Cerebral Venous Thrombosis Mimicking Acute Ischemic Stroke in the Emergency Assessment of Thrombolysis Eligibility: Learning from a Misdiagnosed Case
CONCLUSION: Patients with CVT have a higher risk of thrombolysis-related intracranial hemorrhage than other stroke mimics. A greater focus on noncontrast brain CT and the venous phase of CT angiography help identifying this stroke mimic before thrombolysis.PMID:34841501

COVID-19 Infection and Recurrent Stroke in Young Patients With Protein S Deficiency: A Case Report
Conclusions: SARS-CoV-2 infection potentializes the prothrombotic effect and vascular inflammation by accentuating protein S deficit. The place of steroids seems justifiable in the presence of symptoms of vasculitis in brain imaging.

EP65 Mechanical thrombectomies, initial experiences from Kosovo Case series presentation
ConclusionEndovascular treatment has proven to be safe and efficacious for AIS. Our patients recovered and were discharged within 7–11 days after ictus.mRS after 90 days 0 at two (male) patients,mRS 1 at two patients and one patient is mRS 3.These case series demonstrate the clinical efficacy, safety, and favorable clinical outcome of first mechanical thrombectomies performed in Kosovo.ReferencesHeider D, Simgen A, Wagenpfeil G, et al. Recurrent Transient Neurological Deficit Due to Intracerebral Steal Phenomenon in Association with a Developmental Venous Anomaly
We report a symptomatic developmental venous anomaly (DVA) not causing parenchymal abnormality to provide a pathophysiologic clue in patients with recurrent transient neurologic deficit. A 32-year-old male presented with recurrent transient motor aphasia and headache in the left fronto-temporal region for three years. The symptoms usually lasted for one hour. Brain computed tomography (CT) angiography and magnetic resonance imaging using gradient recalled echo showed a prominent penetrating vein at the left frontal periventricular region.

Modified Prehospital Acute Stroke Severity (mPASS) Scale to Predict Emergent Large Arterial Occlusion
CONCLUSIONS: The mPASS scale had a better discrimination for identifying ELVO than the PASS scale in our retrospective cohort. It might predict ELVO in an effective and simple way for paramedics in the prehospital triage stage or emergency stage. Moreover, cortical symptoms might have relatively high specificities to predict ELVO on their own.PMID:34337028 | PMC:PMC8315848 | DOI:10.1155/2021/5568696

E-057 Prospective prehospital evaluation of VAN and RACE large vessel occlusion prediction instruments on a mobile stroke unit
ConclusionsBoth VAN and RACE demonstrated measurable but limited benefit in predicting LVO in the field. In patients with LKW times of ≤1.5 hours, RACE showed a high NPV of 0.96. In this subset of patients, RACE could be used to rule out LVO in the prehospital setting, avoiding delays in care due to unnecessary transport to a thrombectomy-capable facility.Disclosures J. Contrast-Induced Encephalopathy after Cerebral Angiogram: A Case Series and Review of Literature
We present 4 individual cases of CIE who presented with stroke-mimic symptoms following surgery with localized iodixanol or ioversol injection. We outline a clinical timeline of all patients, showing that CIE follows a general pattern of delayed onset, worsening symptomology, and ultimately full recovery. All patients received IV hydration, corticosteroids, or both as part of their treatment protocol.Case Rep Neurol 2021;13:405 –413

Thrombectomy for ischemic stroke with large vessel occlusion and concomitant subarachnoid hemorrhage
AbstractTo report our experience in treating one patient with nontraumatic subarachnoid hemorrhage (SAH) and concurrent acute ischemic stroke (AIS) due to large vessels occlusion (LVO). A man in his 50  s presented with acute right hemiparesis and aphasia. Brain CT showed a SAH in the left central sulcus; CT-angiography revealed a tandem occlusion of the left internal carotid artery and homolateral middle cerebral artery. He underwent an angiographic procedure with successful recanalization. Foll ow-up CT demonstrated a striatal-lenticular stroke without SAH progression. While the absolute contraindication to IVT during i...

Atherosclerotic Components in Thrombi Retrieved by Thrombectomy for Internal Carotid Artery Occlusion Due to Large Artery Atherosclerosis: A Case Report
Conclusions: Atherosclerotic components in retrieved thrombi might provide useful clues for diagnosing stroke pathogenesis. Further studies are warranted to clarify the utility of assessing atheromatous components in retrieved thrombi in diagnosing stroke etiology.

COVID-19–Associated Ischemic Stroke in a Patient on Therapeutic Anticoagulation
Conclusion: Ischemic stroke in anticoagulated patients with COVID-19 has been previously reported. Such cases emphasize the severity of the coronavirus virus associated hypercoagulable state. A majority of reported cases have occurred in patients continuing their ambulatory therapy. Overall, such cases are likely underreported. There are current trials comparing therapeutic versus prophylactic dose anticoagulation in patients with COVID-19. There are no studies specifically addressing anticoagulation agent failure in these patients. Further research is required this area to determine the optimal therapy for patients with COVID-19.

Bridging Therapy for Acute Stroke as the Initial Manifestation of Takayasu Arteritis: A Case Report and Review of Literature
We report a rare case of a patient with TA initially presenting with AIS treated with bridging therapy with full recovery of neurological function. Bridging therapy should be taken into consideration for AIS in patients with TA. Further study is needed in this regard.

Isolated acute pseudobulbar palsy with infarction of artery of percheron: case report and literature review
Conclusion: To the authors ’ knowledge, there are two cases of individuals with artery of Percheron infarction who devel- oped PBP associated with other clinical syndromes. Still, isolated PBP following infarction of Percheron’s artery was not reported. We hypothesized that the PBP may have occurred because of the existen ce of vascular territory variations in the perforating arteries that arise from the AOP.Keywords: Pseudobulbar palsy; thalamus; infarction.

Y-configuration double-stent-retriever thrombectomy for refractory thrombus in middle cerebral artery bifurcation: A case report
Rationale: Stent retriever mechanical thrombectomy is a recommended treatment for acute ischemic stroke. However, refractory thrombus in artery bifurcation can reduce the rate of successful revascularization. Patient concerns: A 72-year-old male, owing to the acute onset of almost complete right-sided hemiplegia and global aphasia, received bridging therapy. National Institutes of Health Stroke Scale score was 16 at the time of admission. Diagnoses: Cerebral digital subtraction angiography revealed occlusion of the M1 segment of the left MCA. Interventions: Thrombectomy with 3 passes of the Solitai...
