Ischemic Stroke due to Virologically-Confirmed Varicella Zoster Virus Vasculopathy: A Case Series
Background: Limited data are available regarding the characteristics and prognosis of patients with stroke due to varicella zoster virus (VZV) vasculopathy. Methods: We studied 4 patients (2 men and 2 women; age, 38-63 years) from a single center who developed acute ischemic stroke due to VZV vasculopathy. The virological diagnosis was confirmed by detecting VZV DNA and/or the IgG antibody to VZV in the cerebrospinal fluid. Results: Three patients were taking immunosuppressive agents, including prednisolone and/or methotrexate, at baseline.

Attenuation of Myeloid Specific TGF β Signaling Induces Inflammatory Cerebrovascular Disease and Stroke.

Stroke like syndrome and simple partial seizures secondary to intrathecal methotrexate use for the treatment of acute lymphocytic leukemia. (p6.181)
Conclusions:Our case may be the first reported of seizures and stroke like syndrome in the same patient for ALL treated with MTX. Clinicians should include Methotrexate induced leukoencephalopathy in their differential of seizures, stroke like syndrome and a combination of both. MRI DWI, ADC and FLAIR images should be included in the diagnostic evaluation of MTX induced encephalopathy especially with IT methotrexate.Disclosure: Dr. Fares has nothing to disclose. Risk of Myocardial Infarction and Stroke in Patients with Granulomatosis with Polyangiitis: A Population‐Based Study
Conclusion: GPA patients have a significantly increased risk of MI, with a non‐statistically significant trend towards increased risk of ischemic stroke. Monitoring for this complication and vigilance in modifying risk factors is particularly warranted in this patient population, especially early after diagnosis. Characteristics of methotrexate-induced stroke-like neurotoxicity.
Abstract Intrathecal administration of methotrexate (IT-MTX) can lead to neurotoxicity. MTX-induced neurotoxicity occasionally manifests with a stroke-like presentation that is difficult to distinguish from genuine stroke. We retrospectively reviewed records of nine patients with leukemia or lymphoma and episodes of stroke-like presentation at our institute between 2010 and 2015 for whom magnetic resonance imaging (MRI) data were available. Methotrexate might reduce ischemic stroke in patients with rheumatoid arthritis: a population ‐based retrospective cohort study
ConclusionsAt a dosage of > 0.5 defined daily dose, short‐term methotrexate might decrease ischemic stroke risk in RA patients, while hydroxychloroquine and sulfasalazine were neutral.

Methotrexate induced leucoencephalopathy: A stroke mimic
We describe the case of a 17 year old girl with acute lymphoblastic leukemia, who developed stroke like episodes on two consecutive challenges with a chemotherapeutic regime which included intravenous and intrathecal methotrexate. She had MRI changes consistent with acute ischemic stroke on both occasions. Her deficits recovered completely and spontaneously, as did the MRI changes. She did not have any further episodes when methotrexate was excluded from the chemotherapeutic regime.

Acute methotrexate-related neurotoxicity and pseudo-stroke syndrome.
We describe five episodes of transient acute leukoencephalopathy mimicking a stroke, reported in the literature as "pseudo-stroke syndrome". Neurologic symptoms occurred 3-10 days after IV or IT methotrexate and manifested as aphasia and alternating sensorimotor deficit. The fluctuating symptomatology regressed completely within a few days. Brain MRI, which is essential for diagnosis, demonstrated early white matter diffusion restriction in the affected cerebral area.
