Stroke from an External Carotid: Lesion Pattern and Mechanisms
Traditionally, patients with symptomatic external carotid stenosis present with neck or face pain, retinal ischaemic symptoms or jaw claudication. and rarely as ipsilateral cerebrovascular events. In this present case, our patient suffered a stroke from a paradoxical embolism from the external carotid, without involvement of the internal carotid artery. A plaque ulceration of the external carotid’s origin was the cause of this cerebral emboli. Duplex ultrasound showed a pathological left external carotid, with a floating thrombus in the internal carotid.

The debate between carotid artery stenting (CAS) and carotid endarterectomy (CEA) for the management of carotid stenosis continues. Supporters of CAS claim that CAS has achieved clinical equipoise with CEA.1,2 In contrast, based on the results of multicenter randomized controlled trials (RCTs),3–5 others argue that it is currently premature to conclude that CAS is as effective as CEA.6 Even in the best academic centers, CAS is consistently associated with significantly higher rates of stroke or death compared with CEA.

Extended Eversion Carotid Endarterectomy: Computation of Hemodynamics
Stroke prevention includes surgery for significant stenosis of internal carotid artery. Consensus on a standard approach lacks and one alternative approach is eversion carotid endarterectomy (eCEA). In order to overcome disadvantages of eCEA, we developed extended eversion carotid endarterectomy (exeCEA). Aiming to investigate hemodynamics after different surgical approaches, we created computational fluid dynamics models of exeCEA and eCEA with included progressing lumen narrowing - representation of artery restenosis at the incision line.

Carotid Endarterectomy in the Face of a Persistent Hypoglossal Artery
A 68 year-old woman presented with signs and symptoms suggestive of a cerebrovascular accident. Her workup revealed left internal carotid artery stenosis, consistent with her presentation of a left middle cerebral artery distribution stroke. Imaging also incidentally revealed a persistent hypoglossal artery. She underwent carotid endarterectomy. Our intraoperative strategy for management of this persistent fetal anomaly is reviewed.

Carotid Endarterectomy versus Carotid Artery Stenting: No Difference in 30-Day Post-Procedure Readmission Rates
In the United States, ischemic stroke is a major cause of morbidity and mortality, precipitated by carotid artery stenosis in one out of every five individuals who suffer a stroke. Carotid endarterectomy (CEA) and carotid artery stenting (CAS) are two proven means of intervening on this disease process, with similar patient outcomes. Little is known about the burden of readmission following each of these procedures. We hypothesized that no difference in readmission rates within 30 days would exist for these two procedures, in spite of baseline differences that might exist between the two patient populations.
