Recanalized Left Internal Carotid Artery Bifurcation Aneurysm with Adherent Thrombus at Aneurysm Neck and M1 Origin Diagnosed on 2-Dimensional Angiography —Cause of Embolic Stroke
We report a unique case of recanalized left internal carotid artery aneurysm with thrombus adherent to the aneurysm neck and M1 origin diagnosed on 2-dimensional angiography causing embolic stroke in a 66-year-old female who was successfully treated with stent-assisted coiling. This is important to recognize as a cause of embolic stroke.

P-042 Initial experience with tenecteplase as the intravenous thrombolytic of choice before mechanical thrombectomy for large vessel occlusion acute ischemic stroke
ConclusionIntravenous TNK in patients with LVOAIS is feasible, safe and effective. In our cohort from ‘real world’ patients, TNK was associated with lower need for MT, high recanalization rates and shorter hospitalization.Disclosures U. Ugur: None. S. Platko: None. D. Peters: None. E-095 The embotrap stent retriever for treatment of acute ischemic stroke: Impact of hospital procedural volume on real world outcomes
ConclusionsHospital MT volume significantly influenced the rate of readmissions among AIS patients treated with the EmboTrap® (alone or with another MT device) device. The lack of difference in mortality may also be reflective of limited statistical power to detect differences in study groups. Further research to understand the impact of hospital MT volume on patient outcomes, as well as identify the threshold volume that optimizes positive outcomes, could help inform future stroke care guidelines and contribute to improved treatment protocols in low-volume centers.Disclosures S. Thrombectomy for secondary distal, medium vessel occlusions of the posterior circulation: seeking complete reperfusion
Conclusion Thrombectomy for secondary posterior circulation DMVO seems to be safe and technically feasible. Even though thrombi that have migrated preprocedurally may be easier to retract, successful reperfusion can be achieved in the majority of patients with secondary DMVO of the P2 and P3 segment.

Stroke outcome of early antiplatelet in post-thrombolysis haemorrhagic infarction
Conclusions Antiplatelet therapy can be safely used between 24 and 48 hours when HI occurs after IVT, and such therapy is associated with reduced risk of END and improved neurological outcome in patients who had an AIS.

E-055 Internal carotid artery reconstruction with flow diverting stents in the treatment of acute ischemic stroke: technical considerations and medical management
ConclusionsFDS appear to be a safe and effective tool for reconstruction of a symptomatic dissection in tortuous cervical ICAs. Their flexibility allows conformation to complex anatomies and they can be traversed multiple times by aspiration catheters/stentrievers. We found excellent patency at follow up. The unique challenges posed by utilization of high-profile stents in acute stroke require appropriate technical execution and medical management.Disclosures E. Orru: None. F. Bounni: None. M. Concurrent asymptomatic inflammatory aneurysm and ischemic stroke due to cysticercal arteritis
Neurocysticercosis (NCC) is recognized as an important public health problem in developing countries. Cysticercal arteritis (CA) may cause endothelial disruption, edema, thrombosis, and the formation of fusiform aneurysms due to a weakening of the vessel walls . Here, we report a case with concurrent asymptomatic internal carotid artery (ICA) inflammatory aneurysm and contralateral ischemic stroke due to CA.

Validation assessment of risk tools to predict outcome after thrombolytic therapy for acute ischemic stroke
Thrombolytic therapy generally improves clinical outcome after acute ischemic stroke, but the decision to administer thrombolytics may be challenging because of hemorrhagic complications and poor response in some patients [1–4]. Many models for risk stratification of thrombolytic treatment have been developed to support the clinician in this complex decision-making process.

Hounsfield unit value and clot length in the acutely occluded vessel and time required to achieve thrombectomy, complications and outcome
Conclusions We have not found any significant associations between either thrombus length or density and likelihood of recanalization, time to achieve recanalization, intraprocedural complications, postprocedural hemorrhage or functional outcome at 90 days. These results do not support a predictive value for thrombus quantification in the evaluation of AIS.
