This study aims to determine whether CYP2C19 loss-of-function (LoF) variants were associated with long-term ischemic stroke risk in Chinese primary care patients treated with clopidogrel. Intracranial atherosclerotic stenosis (ICAS) is one of the main prevalent causes of ischemic stroke globally.1,2 However, even with aggressive medical management, the annual rate of stroke recurrence remains high among individuals with multiple risk factors and severe stenosis.2 Moreover, ICAS has been connected to an elevated risk of mild cognitive impairment and dementia3, which confers an extra necessity for the identification of risk factors associated with ICAS.
