A case of emphysematous pyelonephritis in an older man with poorly controlled type 2 diabetes mellitus
We present the case of a 75-year-old man who was diagnosed early with EPN and received multidisciplinary treatment. He had poorly controlled type 2 diabetes mellitus (DM) and chronic kidney disease (CKD), and was treated with oral hypoglycemic drugs, including a sodium –glucose co-transporter-2 inhibitor. He experienced the onset of back pain in the midsection of his back, tenderness in the costovertebral angle, and a high fever (>  39 °C), accompanied by tachycardia, hypotension, and tachypnea. Cardiac Tamponade due to Purulent Pericarditis
A 65-year-old man was admitted to the hospital because of lower back pain that had persisted for 1 month. Pyogenic vertebral osteomyelitis was suspected based on elevated C-reactive protein (CRP) levels and CT findings of a compression fracture of the 12th thoracic vertebra. Empirical antimicrobial therapy was initiated after blood cultures were obtained. On the third day of treatment, his blood pressure rapidly decreased and respiratory failure developed, necessitating transfer to the intensive care unit.

Pediatric appendicular actinomycosis: a case report and literature review
CONCLUSIONS: Although appendicitis is very common in the general population, appendicitis associated with ACM is very rare, accounting for 0.02% - 0.06%, especially in the pediatric population. Diagnosis can be very challenging because they usually present with non-specific symptoms, and can form masses that mimic malignancies. Although rare, clinicians and pathologists should be aware of this entity. Satisfactory results and complete cure are achieved with adequate antibiotic therapy and surgery.
