Thrombus-associated microbiota in acute ischemic stroke patients
CONCLUSION: In this small study, we demonstrate proof of concept and technical feasibility for amplicon-based metagenomic sequencing of arterial thrombi and briefly discuss preliminary findings, challenges, and near-term translational opportunities for thrombus genomics.PMID:35855159 | PMC:PMC9282817 | DOI:10.25259/SNI_104_2022

E-093 Carotid webs and stroke: a comprehensive overview of clinical characteristics, risk factors, and treatment modalities
ConclusionThis is the largest systematic review of CWs to date. This study provides novel information not only about risk factors and common treatment modalities for CWs, but also the relationship between CWs and stroke. This data may aid physicians in diagnosis of CWs in younger patients who present with stroke-like symptoms and no cardiovascular risk factors. Furthermore, medical treatment alone may not be as effective as stenting or surgical options.Disclosures N. Gupta: None. V. Kasulla: None. A. Shahbandi: None. J. Gendreau: None. R. Singh: None. N. E-189 Outcomes of tenecteplase versus alteplase after mechanical thrombectomy in a multiethnic stroke consortium registry
ConclusionIn this prospective, multi-center stroke consortium study, IV TNK in comparison to tPA prior to MT for ischemic stroke resulted in non-inferior outcomes related to angiographic reperfusion and functional status at discharge. These findings compliment the current literature and include a large Hispanic US cohort. Further analysis will include expansion of contributing centers and use of propensity scoring.Abstract E-189 Table 1 and 2Disclosures M. Gaub: None. D. Ramaswamy: None. A. Cardentey: None. S. 64-detector CT angiography within 24 hours after carotid endarterectomy and correlation with postoperative stroke.
CONCLUSIONS CTA within 24 hours of CEA demonstrates characteristic anatomical findings. CCA step-offs and ECA flaps are relatively common and clinically insignificant, whereas ICA/CCA flaps and thrombi are less frequently seen and are associated with postoperative stroke/TIA. Outcomes and Prognostic Factors After Emergent Carotid Artery Stenting for Hyperacute Stroke Within 6 Hours of Symptom Onset
This study sought to evaluate clinical outcomes and prognostic factors after carotid artery stenting (CAS) in patients with hyperacute stroke within 6 hours of onset. METHODS: Forty-seven patients with hyperacute stroke attributable to atherosclerotic C-ICA occlusion underwent emergent CAS. Forty-two patients (89.4%) had tandem intracranial artery occlusion (TIO). When patients showed remnant M1 or proximal M2 occlusions after CAS, intracranial recanalization therapy was performed by using pharmacologic thrombolysis and mechanical thrombectomy with a Solitaire stent. Clinical and radiologic data were compared between patie...

E-039 Comparison of Medical Therapy vs. Intra-Arterial Therapy for Acute Vertebrobasilar Stroke
ConclusionCompared to medical trials in 2015, which demonstrate clinical benefit for IAT over medical therapy for acute ischemic stroke of the anterior circulation, the posterior circulation is less well studied. Important prognostic factors for stroke due to large vessel occlusion of posterior circulation include posterior ASPECTS score and infarct volume in the brainstem. In this small retrospective study, patients treated with IAT had greater stroke severity than the medical cohort, yet they had similar rates of good functional outcome. Parameters of this retrospective analysis may be used to improve triage and treatmen...

E-065 "Rescue" Intracranial Stenting for Refractory Large Vessel (Re)occlusion in Acute Ischemic Stroke
ConclusionIntracranial stent placement for refractory large vessel (re)occlusion in acute ischemic stroke is feasible with high technical success rate, however, this is not always associated with favorable clinical outcome. If refractory vessel occlusion or reocclusion is encountered during acute stroke intervention, early stent placement and recanalization may lead to more favorable outcome. Verification in a larger cohort is necessary.DisclosuresG. Toth: None. S. John: None. M. Elgabaly: None. M. Hussain: None. M. Bain: None.
