E-187 Pipeline embolization stent for treatment of giant supraclinoid aneurysms: a case series
ConclusionThe Pipeline embolization device is a novel technique that can be utilized for treatment of giant cerebrovascular aneurysms with emerging evidence of immediate and long-term success. Further patient enrollment and clinical trials will be required to assess its efficacy in comparison to other endovascular and surgical approaches for the treatment of giant aneurysms.Disclosures M. Pre-admission antithrombotic use is associated with 3-month mRS score after thrombectomy for acute ischemic stroke
AbstractIn patients who undergo thrombectomy for acute ischemic stroke, the relationship between pre-admission antithrombotic (anticoagulation or antiplatelet) use and both radiographic and functional outcome is not well understood. We sought to explore the relationship between pre-admission antithrombotic use in patients who underwent thrombectomy for acute ischemic stroke at two medical centers in New York City between December 2018 and November 2020. Analyses were performed using analysis of variance and Pearson ’s chi-squared tests. Of 234 patients in the analysis cohort, 65 (28%) were on anticoagulation, 64 (27%) we...

Safety of early posterior fusion surgery without endovascular embolization for asymptomatic vertebral artery occlusion associated with cervical spine trauma
ConclusionDespite the lack of a concurrent control group with preoperative antiplatelet therapy or endovascular embolization for VAO, our results showed low symptomatic stroke rate (4.5%), high recanalization rate (40.9%), and low mortality rate (0%). Therefore, we believe that the indication for early stabilization surgery as management strategy of asymptomatic VAO might be one of the safe and effective treatment options for prevention of symptomatic cerebral infarction.

Clot Meniscus Sign Is Associated With Thrombus Permeability and Choice of Mechanical Thrombectomy Technique in Acute Middle Cerebral Artery Occlusion
ConclusionPatients with MCAO with positive meniscus sign have higher thrombus permeability and are more suitable for CA to acquire better clinical outcomes.

COVID-19 and supra-aortic trunks disease: review of literature about critical phase and sequelae
J Cardiovasc Surg (Torino). 2021 Sep 28. doi: 10.23736/S0021-9509.21.12021-X. Online ahead of print.ABSTRACTThe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is responsible for the COVID-19 disease, a global pandemic. A strong association has been documented between COVID-19 and cardiovascular events, although the exact pathophysiological mechanism is still unclear. Carotid atherothrombosis and ischemic stroke represents one of the possible severe manifestations of COVID-19, as a leading cause of long-term disability and death. Different complex intertwined mechanisms seem to underlie the endothelitis which is the ...

Antimicrobial Protein REG3A and Signaling Networks are Predictive of Stroke Outcomes
AbstractRegenerating Family Member 3 Alpha (REG3A) is a multifunctional protein with antimicrobial activity, and primarily secreted by the intestine and pancreas. Studies have shown an increased expression of REG3A in systemic inflammatory responses to acute injury and infection, but studies investigating REG3A during the pathogenesis of ischemic stroke are limited. The aims of this study were to examine the associations between arterial expression of REG3A and other arterial inflammatory proteins implicated in stroke pathogenesis, as well as associations between REG3A and markers of poor outcome for ischemic stroke. The U...

LB-005 The INSIGHT registry: a multi-center, multi-omic stroke clot study, preliminary results
ConclusionThese preliminary results demonstrate the real-world applicability of the INSIGHT study to typical stroke thrombectomy population. Furthermore, the tissue processing protocols are providing high-quality RNA and protein for analysis, which is currently underway.DisclosuresJ. Routine Stent-Bridging to the Supraaortic Vessels in Aortic Arch Replacement - 10 year-experience
CONCLUSIONS: SAVSTEB is a comparatively simple, safe and efficacious technique to create the anastomosis between tetrabranched arch grafts and the supra-aortic arteries in the short and intermediate term. Bleeding from the anastomoses, kinking and scar associated stenosis are negligible; however, vertebral overstenting remains a critical technical issue.PMID:34186091 | DOI:10.1016/j.athoracsur.2021.05.074

Hemorrhagic Transformation and Stroke Recurrence in Children with Cardiac Disease Receiving Antithrombotic Therapy for Secondary Stroke Prevention
CONCLUSIONS: In spite of the 24% hemorrhagic transformation rate, antithrombotic therapy has a positive risk-balance in certain cardioembolic stroke subgroups, particularly in those with single-ventricle physiology, when accounting for stroke volume.PMID:34152075 | DOI:10.1111/jth.15428

Telemedicine remote controlled stroke evaluation and treatment, the experience of radiographers, paramedics and junior doctors in a novel rural stroke management team
CONCLUSIONS: The telemedicine-based, remote controlled, stroke evaluation and treatment was experienced, by the participants, to be well organised and of high quality. Communication and image reading appear to be the salient challenges. Regular training sessions and follow-up, as well as an evaluation of incidents by the project manager, proved to be of great importance in retaining and securing the continued running of the service and ensuring high-quality treatment. Further research is indicated in the comparison of this telemedicine service with stroke treatment given in a mainstream hospital.PMID:34090447 | DOI:10.1186/s12913-021-06591-1
