Abstract Approximately 80% of patients with rheumatoid arthritis (RA) suffer from comorbidities including more than 50% from cardiovascular (CV) diseases. Inflammatory activity is the main factor connecting RA with atherosclerosis, coronary heart disease, stroke, thromboembolic events and heart failure. Altogether these affect RA patients twice as frequently as the general population and CV events are the major cause of death in RA. Ibudilast inhibits chemokine expression in rheumatoid synovial fibroblasts and exhibits immunomodulatory activity in experimental arthritis
ConclusionIbudilast reduced the expression and/or secretion of inflammatory mediators from activated human leukocytes and RASF, inhibited Th17 responses in vivo and improved established arthritis. Given the established safety profile of ibudilast in man, its clinical evaluation in RA, either alone or in combination with a TNF inhibitor, should be considered.This article is protected by copyright. Cardiovascular (CV) Risk after Initiation of Abatacept versus TNF Inhibitors in Rheumatoid Arthritis Patients with and without Baseline CV Disease.
CONCLUSION: In this large cohort of publicly or privately insured patients with RA in the United States, ABA was associated with a 20% reduced risk of CVD versus TNFi. Is Rheumatoid Arthritis a Cardiovascular Risk Equivalent to Diabetes?
ConclusionFindings from the present study suggest that while CVD risk in RA is elevated, it is lower in magnitude compared to the CVD risk associated with diabetes. Association Between Glucocorticoid Exposure and Healthcare Expenditures for Potential Glucocorticoid-related Adverse Events in Patients with Rheumatoid Arthritis.
