Long-term outcomes after conservative and EDAS treatment for 111 elderly patients with moyamoya disease: longitudinal and cross-sectional study
CONCLUSIONS: The postoperative long-term stroke rate among elderly patients with MMD was lower in the EDAS group than in the conservative treatment group. Long-term stroke events were associated with advanced age, aneurysm, and initial ischemia after conservative treatment and only initial hemorrhage after EDAS.PMID:37724788 | DOI:10.3171/2023.7.JNS231060

Management of aneurysmal recurrence after Woven EndoBridge (WEB) treatment
Conclusion The retreatment rate after an initial WEB treatment seems to compare favorably with that of coiling. Endovascular treatment of recurrence following WEB implantation is feasible in most situations; it generally requires the use of a stent and leads to a high rate of satisfactory occlusion.

Direct puncture of the superior ophthalmic vein for carotid cavernous fistulas: a 21-year experience
Conclusion Direct SOV cannulation to treat CCFs is safe and effective. Although it is typically used after other endovascular approaches have failed, SOV access for CCF treatment may be warranted as a first-line treatment for select patients.

Relationship between cerebral vasospasm vascular territory and functional outcome after aneurysmal subarachnoid hemorrhage
Conclusions In aneurysmal SAH, left-sided vasospasm is associated with worse discharge functional status. At 6 months, both ACA and BA vasospasm are associated with unfavorable functional status.

Selecting stroke patients for thrombectomy: is CTA+ASPECTS enough?
Since 2014, a series of randomised controlled trails (RCTs) have demonstrated the benefit of endovascular thrombectomy (EVT) on functional outcome for the participants who had acute ischaemic stroke (AIS) with occlusion of a proximal intracranial artery in the anterior circulation. The characteristics of the people enrolled in these RCTs have been used to define recommended criteria to select patients for thrombectomy in clinical practice. The initial group of RCTs largely enrolled participants early after AIS (mostly under 6 hours) who had National Institutes of Health stroke scale scores ≥6 (range 0–42 with lowe...

Spontaneous middle cerebral artery dissection: a series of six cases and literature review
AbstractMiddle cerebral artery (MCA) dissection is rare, and various clinical presentations, including hemorrhage, ischemia, or comorbidities, and the changes in imaging findings over time hinder treatment decisions. The European Stroke Organization guidelines exclude MCA dissection. Few cases have been reported with no review of the relevant literature. Therefore, we reviewed the relevant literature and our own experience with non-traumatic MCA dissection cases to determine appropriate treatment strategies. At our institution and affiliated institutions, we encountered six cases of MCA dissection —five with infarction a...

Stroke imaging modality for endovascular therapy in the extended window: systematic review and meta-analysis
Conclusions Our study suggests that, in anterior circulation LVO, the rates of functional independence may be similar when patients are selected using ADVI or NCCT for EVT in the extended time window. A simplified triage protocol does not seem to increase mortality or sICH. Protocol registration number (PROSPERO ID: CRD42021236092).

P064/122 Treatment of intracranial aneurysms with flow diversion in a real-world scenario - 10-year single center cohort
ConclusionOur series suggests that treatment with FD is feasible in a wide spectrum of cases, including aneurysms in different locations, sizes and morphology, ruptured and non-ruptured, reflecting a real-world scenario. Although available devices may differ, they seem to demonstrate comparable and adequate safety and effectiveness.Disclosure of InterestNothing to disclose

P067/132 Cerebral vasospasm treatment using comaneci device - registry study
ConclusionThe Comaneci device has the potential to serve as a first-line device for providing therapeutic benefit to patients with SAH and vasospasm. However, future prospective trials are required.Disclosure of InterestNothing to disclose

P077/180 Silk vista baby flow diverter stent for unruptured distal brain aneurysms: a Brazilian retrospective observational study
ConclusionThe SVB has high rates of technical success and an acceptable safety profile. Distal aneurysms may occlude slower due to relative oversizing of the devices and coils could help to improve the occlusion rate.Disclosure of InterestNothing to disclose

P089/268 Safety and efficacy of the fourth-generation Acandis Acclino Flex Plus stent in managing complex intracranial aneurysms
ConclusionThe AFP stent demonstrated feasibility and favourable safety and efficacy in the treatment of complex aneurysms. These results support its clinical use and highlight the need for further studies to evaluate its performance in different aneurysm subsets and to determine its specific role in the endovascular treatment of aneurysms.Disclosure of InterestCK serves as consultants for Acandis GmbH (Pforzheim, Germany) and proctor for MicroVention Inc./Sequent Medical (Aliso Viejo, CA, USA). P005/44 Using the pEGASUS-stent with antithrombogenic properties and single antiplatelet therapy in a case of thrombectomy
IntroductionRescue stenting is used more and more in stroke cases with large-vessel-occlusion or medium-vessel-occlusion refractory to mechanical thrombectomy. The pEGASUS-stent(Phenox) represents a device which is equipped with a hydrophilic polymer coating(HPC) with antithrombogenic properties. The coating allows for implantation under single antiplatelet therapy, thus possibly reducing the risk of bleeding in acute stroke cases.Case HistoryA 79-year-old woman was rushed to emergency room due to left-sided hemiparesis and National-Institutes-of-Health-Stroke-Scale(NIHSS) of 14. Computed-tomography-image showed an occlusi...

P008/85 Transarterial embolization (including hypoglossal artery) of dural arteriovenous fistula with multiple venous varices: Case report
ConclusionDAVF with venous varix is defined as high risk for venous hypertension or hemorrhage. There is a possibility of lower CN injury during embolization for hypoglossal artery, so accurate knowledge of anatomical structures is essential. Transarterial embolization for DAVF with large venous varix is a safe and considerable treatment option, even when fed by hypoglossal artery.Disclosure of InterestNothing to disclose
