Acute Stroke After Carotid Endarterectomy: Time for a Paradigm Shift? Multicenter Experience With Emergent Carotid Artery Stenting With or Without Intracranial Tandem Occlusion Thrombectomy
CONCLUSION: Emergent endovascular evaluation in the setting of acute post--carotid endarterectomy thrombosis is a safe and timely treatment option, with the benefit of detecting and treating embolic intracranial lesions. Immediate angiography and intervention in this rare surgical complication show promising initial results. A novel model of large vessel ischemic stroke in rabbits: microcatheter occlusion of the posterior cerebral artery
Conclusions Image-guided microcatheter induction of PCA occlusion in rabbits can consistently produce time-dependent infarction of cortical and subcortical structures that is reliably detected by diffusion-weighted MRI, and thus may be a useful model for therapeutic studies in acute ischemic stroke.

Posterior circulation flow diversion: a single-center experience and literature review
Conclusions Flow diversion may be a possible treatment in carefully selected patients with high-risk atypical posterior circulation aneurysms, with poor natural history and no optimal treatment strategy. Symptomatic and fusiform large aneurysms appear to carry the highest risk. Further studies are necessary to assess the role of flow diversion in the posterior circulation.

E-003 delayed reocclusion of recanalized intracranial arteries after mechanical thrombectomy in acute stroke patients
ConclusionsWe reviewed these patients and found that delayed reocclusion of recanalized vessels were noted within 24 h after first mechanical thrombectomy. The factors contributed to these results are assumed as TICI grade before and after the first mechanical thrombectomy. But, further evaluation of factors such as NIHSS score, underlying disease, laboratory findings and follow-up angiogram are needed. Also, procedural complications as vasospasm, dissection and hemorrhage should be investigated.DisclosuresK. Jang: None. S. Park: None. D. Jang: None. B. Moon: None. Y. Han: None. Y. Park: None.

E-033 variety of clot types and their implication on technique during endovascular stroke therapy
ConclusionOur data indicate that thrombectomy with a stent-retriever and balloon guide system provides the best opportunity for good outcomes. Because of the risks of shearing off part or all of a clot when aspirating an oversized thrombus into an intermediate catheter and further retrieving against the flow in the internal carotid, our strategy is to use intermediate catheters in conjunction with a balloon guide whenever possible to achieve flow-arrest. We did not have a large enough sample size to draw conclusions about the impact of technique on the number of passes. Further evaluation of how to optimize techniques in d...

Stent retriever thrombectomy with the Cover accessory device versus proximal protection with a balloon guide catheter: in vitro stroke model comparison
Conclusions Application of the Cover device in this experimental model resulted in higher successful recanalization rates, no embolic events, and was more effective than use of the conventional guide catheter or BGC.

E-025 First Reported Case of Mechanical Thrombectomy for Acute Ischemic Stroke in an Individual with a Total Artificial Heart
ConclusionEVT has been proven to be safe and efficacious for AIS. Prior reports have demonstrated benefit for patients with concurrent mechanical circulatory support devices; however, our report is the first case demonstrating efficacy with a TAH device. Presence of mechanical circulatory support devices should not exclude EVT in individuals with AIS.DisclosuresM. Nezhad: None. P. Eboli: None. M. Austin: None. K. Schlick: None. M. Alexander: None.

The clinical management of cerebral venous sinus thrombosis. a uk survey
Conclusion Overall respondents were confident in the use of heparin for the treatment of CVST. Respondents used LMWH predominantly, in keeping with EFNS guidance, yet in contrast to a previous European survey.1

Neck vessel dissection following whiplash-type neck injury
Arterial dissection is a recognised complication of neck manipulation and trauma. In a systematic audit of 148 complex medicolegal whiplash cases 131 had only neck pain, 8 had radiculopathic symptoms, and 9 had symptoms and signs suggesting an arterial dissection of a neck vessel with presumed secondary thromboembolism. Their ages ranged from 28–55. Three had hemipareses supported by scan abnormalities, and one had a probable posterior fossa stroke. Five cases had prolonged symptoms without imaging abnormalities. This series cannot be considered representative of whiplash cases as a whole. The dissection is necessari...

Experimental evaluation of stent retrievers mechanical properties and effectiveness
Conclusions None of the tested devices were effective in removing white clots of large diameter (≥6 mm). Constant radial force during retrieval allows constant cohesion to the vessel wall and pressure over the clot; such features allow for a higher rate of clot removal.

Successful recanalization in acute basilar artery occlusion treated with endovascular therapy is independent of thrombus length
Conclusions Successful recanalization does not depend on thrombus length in patients with BAO treated with EVT. Recanalization can therefore be achieved despite high clot burden. Additionally, a high density of thrombi was a strong predictor of a favorable outcome.

Endovascular mechanical thrombectomy for cerebral venous sinus thrombosis: a systematic review
Conclusions EMT is an effective salvage therapy for refractory CVST, with a reasonable safety profile. Chemical thrombolysis, in conjunction with EMT, did not appear to result in additional harm or benefit. Further analysis is warranted to determine predictors of success after EMT for CVST.

E-018 Post endovascular treatment of acute vertebro-basilar stroke: anticoagulation versus antiplatelet therapy
Conclusion Preliminary analysis of this ongoing systematic review did not show any differences in outcome or hemorrhagic complications in patients with AVBS on APT versus ACT post intervention. Either modality of treatment was superior to no treatment with a higher risk of hemorrhagic complications. Disclosures M. Weber: None. N. Khan: None. F. Siddiqui: None. J. Blessman: None. A.
