Diagnosis of atrial fibrillation in young patients with ischemic stroke: A systematic review and meta-analysis
The recommended cardiac rhythm evaluation to determine the etiology of ischemic stroke (IS) is similar in all patients regardless of their age and includes an electrocardiogram and at least a 24-hour heart rhythm monitoring. However, it is known that the main causes of IS vary according to patients ’ age. There is a higher preponderance of arterial dissections and patent foramen ovale in younger patients, while atrial fibrillation (AF) is more common in older patients.

Modified risk of paradoxical embolism more effectively evaluates the risk of stroke associated with patent foramen ovale
CONCLUSION: LDL-C levels and transverse diameter of the left atrium were identified as independent risk factors for PS. Risk of recurrent ischemic stroke in patients with patent foramen ovale: The role of D-dimer
Paradoxical embolism under elevated thromboembolic conditions is known to be the primary mechanism of patent foramen ovale (PFO)-related stroke. We hypothesized that higher levels of D-dimer, a marker of thromboembolism, could increase the risk of stroke recurrence in patients with PFO.

Synchronous multimode ultrasound for assessing right-to-left shunt: a prospective clinical study
ConclusionSynchronous multimodal ultrasound significantly improves the detection rate and test efficiency, quantifies RLS more accurately, and reduces testing risks and medical costs. We conclude that synchronous multimodal ultrasound has significant potential for clinical applications.

Patent foramen ovale closure in non-elderly and elderly patients with cryptogenic stroke: a hospital-based cohort study
DiscussionPFO closure was associated with a reduced risk of the primary outcome in patients with PFO and CS in our total cohort, which included non-elderly and elderly patients. Compared to those without PFO closure, elderly patients with PFO closure had a better functional outcome at 180 days. PFO closure might be considered in selected elderly patients with PFO.

Detection of patent foramen ovale in patients with ischemic stroke on prospective ECG-gated cardiac CT compared to transthoracic echocardiography
ConclusionProspective ECG-gated cardiac CT acquired during the acute stroke imaging protocol does not appear to be a suitable screening method for PFO due to its low sensitivity. Our data suggest that if cardiac CT is used as a first-line screening method for cardioembolism, additional echocardiography remains indicated in young patients with cryptogenic stroke, in whom PFO detection would have therapeutic consequences. These results need to be confirmed in larger cohorts.

Percutaneous atrial shunt closure and the risk of recurrent ischemic stroke: A register-based, nationwide cohort study
Patent foramen ovale may cause paradoxical emboli1. Cryptogenic ischemic cerebrovascular events (stroke or transient ischemic attack) (CVE) accounts for about 25% of ischemic strokes.2,3 Transcatheter closure of atrial shunts, including atrial septal defects (ASD) and patent foramen ovale (PFO), was introduced in 1996.4-6 Initially,randomized studies failed7-9 but more recent randomized studies have shown superiority of interventional treatment, with 68 % reduction of recurrent stroke after transcatheter closure of PFO compared with medical treatment in patients with cryptogenic CVE.
