Efficacy of High-Dose and Low-Dose Simvastatin on Vascular Oxidative Stress and Neurological Outcomes in Patient with Acute Ischemic Stroke: A Randomized, Double-Blind, Parallel, Controlled Trial.
Conclusion: High-dose simvastatin might be helpful to reduce serum sLOX-1. But no difference in clinical outcomes was found between high- and low-dose simvastatin. Further more intensive clinical trial is needed to confirm the appropriate dosage of simvastatin in patients with acute ischemic stroke. This trial is registered with ClinicalTrials.gov ID: NCT03402204. PMID: 29850244 [PubMed]

Escaping the atherogenic trap: Preventing LDL fusion and binding in the intima
Low-density lipoprotein (LDL) cholesterol plasma concentrations have served as the target of clinical treatment of atherosclerosis-related diseases since the development of simvastatin three decades ago. Targeting endogenous cholesterol synthesis by the inhibition of β-hydroxy-β-methylglutaryl co-enzyme A (HMG-CoA) reductase, statins have since become a mainstay in the lowering of plasma cholesterol and LDL levels and the improvement of cardiovascular outcomes such as myocardial infarction and stroke.

Severe Symptoms, but a Truly Treatable Disease
BY NOURA MAHDI; DARRON LEWIS; JEREMY OSBORNE; & AHMED RAZIUDDIN, MDA 73-year-old man was brought to the emergency department from his nursing home for rectal bleeding and anemia. The patient mentioned he had had episodes of bright red rectal bleeding and constipation for a few months. A colonoscopy had been done prior to the visit, which revealed a large intestine tumor and biopsy confirming adenocarcinoma. He was awaiting an appointment with his surgeon. The patient reported bloody rectal leakage, and a CBC done at the nursing home showed a hemoglobin level of 7.2. He also complained of dyspnea but denied any other ...

Simvastatin blocks soluble SSAO/VAP-1 release in experimental models of cerebral ischemia: Possible benefits for stroke-induced inflammation control
Publication date: February 2018 Source:Biochimica et Biophysica Acta (BBA) - Molecular Basis of Disease, Volume 1864, Issue 2 Author(s): Ping Sun, Mar Hernandez-Guillamón, Mireia Campos-Martorell, Alba Simats, Joan Montaner, Mercedes Unzeta, Montse Solé Beyond cholesterol reduction, statins mediate their beneficial effects on stroke patients through pleiotropic actions. They have shown anti-inflammatory properties by a number of different mechanisms, including the inhibition of NF-κB transcriptional activity and the consequent increase and release of adhesion molecules. We have studied simvastatin's effects on the vasc...

Benefit of Ezetimibe Added to Simvastatin in Reduced Kidney Function
Efficacy of statin-based therapies in reducing cardiovascular mortality in individuals with CKD seems to diminish as eGFR declines. The strongest evidence supporting the cardiovascular benefit of statins in individuals with CKD was shown with ezetimibe plus simvastatin versus placebo. However, whether combination therapy or statin alone resulted in cardiovascular benefit is uncertain. Therefore, we estimated GFR in 18,015 individuals from the IMPROVE-IT (ezetimibe plus simvastatin versus simvastatin alone in individuals with cardiovascular disease and creatinine clearance >30 ml/min) and examined post hoc the relationsh...

Impact of Ezetimibe on the Rate of Cardiovascular-Related Hospitalizations and Associated Costs Among Patients With a Recent Acute Coronary Syndrome: Results From the IMPROVE-IT Trial (Improved Reduction of Outcomes: Vytorin Efficacy International Trial) Original Articles
Conclusions— Addition of ezetimibe to statin therapy in patients with a recent acute coronary syndrome leads to reductions in cardiovascular-related hospitalizations and associated costs, with the greatest cost offsets in high-risk patients. These cost reductions may completely offset the cost of the drug once ezetimibe becomes generic, and may lead to cost savings from the perspective of the healthcare system, if treatment with ezetimibe is targeted to high-risk patients. Using Lower Cost Statins Improves Outcomes for Normal Cholesterol Non-diabetic Patients.
CONCLUSIONS Under budget constraints, using Simvastatin instead of branded Rosuvastatin resulted in a 47 fold increase of prevention of MACE for the entire IUP. These results should be considered while initiating statin therapy in this target population. PMID: 28277855 [PubMed - as supplied by publisher]

Association between statin use and ischemic stroke or major hemorrhage in patients taking dabigatran for atrial fibrillation.
Authors: Antoniou T, Macdonald EM, Yao Z, Hollands S, Gomes T, Tadrous M, Mamdani MM, Juurlink DN, Canadian Drug Safety and Effectiveness Research Network Abstract BACKGROUND: Dabigatran etexilate is a prodrug whose absorption is opposed by intestinal P-glycoprotein and which is converted by carboxylesterase to its active form, dabigatran. Unlike other statins, simvastatin and lovastatin are potent inhibitors of P-glycoprotein and carboxylesterase, and might either increase the risk of hemorrhage with dabigatran etexilate or decrease its effectiveness. METHODS: We conducted 2 population-based, nested case-contr...

154: a pharmacoeconomic evaluation to make decision: is the use of simvastatin 10 mg as an otc medication cost-effective for the primary prevention of stroke amongst women in iran?
Conclusion: In Iran no threshold has been determined but the results were compared with the threshold that WHO recommended (GDP per capita for Iran is $5442.9 for 2014 by World Bank). Considering this threshold, the evaluated intervention could be considered cost-effective.

Simvastatin Promotes Hematoma Absorption and Reduces Hydrocephalus Following Intraventricular Hemorrhage in Part by Upregulating CD36
AbstractWe previously found that hematoma worsens hydrocephalus after intraventricular hemorrhage (IVH) via increasing iron deposition and aggravating ependymal cilia injury; therefore, promoting hematoma absorption may be a promising strategy for IVH. Recently, some investigations imply that simvastatin has the ability of accelerating hematoma absorption. Thus, this study was designed to examine the efficacy of simvastatin for IVH in rats. Intracerebral hemorrhage with ventricular extension was induced in adult male Sprague –Dawley rats after autologous blood injection. Simvastatin or vehicle was administered orally at ...
