Cognitive impairment predicts engagement in inpatient stroke rehabilitation
Patient engagement during inpatient rehabilitation is an important component of rehabilitation therapy, as lower levels of engagement are associated with poorer outcomes. Cognitive deficits may impact patient engagement during inpatient stroke rehabilitation. Here, we assess whether patient performance on the cognitive tasks of the 30-min National Institute of Neurologic Disorders and Stroke – Canadian Stroke Network (NINDS-CSN) screening battery predicts engagement in inpatient stroke rehabilitation. Prospective data from 110 participants completing inpatient stroke rehabilitation at an academic medical center were util...

Dual-Tasking in Daily Activities Among Adults With and Without Stroke
CONCLUSIONS AND RELEVANCE: The initiation of dual-tasking is curtailed in the nondominant and paretic UEs, extending previous laboratory-based findings to a more naturalistic setting. These results may reflect a demand on neural resources that is exceeded when these limbs are used. What This Article Adds: DOPs, a functional strategy to simultaneously engage two objects during ADLs, could serve as a behavioral marker of dual-tasking in real-world activities, supporting their investigation more broadly. Practicing DOPs in rehabilitation could also train the integration of dual-tasking strategies in activity execution.PMID:36...

Association Between Spotty Calcification in Nonstenosing Extracranial Carotid Artery Plaque and Ipsilateral Ischemic Stroke
Conclusions Spotty calcifications associated with low-density plaque and maximum plaque thickness were associated with ipsilateral ischemic stroke in patients with nonstenotic carotid atherosclerosis, suggesting a role as imaging markers of high-risk plaque.PMID:37183863 | DOI:10.1161/JAHA.122.028525

Eligibility Determination for Intravenous Thrombolysis Based on Radiology Interpretation Report of the Head CT Scan in Patients with Acute Ischemic Stroke
CONCLUSIONOur study demonstrates that administering IV t‐PA to patients based on the stroke team's interpretation of the CT scan versus review of the radiology interpretation does not lead to significant differences in clinical outcome, aICH, or sICH.

Stroke in a Young Patient: A Sentinel Presentation of Neurosyphilis and HIV (P2.109)
Conclusions: Neurosyphilis should be a diagnostic consideration in young patients with ischemic stroke. Clinicians should be aware of the high incidence of concomitant syphilis and HIV in at-risk populations. Effort should be made to inquire about high-risk behaviors and initiate testing for these infections early in the diagnostic workup to avoid missing these crucial diagnoses.Disclosure: Dr. Flaherty has nothing to disclose. Endovascular Stroke Therapy Results Improve over Time: The ‘Learning Curve' at a New Comprehensive Stoke Center
Conclusions: At a new community-based CSC, improved outcome occurred from year 1 to year 2, and trends towards decreased mortality, fewer complications, and improved revascularization were observed. Furthermore, the data shows that improvement in NIHSS score after EST is associated with decreased mortality following stroke in this setting, implying a net benefit.Cerebrovasc Dis Extra 2014;4:235-242

Balancing Autonomy and Beneficence in Acute Stroke Therapy: Neurologist Attitudes Toward Delaying Thrombolysis in Order to Obtain Informed Consent (P7.332)
CONCLUSIONS: Vascular neurologists indicate they emphasize beneficence over autonomy in deciding when to forego further attempts to reach consent providers to proceed with thrombolytic therapy under the doctrine of presumed consent. Their preference for rapid intervention is magnified when decisions are framed in person failure to benefit and neuron loss, compared with simple chronologic time.Disclosure: Dr. Kaplan has nothing to disclose. Abstract 105: Obesity Paradox in Patients with Non-Hemorrhagic Stroke: Increased Short-Term Survival with Decreased Longevity Session Title: Poster Session I
Conclusions: Obese patients present with non-hemorrhagic stroke at a younger age. Despite short term improved outcomes, obesity is not associated with improved overall longevity.

The Association Between Emergency Department Crowding and the Disposition of Patients With Transient Ischemic Attack or Minor Stroke
ConclusionsThese results suggest that crowding may influence clinical decision‐making in the disposition of patients with TIA or minor stroke and that, as crowding worsens, the likelihood of hospitalization increases.

Community physicians’ knowledge of secondary prevention after ischemic stroke: a questionnaire survey in Shanxi Province, China
Conclusion: The survey revealed a huge gap in knowledge of current guidelines for secondary prevention of ischemic stroke and TIA among the physicians surveyed. Continuing education and training of community physicians, administered as a public health program, is needed to improve the healthcare of ischemic stroke and TIA patients.

Monte Carlo Simulation Modeling of a Regional Stroke Team's Use of Telemedicine
ConclusionsGiven the potential societal benefits, continued efforts to deploy telemedicine appear warranted. Aligning the incentives between those who would have to fund the up‐front technology investments and those who will benefit over time from reduced ongoing health care expenses will be necessary to fully realize the benefits of telemedicine for stroke care.

Randomized Trial of Peripheral Nerve Stimulation to Enhance Modified Constraint-Induced Therapy After Stroke.
CONCLUSION: Pairing peripheral nerve stimulation with modified constraint-induced therapy can lead to significantly more improvement in upper extremity movement function than modified constraint-induced therapy alone. Future research is recommended to help establish longitudinal effects of this paired intervention, particularly as it affects movement function and daily life participation.
