Long-term Prognosis of Patients Presenting First-ever Vestibular Symptoms in a Community-based Study
Conclusions: First-ever-in-lifetime VSs are common in patients with FNS and may represent a good opportunity for preventing a serious vascular event, particularly in patients with vascular comorbidity (silent infarctions and VRFs).

Early MRI-negative posterior circulation stroke presenting as acute dizziness
AbstractThe aim of this study was to determine the frequency, clinical and radiological features, and efficacy of clinical evaluation and perfusion-weighted imaging (PWI) for the prediction of final stroke in patients with DWI/MRI-negative posterior circulation stroke (PCS) presenting acute dizziness/vertigo. From our comprehensive prospective stroke registry of acute ischemic stroke during a 7-year period, we identified 1846 consecutive patients with PCS, 850 of whom presented with acute dizziness/vertigo. Of these 850 patients, initial DWI-MRI was negative in 35 (4.1%). In these 35 patients, dizziness/vertigo was acute p...

Relation of infarction location and volume to vertigo in vertebrobasilar stroke
ConclusionInfarction location in the cerebellum and/or dorsal brainstem is an independent predictor of vertigo. Furthermore, larger infarction volume in these structures is associated with vertigo. A considerable proportion of patients with vascular vertigo present without focal neurological deficits posing a diagnostic challenge. National Institute of Health Stroke Scale is not sensitive for vertebrobasilar stroke.

Diagnosing Stroke in Acute Vertigo: Sensitivity and Specificity of HINTS Battery in Indian Population
Conclusions: The HINTS battery was more sensitive than the initial MRI of the brain in diagnosing stroke in first 24 h in patients presented with acute-onset first episode of spontaneous vertigo.

Case Report: Right Insular Stroke Causing Simultaneous Onset of a Functional Vestibular Disorder and Psychiatric Disorder —Persistent Postural–Perceptual Dizziness and Post–stroke Depression
DiscussionPrior case reports described short-lived vertigo following insular strokes, but no long-term vestibular symptoms without ongoing nystagmus or gait ataxia. In this case, chronic dizziness and motion sensitivity continued in the absence of focal neurologic deficits, invoking the possibility that changes in functioning of brain networks subserving spatial orientation persisted despite otherwise adequate recovery from the stroke, a mechanism previously proposed for PPPD. This case also reinforced prior work implicating pathways through the insula in PSD. Co-occurrence of PPPD and PSD offers insights into simultaneous...

Missed Strokes Using Computed Tomography Imaging in Patients With Vertigo: Population-Based Cohort Study Clinical Sciences
Conclusions— One fifth of patients diagnosed with peripheral vertigo in Ontario received imaging that is not recommended in guidelines, and that imaging was associated with missed strokes.

Stroke-Like Presentation of Paraneoplastic Cerebellar Degeneration: a Single-Center Experience and Review of the Literature.
We report herein incidence and clinical features of hyperacute onset PCD, a vertebrobasilar stroke mimic. We performed a retrospective analysis of all suspected PCD cases referred to the Udine University Hospital between 2009 and 2017. Our center provides the only neuroimmunology laboratory for three provinces of the Friuli-Venezia Giulia region, Italy (983,190 people as of January 1, 2017). Inclusion criteria were (1) abrupt onset of neurological symptoms; (2) initial consideration of a vascular etiology; (3) final diagnosis of "definite PCD." We also carried out a systematic review of the literature in order to identify ...

A Dynamic Nomogram to Predict the Risk of Stroke in Emergency Department Patients With Acute Dizziness
ConclusionOur new prediction model exhibited good performance and could be useful for stroke identification in patients presenting with dizziness, vertigo, or imbalance. Further externally validation study is needed to increase the strength of our findings.

Identification of Stroke and TIA in Patients With Acute Dizziness, Vertigo or Imbalance in Emergency Departments of Primary Care Hospitals: Early Experiences With a Network-Based Telemedical Approach
DiscussionThe concept shows good acceptance for a telemedical and network-based approach to manage ADVIUC cases in the ED of primary care hospitals. Identification of stroke cases is accurate, while specificity needs further improvement. The concept could be a major step toward a broadly available state of the art diagnostics and therapy for patients with ADVIUC in primary care hospitals.

Clinical Features Differ Between Patients With Vertigo Attack Only and Weakness Attack Accompanying Vertigo Before Vertebrobasilar Stroke: A Retrospective Study
ConclusionsClinical parameters, such as infarction location, relative risk factors, and ABCD2 score, differed between patients with vertigo symptoms with or without weakness attacks. These findings highlight the different clinical features of patients with vertigo attack only and those with weakness attacks accompanying vertigo prior to vertebrobasilar ischemic stroke.

Simulation ‐based team‐training in acute stroke: Is it safe to speed up?
ConclusionsSimulation training of the acute stroke treatment team was associated with an increase in the proportion of patients treated with IVT for a suspected AIS who were later diagnosed with peripheral vertigo. The proportion of other SM groups among IVT-treated patients did not change significantly. No sICH was detected in IVT-treated SM patients.

Quantitative Video-oculography to Differentiate Stroke from Vestibular Neuritis in Acute Vertigo-Vestibulo-ocular Reflex Gain Distributions and Optimal Test Thresholds for Diagnosis (P6.289)
CONCLUSIONS: Peripheral and central causes of AVS can be differentiated by quantitative, portable video-oculography. Some AICA strokes mimic vestibular neuritis closely. Early Diagnosis of Posterior Fossa Stroke with Brainstem Evoked Potentials (I1-3B)
Conclusion: BAP is more sensitive for diagnostic purposes towards posterior fossa small strokes and should be considered in patients when the initial MRI brain is negative. This will allow for correct diagnosis and treatment regimen without causing a delay for the patient.Disclosure: Dr. Duggal has nothing to disclose. Dr. Ahmed has nothing to disclose. Enterobacter Cloacae Endocarditis Presenting as an Ischemic Stroke (P2.279)
CONCLUSIONS: This is the first case which demonstrates that ischemic stroke can be the presenting clinical event of E. cloacae endocarditis. Clinicians who provide acute care to patients with stroke should be aware of this rare cause of embolic brain infarction. Our case shows that early diagnosis of E. cloacae endocarditis and appropriate medical management can promote full neurological recovery in these patients.
