
Conclusion: Cannabis use is associated with arterial disease such as stroke, myocardial infarction, and limbs arteritis. It appears essential to investigate cannabis use in young patients presenting with such arterial manifestations, as outcome is closely correlated with cannabis withdrawal.

New Cerebral Ischemic Lesions After Carotid Endarterectomy
Conclusions: New ischemic lesions on DW-MRI are detected in 7.5% of patients after CEA, and most of these lesions are clinically silent. Plaque echogenicity does not affect their incidence. New lesions seen on DW-MRI may be generated outside of the treated carotid artery territory.
