Coronary computed tomography in emergencies: The importance of the radiologist's experience
CONCLUSION: The agreement among attending radiologists regarding the assessment of urgent coronary CTA studies is excellent. The agreement is lower when residents are paired with attending radiologists. These findings should be taken into consideration when implementing coronary CTA in emergency departments and in the organisation of radiological staff for interpreting and reporting this imaging test.PMID:37516483 | DOI:10.1016/j.rxeng.2023.07.002

A case report: anteroseptal ST elevation due to acute isolated right ventricular infarction
DiscussionRV myocardial infarction is usually due to an occlusion of the dominant RCA proximal to the origin of its RV wall branch, which often results in inferior ST elevation with reciprocal anterior ST depression. The ST elevation over V1 which would accompany RV infarction is often masked due to the more dominant electrical forces of inferior and posterior LV wall infarction. Our case demonstrates that in isolated RV infarction due to non-dominant proximal RCA occlusion, anterior ST elevation can be seen over V1-3, being most prominent in V1, which overlies the right ventricle, and resolved after restoring flow to the ...

Better safe than so ray: national survey of radiation protection amongst interventional radiology trainees in the United Kingdom
CONCLUSION: Provision of radiation PPE and dose monitoring for IR trainees is suboptimal, particularly access to adequate eye protection or suitably fitting leads. Based on the findings of this survey, recommendations have been made to promote the safety and radiation awareness of IR trainees.ADVANCES IN KNOWLEDGE: Radiation protection practices for IR trainees nationally are poor. Provision of suitable eye protection and well fitting lead body protection is low.PMID:37493155 | DOI:10.1259/bjr.20230071

Outpatient Vascular Care: Good, bad or ugly?
BY ANISH KOKA Filling in the holes of recent stories in the New York Times, and Propublica on the outpatient care of patients with peripheral arterial disease Most have gotten used to egregiously bad coverage of current events that fills the pages of today’s New York Times, but even by their now very low standards a recent telling of a story about peripheral artery disease was very bad. The scintillating allegation by Katie Thomas, Jessica Silver-Greenberg and Robert Gebeloff is that “medical device makers are bankrolling doctors to perform artery clearing procedures that can lead to amputations...

Accuracy and limitation of plaque detection by coronary CTA: a section-to-section comparison with optical coherence tomography
Sci Rep. 2023 Jul 22;13(1):11845. doi: 10.1038/s41598-023-38675-9.ABSTRACTPlaques identified by Coronary CT angiography (CCTA) are important in clinical diagnosis and primary prevention. High-risk plaque features by CCTA have been extensively validated using optical coherence tomography (OCT). However, since their general diagnostic performance and limitations have not been fully investigated, we sought to compare CCTA with OCT among consecutive vessel sections. We retrospectively compared 188 consecutive plaques and 84 normal sections in 41 vessels from 40 consecutive patients referred for chest pain evaluation who had bo...

Anti ‐TNFα induced lupus due to infliximab therapy in a patient with concurrent Crohn's disease
In this report, we present an interesting case of ATIL that developed rather gradually upon anti-TNFa used to treat Crohn's disease. The patient presented with oral ulcers, photosensitive rash, diffuse alopecia, inflammation of the hands, and pleuritic chest pain. Comprehensive serological testing revealed the presence of antinuclear antibodies and anti-DNA antibodies. During the evaluation, the patient developed headaches, followed by a brain MRI that suggested nonspecific white matter lesions. Given the chronic development of symptoms, invasive examinations such as an arteriogram were performed to exclude CNS vasculitis,...

Peripheral Arterial Disease: A Narrative Review
Cureus. 2023 Jun 11;15(6):e40267. doi: 10.7759/cureus.40267. eCollection 2023 Jun.ABSTRACTPeripheral arterial disease (PAD) describes the partial or complete occlusion of blood flow in the distal arteries of the body. A decreased arterial patency may occur due to a reduction in the elasticity or diameter of the vessel. The goal of interventions is to decrease incidence and reduce complications by identifying and minimizing the primary causes. This paper discusses PAD affecting the aortoiliac, common femoral, and femoropopliteal arteries. In a significant portion of the population, PAD may lack usual symptoms such as limb p...

Importance of lead aVL in the diagnosis of inferior wall myocardial infarction: A case report
Malays Fam Physician. 2023 Jun 20;18:38. doi: 10.51866/cr.370. eCollection 2023.ABSTRACTIschaemic heart disorders are among the leading causes of mortality worldwide. There has been a growing occurrence of heart disease among young adults. Thus, acute myocardial infarction (MI) should be considered in all patients with central chest pain. Herein, we report the case of a young, fit, active smoker with underlying dyslipidaemia presenting with acute MI, characterised by dynamic changes in lead aVL wherein T wave flattening progressed to inversion, suggestive of early reciprocal changes. Soon after, electrocardiogram (ECG) rev...
