Flat Detector Angio-CT following Intra-Arterial Therapy of Acute Ischemic Stroke: Identification of Hemorrhage and Distinction from Contrast Accumulation due to Blood-Brain Barrier Disruption INTERVENTIONAL
CONCLUSIONS: Flat panel CT after endovascular stroke treatment was able to exclude the rare event of an intracranial hemorrhage with a high negative predictive value. Future studies should evaluate the predictive value of BBB disruptions in flat panel detector CT for the development of relevant hematomas.

Ctbrain machine learning predicts stroke thrombolysis result
Conclusions This proof-of-concept study shows that machine learning methods applied to acute stroke CT-scans potentially offers automation, and improved performance in SICH prediction following thrombolysis. Larger-scale cohorts, and incorporation of CT perfusion/angiography data, should be tested with such methods.

E-074 Emergency Carotid Artery Stenting in Acute Ischemic Stroke
Conclusion Endovascular recanalization treatment with emergency carotid artery stenting is technically feasible. Clinical, angiographic and radiographic outcome differences among mechanical thrombectomy devices: initial experience of a large-volume center
Conclusions Although our initial data confirm the superiority of SR technology over the Merci device, there was no significant difference in near complete/complete reperfusion, final infarct volumes or clinical outcomes between SR and Penumbra thromboaspiration.

Unprotected carotid artery stenting in symptomatic elderly patients: a single-center experience
Conclusions Endovascular treatment of carotid stenosis without protection devices in symptomatic patients aged >75 years is an alternative to endovascular treatment with protection devices. Complications and mortality rates are similar to studies that used protection devices in lower risk patients.

E-095 what are the risk factors and outcomes relating to hemorrhagic transformation with large vessel occlusion in the anterior circulation at a comprehensive stroke center?
ConclusionMT is associated with an increased rate of HI hemorrhages but not PH hemorrhages. HI hemorrhages have no correlation with death or mRS. mRS may be too insensitive to detect deficits relating to HI hemorrhages, NIHSS should be used in studies investigating clinical outcomes relating to HI hemorrhages. BG involvement may correlate with both HI and PH hemorrhages in patients treated with and without MT. However, having BG involvement does not seem to correlate with a worse prognosis in MT patients with any type of hemorrhage.DisclosuresG. Bennett: None. M. Al Hasan: None. G. Vidal: 3; C; Penumbra. Isolated Posterior Inferior Cerebellar Artery Dissection as a Cause of Ischemic Stroke: Clinical Features and Prognosis
Background: The purpose of this study was to clarify the incidence, clinical and radiological features and outcomes of isolated posterior inferior cerebellar artery (PICA) dissection in isolated PICA territory infarctions. Methods: We retrospectively reviewed consecutive inpatients with ischemic stroke secondary to isolated PICA dissection from our stroke database between January 2004 and December 2013 and reviewed the literature with regard to those patients. Results: Of 167 consecutive patients with an isolated PICA territory infarction, a total of 10 patients (6.0%, 3 women, 48.1 ± 7.1 years) were diagnosed as having a...

Extensive stretching of intracranial aneurysm coil causing TIAs
A 58-year-old man with a history of ruptured posterior cerebral artery aneurysm, repaired with coil embolization 5 years previously, presented with right hemi-numbness lasting 30 minutes. Cerebral MRI did not reveal acute stroke. CT angiogram showed a stretched wire complex extending through the posterior communicating artery, down the carotid artery, and into the most inferior visualized portion of descending aorta, terminating in a loosely coiled wire (figure, A and B). A transesophageal echocardiogram showed evidence of a possible thrombus on the coil in the aortic arch (figure, C and D; video on the Neurology® Web ...

Emergency Stenting of the Extracranial Internal Carotid Artery in Combination with Anterior Circulation Thrombectomy in Acute Ischemic Stroke: A Retrospective Multicenter Study EXTRACRANIAL VASCULAR
CONCLUSIONS: Emergency stent placement in the extracranial internal carotid artery in combination with anterior circulation thrombectomy is effective and safe. It is not associated with a significantly higher risk of symptomatic intracranial hemorrhage compared with published series for mechanical thrombectomy alone.

Intravenous thrombolysis, mechanical embolectomy, and intracranial stenting for hyperacute ischemic stroke in a patient with moyamoya disease
Publication date: Available online 17 February 2016 Source:Journal of Clinical Neuroscience Author(s): Debora S. Argetsinger, Jeffrey W. Miller, Jeffrey J. Fletcher The Japanese translation of moyamoya means “puff of smoke” and refers to the angiographic appearance of dilated collateral vessels seen during chronic progressive narrowing of the intracranial supraclinoid portions of the internal carotid arteries. Despite cerebral ischemia being the most common presenting symptom, 20% to 40% of adults suffer a hemorrhagic stroke. Due to the lack of evidence and histopathologic findings, intravenous and endovascular re...

Aetiology of Stroke in a Young Cohort: A Longitudinal Study (P1.112)
Conclusions: This descriptive study reinforces the different aetiologies encountered in strokes in those under age 65 years. In this group, especially those under 45 years, large vessel atherosclerosis accounts for a smaller proportion and more unusual aetiologies must be sought with more extensive haematological and cardiological investigations.Disclosure: Dr. Lockhart has nothing to disclose. Middle cerebral artery fusiform aneurysm presented with stroke and delayed subarachnoid hemorrhage trapping, thrombectomy, and bypass.
CONCLUSION: Our case describes a complex aneurysm treatment that consisted of aneurysm trapping, thrombus removal and an STA-M3 MCA branch bypass creation for the protection of the patent M3 insular MCA branch and prevention of further ischemia. This procedure rewarded us with an excellent clinical result. PMID: 27127709 [PubMed]

Chapter 66 Endovascular treatment of acute ischemic stroke
Publication date: 2016 Source:Handbook of Clinical Neurology, Volume 136 Author(s): Thabele Leslie-Mazwi, James Rabinov, Joshua A. Hirsch Endovascular thrombectomy is an effective treatment for major acute ischemic stroke syndromes caused by major anterior circulation artery occlusions (commonly referred to as large vessel occlusion) and is superior to intravenous thrombolysis and medical management. Treatment should occur as quickly as is reasonably possible. All patients with moderate to severe symptoms (National Institutes of Health stroke scale >8) and a treatable occlusion should be considered. The use of ...
