Blood Flow Restriction Exercise Attenuates the Exercise-Induced Endothelial Progenitor Cell Response in Healthy, Young Men
Conclusion In summary, this is the first study to show that BFR exercise did not augment EPC response to exercise, and in fact blunted the EPC response to low load unilateral KE exercise in young, healthy males. Ethics Statement This study was carried out in accordance with the recommendations of Edinburgh Napier University Research and Ethics Governance Committee. The study was ethically approved by Edinburgh Napier University Research and Ethics Governance Committee. All participants gave written informed consent in accordance with the Declaration of Helsinki. Author Contributions MR, RM, AP, CW, GF-J designed the s...

Asymptomatic Carotid Stenosis Is Associated With Circadian and Other Variability in Embolus Detection
Conclusions: Embolism associated with asymptomatic carotid stenosis shows circadian variation with highest rates 4–6 h before midday. This corresponds with peak circadian incidence of stroke and other vascular complications. These and ASED Study results show that monitoring frequency, duration, and time of day are important in ES detection. Introduction Transcranial Doppler (TCD) detected microembolism in the ipsilateral middle cerebral artery (MCA) may help stratify the risk of stroke and other arterial disease complications in persons with advanced (≥60%) asymptomatic carotid stenosis. If so, this t...

Update in Cardiovascular Safety of Incretin-Based Therapy in Adults with Type 2 Diabetes Mellitus: A Meta-Analysis of Cardiovascular Outcome Trials
ConclusionThis meta-analysis demonstrated that GLP-1RAs were associated with a significant reduction in MACE, CV death, stroke, and death from any cause, while DPP-4 inhibitors were comparable to placebo for all CV outcomes, including hospitalizations for HF.

Glucose screening in pregnancy and future risk of cardiovascular disease in women: a retrospective, population-based cohort study
Publication date: Available online 27 March 2019Source: The Lancet Diabetes & EndocrinologyAuthor(s): Ravi Retnakaran, Baiju R ShahSummaryBackgroundIn studies to date, gestational diabetes has consistently been associated with an increased future risk of cardiovascular disease, irrespective of the antepartum screening protocol or diagnostic criteria by which gestational diabetes is diagnosed. We reasoned that the resultant heterogeneity in the severity of dysglycaemia in women with gestational diabetes suggests that the relationship between gestational glycaemia and subsequent cardiovascular disease probably extends into t...

Efficacy and safety of erythropoietin and iron therapy to reduce red blood cell transfusion in surgical patients: a systematic review and meta-analysis.
CONCLUSION: Administration of ESA and iron therapy reduced the risk for RBC transfusion compared with iron therapy alone in patients undergoing cardiac and non-cardiac surgery. Nevertheless, publication bias and heterogeneity reduces the confidence of the finding. Although the analysis was probably under-powered for some outcomes, no difference in the incidence of serious adverse events was observed with ESA and iron compared with iron alone. Further large prospective trials are required to confirm these findings. PMID: 30924000 [PubMed - as supplied by publisher]

Statin Use in Primary Prevention; a Simple Trial Based Approach Compared to Guideline Recommended Risk Algorithms for Selection of Eligible Patients
ConclusionThe pragmatic HOPE-3 trial approach identifies in an ethnically diverse primary prevention population individuals at intermediate risk who benefit from statin therapy using simple clinical characteristics without the need for complex, currently used risk assessment tools.

Early complete revascularization in hemodynamically stable patients with ST-segment elevation myocardial infarction and multivessel disease
ConclusionsEarly complete revascularization should be considered in hemodynamically stable patients with STEMI and multivessel disease deemed suitable for percutaneous interventions. FFR-guided complete revascularization may be superior to angiography-guided procedure in reducing need for further interventions.

Comparison of Readmission and Death Among Patients With Cardiac Disease in Northern vs Southern Ontario
ConclusionsReadmissions were increased in those residing and hospitalized in the North. To reduce readmissions in the North, the quality of postacute transitional care should be examined further.RésuméContexteDes facteurs géographiques peuvent influer sur l’issue de maladies cardiovasculaires au Canada.

Evolution of blood pressure clinical practice guidelines: a personal perspective
Publication date: Available online 27 February 2019Source: Canadian Journal of CardiologyAuthor(s): Paul K. WheltonAbstractPrior to the second half of the 20th century, most clinical decision-making was based on expert opinion. By the 1960s, experience in actuarial and research cohort studies had provided strong evidence that blood pressure was an important risk factor for cardiovascular disease. The landmark 1967 and 1970 Veterans Administration Cooperative Study trials confirmed the value of antihypertensive drug therapy in preventing stroke, myocardial infarction, and heart failure in adults with a high level of diastol...

Featured Review: Mediterranean-style diet for the prevention of cardiovascular disease
In this Q&A we asked the lead author Professor Saverio Stranges from Western University Canada to explain more about the mediterranean diet and its role in preventing cardiovascular disease following the publication of aCochrane Review on this topic. What makes a diet ‘Mediterranean’?Scientific interest in the traditional Mediterranean dietary pattern originated in the 1960s because of the observation that populations in countries of the Mediterranean region, such as Greece and Italy, had lower mortality from cardiovascular disease compared with northern European populations or the US, probably as a result of diff...

Optimal duration of dual antiplatelet therapy following percutaneous coronary intervention: An umbrella review
ConclusionsExtended DAPT may reduce the risk of MI and stent thrombosis but increase major bleeding and death. Whether the effects of extended DAPT are consistent across patient subgroups is unclear, and future SRs should address this knowledge gap. PROSPERO: CRD42016047735.
