Concomitant Coronary Atheroma Regression and Stabilization in Response to Lipid-Lowering Therapy
CONCLUSIONS: Triple regression occurred in one third of AMI patients receiving high-intensity lipid-lowering therapy and was associated with alirocumab treatment, higher baseline lipid content and reduced cardiovascular events.PMID:37640248 | DOI:10.1016/j.jacc.2023.08.019

Recommendations for statin management in primary prevention: disparities among international risk scores
CONCLUSION: Risk algorithms differ substantially in their estimation of risk, recommendations for statin treatment, and use of ancillary testing, even in identical patients. These results highlight the limitations of currently used risk-based approaches for addressing lipid-specific risk in primary prevention.PMID:37638490 | DOI:10.1093/eurheartj/ehad539

Silent Myocardial Infarction: A Case Report
We present a case of a 59-year-old overweight woman with prediabetes, primary hypertension, and hypercholesterolemia who presented for herpes zoster (HZ) follow-up; she reported having skipped heartbeats and heart rate fluctuations during the review of systems. On further workup, ECG revealed low voltage QRS complexes, flat QRS complexes, flat T waves, and pathological Q waves, suggesting the diagnosis of SMI. Based on the identified risk factors, including high BMI, prediabetes, primary hypertension, hypercholesterolemia, HZ, and newly diagnosed SMI, the patient was advised to continue with lisinopril 20 mg daily, prescri...

Coronary Artery Calcium Scoring for Cardiovascular Risk Assessment in Patients with Inflammatory Bowel Disease
CONCLUSION: Subclinical atherosclerosis, as evaluated by CAC scoring, is prevalent in patients with IBD, and is associated with cardiovascular events. Further studies are needed to understand underlying biological processes of increased atherosclerotic disease risk among adults with IBD.PMID:37634654 | DOI:10.1016/j.ahj.2023.08.011

Atorvastatin reduces renal interstitial fibrosis caused by unilateral ureteral obstruction through inhibiting the transcriptional activity of YAP
In this study, we established a mouse model of obstructive kidney disease induced by unilateral ureteral obstruction (UUO). The UUO procedure significantly upregulated YAP and fibrosis-related gene expression in a time-dependent manner. Morphologically, the renal fibrotic lesions associated with hydronephrosis progressively worsened over time in the UUO group. Atorvastatin, which is widely used to lower blood cholesterol levels, has recently been shown to inhibit Yes1 associated protein (YAP). We treated UUO mice with atorvastatin for 3 and 10 days and observed a decrease in the expression of YAP and fibrosis-related genes...

Novel Anti-Inflammatory Therapies in Coronary Artery Disease and Acute Coronary Syndromes
Life (Basel). 2023 Jul 31;13(8):1669. doi: 10.3390/life13081669.ABSTRACTEvidence suggests that inflammation plays an important role in atherosclerosis and the consequent clinical presentation, including stable coronary artery disease (CAD) and acute coronary syndromes (ACS). The most essential elements are cytokines, proteins with hormone-like properties that are produced by the immune cells, endothelial cells, platelets, fibroblasts, and some stromal cells. Interleukins (IL-1β and IL-6), chemokines, interferon-γ (IFN-γ), and tumor necrosis factor-alpha (TNF-α) are the cytokines commonly associated with endothelial dys...

Acute Myocardial Infarction and Risk of Cognitive Impairment and Dementia: A Review
Biology (Basel). 2023 Aug 21;12(8):1154. doi: 10.3390/biology12081154.ABSTRACTCognitive impairment (CI) shares common cardiovascular risk factors with acute myocardial infarction (AMI), and is increasingly prevalent in our ageing population. Whilst AMI is associated with increased rates of CI, CI remains underreported and infrequently identified in patients with AMI. In this review, we discuss the evidence surrounding AMI and its links to dementia and CI, including pathophysiology, risk factors, management and interventions. Vascular dysregulation plays a major role in CI, with atherosclerosis, platelet activation, microin...

A Systematic Review and Meta-analysis on the Role of Statins in the Prevention of Mortality Following Pancreatic Cancer
CONCLUSION: Statins reduce the risk of mortality in patients with PC. However, no significant relation has been observed in RCTs. Therefore, it is necessary to be cautious in interpreting the results.PMID:37622694 | DOI:10.2174/1871520623666230824095226

Coronary CT angiography and coronary atherosclerosis : Where do we stand today?
Herz. 2023 Aug 25. doi: 10.1007/s00059-023-05207-0. Online ahead of print.ABSTRACTCardiac computed tomography (CT) has made substantial progress in recent years. The main field of application is CT coronary angiography for visualization of the coronary arteries and for the detection and exclusion of coronary artery stenosis. This has been included in international guidelines for the management of stable coronary artery disease or chronic coronary syndrome as well as for the diagnostic work-up of patients with acute chest pain; however, it must be taken into account that the diagnostic validity is only sufficiently high whe...

Acute hepatitis of unknown etiology in an adult female: A case report
CONCLUSION: Idiopathic hepatitis makes treatment challenging. It can leave patients feeling confused and unfulfilled. Thus, educating the patient thoroughly for shared decision-making and management becomes essential.PMID:37621598 | PMC:PMC10445075 | DOI:10.12998/wjcc.v11.i22.5288
